Purpose of the post:

To provide specialised, holistic support to young people between the age of 18 and 25 who have experienced domestic or sexual abuse, harm and violence, and are being supported within Community services in Torfaen.

We are looking for a compassionate, reflective, and person-centred individual to join our team in delivering trauma-informed support to adults and families. This role is about more than providing a service—it’s about striving for people to feel safer, gain more control, and have choice over decisions about their future.

You’ll work collaboratively to create environments where individuals feel seen, heard, and respected, and where their experiences and strengths shape the support, they receive.

Specific responsibilities

• Collaborate with individuals and team members to provide trauma informed support to a caseload of young people within community settings.
• Assess and manage risk sensitively and collaboratively, promoting safety through strengths-based approaches that recognise each person’s lived experience.
• Co-develop personalised support and/or safety plans with individuals, ensuring goals are meaningful, achievable, and shaped by the person’s own priorities and values.

Purpose of the post  

To assist in the management and delivery of high quality, responsive, specialist refuge support and services that enable people to feel safe, to be empowered, and to flourish in a life free from Domestic Abuse and Sexual violence

Responsibilities

• As a key member of the team, contribute to achieving Cyfannol Women’s Aid’s vision, mission and ambition.
• To demonstrate and positively promote the organisation’s values, culture, and qualities
• Collaborate with the support services coordinator and the refuge team to plan and deliver high quality, specialised, trauma informed, person-led support services.
• Line management of refuge team members (Support Worker, Support Work Assistants, Out Of Hours and Relief Workers) in line with policy in an inclusive, engaging and empowering way. (including supporting recruitment, completing induction, supervision, training and development) supporting and coaching.
• Case hold, providing direct support to adults and children staying in Blaenau Gwent Refuge Services.
• Oversee all rotas for refuge staff and on-call for Blaenau Gwent and make sure the refuges are adequately staffed at all times.
• Work collaboratively with the support services coordinator to ensure refuge referral paths are easily accessible are clear and concise, and to continue to promote a whole team approach.
• Support women and children to have a voice and provide feedback within the organisation and the sector, including surveys, listening sessions, and external opportunities
• Oversee operational Health & Safety requirements within refuge services and to oversee housekeeping, maximise refuge occupancy.
• Key collaborator, supporting the team members to keep accurate and timely records in all aspects of the service.
• Monitor personal charges and petty cash liaising with the Finance Department, Support Services Coordinator and Service Manager.
• Lead regular House Meetings and refuge Team meetings and to participate in Blaenau Gwent full team meetings
• Support the coordinator in compiling reports for Welsh Women’s Aid (WWA) Statistics and other relevant reports for projects and services
• Assist with the attendance of core group and other similar meetings, ensuring completion of all tasks allocated to Cyfannol Women’s Aid (CWA) where coordinator is unable to.
• Support the refuge team to appropriately signpost and refer women and children to the services they wish to engage with for example social services, the police, parenting, health, legal, DWP, housing, homelessness, recreational, educational, and cultural services and also CWA’s other internal services.
• Being responsive to requests from other members of the organisation for quantitative and qualitative data, including for example, the creation of case studies.

About the Role

· Located at a 24-hour supported housing service in Clapham, we provide high quality self-contained accommodation for 15 women who have experienced homelessness and multiple disadvantage.  The aim of the service is to provide a therapeutic environment in which women feel safe and can flourish, enabling them to move on into independent, permanent accommodation. The accommodation also includes overnight safe spaces.

· In this role, you will provide direct support to all the women, as well as holding a small case load of residents who you will keywork.

· You will work closely with support workers and the in-house psychologist. You will model person-led, psychological, gender, and culturally informed support, will be skilled at building trust and you will be a great team player.

· You will undertake 8-hour shifts, normally Monday to Friday but with flexibility for weekend working, covering the hours 8am to 8pm. Please be aware that the building has several floors and no lifts.

Based across two properties, you and your team will be supporting ten adults with moderate to severe learning disabilities, as well as additional mental and physical health needs.

You will play a pivotal ambassadorial role, continuing Centre’s 404 service expansion into the London Brough of Brent and bringing our expertise, vision and values to families in need.

This varied and dynamic role will lead on the set up, coordination and provision of excellent person-centred care and support to service users, enabling them to live independently in their home and be supported in all aspects of day-to-day living. As Project Manager, you will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet individual needs. You will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of support workers.

The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage. Role and Responsibilities

Housing and Service Delivery

• Provide strong and effective support to residents in Acorn House that focuses on their strengths and goals, and is delivered in a personalised way and within a framework of active engagement and co-production

• Developing and reviewing support plans that reflect the interests and aspirations of residents

• Induct new residents in line with the service’s policies and procedures and support them to continue in their journey towards recovery from addiction

• Carry out assessments of those referred to Acorn House, and induct new residents

• Ensure the fundamental needs of residents are met including good quality accommodation and facilities, healthy food, advice and guidance around benefits, and alcohol and drug testing (to help maintain an abstinent environment)

• Deal with the immediate support needs of residents as appropriate, providing crisis intervention where necessary

• Support residents in addressing physical and mental health issues, including liaison with primary and secondary health teams

• Ensure residents are supported through court or welfare rulings (if necessary)

• Ensure effective and compliant administration of resident data using a bespoke Salesforce- based platform (In-Form)

• Ensure Housing Benefit and Services Charges are collected and managed effectively 

• Be flexible and responsive to the changing needs of the service, ensuring high quality support is provided

• Ensure each resident’s support plan is written, monitored and reviewed as appropriate to reflect their changing needs and development

• Ensure residents progress and engage with other SCT programmes (when required)

• Ensure conditions and requirements relating to any contracts or management agreements are applied throughout SCT’s services

• Work effectively as part of a team, attending team/ staff meetings and making a positive contribution to the development of the team and wider SCT services

• Effectively control and manage financial records appropriate to the duties of the post (petty cash/rent) 

• Participate in a feedback culture for continuous improvement

• Promote social inclusion, supporting residents to access all benefits available to them

• Support residents with physical and mental health issues, including liaison primary and secondary health care services as required

• Ensure all repairs and maintenance needs are raised

Monitoring and Evaluation

• Maintain accurate records of residents on In-Form

Operational Support

• Comply with Health and Safety, including but not limited to any fire maintenance, risk assessments and complete room checks and raise any risks to the Supported Housing Manager

• Ensure equipment of all applicable facilities is in good working order

• To provide an on-call/out-of-hours service when required

• Carry out other duties as may reasonably be required 

Special Conditions

• This post requires the holder to work varying shifts between 08.00 am and 8.00 pm and participation in the call rota system (for which an additional allowance is paid)

• The post-holder is required to work weekends as part of a rota and Bank Holidays when required

• On occasions you may be requested to change your rota to ensure the requirements of the service are covered

We are looking for a Housing Mobile Night Worker to provide floating support to our teams in high support 24-hour projects as well as medium support and transitional housing units to ensure that our properties are safe, secure, and welcoming through the night.

Your shift will commence at Eastbourne Foyer and will support the static night worker there during the night as well as undertaking your mobile duties across Eastbourne, Hailsham, Bexhill and Hastings.

Working on site you will be responsible for ensuring the service is safe by keeping clear records of night time activity within the building. You will regularly complete building checks and monitor CCTV upkeeping health and safety regulations in accordance with YMCA DLG's policies and procedures.  You will work to prevent any anti-social behaviour from impacting our neighbours and other residents. You will work alongside other support staff to maximise the wellbeing of residents by completing one to one interventions with young people who may be experiencing mental health crisis and/or who may need medical assistance.

You will also be lone working as part of the role, so you will need to be comfortable with decision making and following procedures.

You will already have clear verbal and written communication skills, good IT and keyboard skills and the ability to self-motivate. You will be an effective communicator who is able to deliver robust, clear handovers and build strong relationships with other members of your team. In addition, you will have the ability to de-escalate volatile situations and manage challenging behaviour appropriately.

About the Role

As Young People Case Worker you will work under the direction, guidance and support of a Service Manager and an Assistant Area Director, you will be responsible for a caseload of clients as well as share responsibility for the safety and wellbeing of all clients, volunteers and staff in the service.

The service consists of two hubs, 1 in Bishop Auckland and 1 in Tow Law and we have an Outreach service to 7 dispersed properties in Bishop Auckland. Your role will span all three areas for job rotation and continuity of service.

The team follow a 5-week shift pattern of 8-hour shifts between 0730 – 2200 hrs including weekends and bank holidays as one service is 24/7 and both sites have dedicated night staff.

Early shifts 0730-1530, Late shifts 1400-2200 and Main shifts 0900-1700/1000-1800 (Depending on hub)

The accommodation services are based in Bishop Auckland and Tow Law in County Durham. You will ensure that the accommodation is properly maintained and support the client to sustain their tenancy and participate in positive and successful move on. You will be responsible for a caseload of clients as well as shared responsibility for the safety and wellbeing of all clients, volunteers and staff in the service. This includes outreach support to clients in dispersed properties in the area.

The role does involve lone working.

Key responsibilities include:

· Ensure the safety and wellbeing of all clients, following safeguarding and emergency procedures as needed.

· Act as key worker for a caseload of clients, leading on risk and needs assessments and SMART, outcomes-based support plans.

· Provide personalised, flexible, and creative support, involving clients in decision-making and encouraging participation in activities.

· Collaborate with external agencies to support clients, adhering to data protection and information sharing protocols.

It comprises 11 self-contained flats offering accommodation to both male and female residents. As a low-need service, the role involves supporting customers in developing essential life skills.
