You will deliver predominantly one to one wellbeing coaching with some group support to both adults and young people. Ideally you will have some experience of using coaching and CBT techniques such as guided self-help, psychoeducation and thought challenging.

This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.

The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.  

You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.

Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. Experience of working in mental health or dementia services would be an advantage.

You should be an excellent communicator and able to motivate and empower others. This is a full-time role.

The Women’s Gambling Link Worker will deliver an awareness-raising and personalised, holistic support programme for women in Lambeth experiencing gambling and gambling addiction-related harm, or those impacted by the gambling of a close family member. You will:

• Deliver an outreach programme by regularly attending community events, visiting local groups/ spaces and organisations frequented by women in Lambeth.

• Develop partnerships and deliver outreach support to primary and secondary care partners and services across Lambeth.

• Speak with women's groups in Lambeth, either 1-2-1 or presenting to groups, to discuss the type of support available and promote the work and support of the Primary Care Gambling Service and Age UK Lambeth services. 

• Provide holistic support to patients referred to the Primary Care Gambling Service, helping to address housing, debt, bereavement, loneliness, mental health and other related social issues, that are both a symptom and/or a cause of their gambling.

• Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss ‘what matters to me’.

Ruils is a user-led charity that supports disabled adults and people with long-term health and/or mental health conditions to live independently, be part of their community and live life to the full. About the Service

Ella House is an Accomodation Service that provides support to women who have experience Rough Sleeping, Trauma and Multiple Disadvantage.  The aim of the project is to provide support to women for around 18 months to build skills to be able to live independently. 

The service is a 24 hour service that runs on a shift pattern over the week and has a concierge service covering nights.  Staff will provide holistic support to service users looking at all their needs and putting a robust plan in place.

About the Role

The Activities and Wellbeing worker will provide both 121 and group sessions. Speaking with women to understand their needs and wants and developing appropriate session plans.  These can be wellbeing activities, skills based  or creative depending on what the women require.  There will be a small budget to accommodate activities.

About You

The ideal candidate will be passionate about working with vulnerable people from a strengths based approach.  An understanding of Trauma and how to work in psychologiacally informed environments is essential in this role.  Someone who is motivated and creative and able to build relationships with women and get them to engage in a variety of activities.

Dementia Wellbeing Worker 

Age UK Camden Dementia Wellbeing Service is looking to recruit a Dementia Wellbeing Worker to join their team working 35 hours per week on a permanent basis. 

The Dementia Wellbeing Service is part of our specialist dementia services and works closely with the Dementia Befriending Service.

The service supports Camden residents over the age of 55 in the early or moderate stages of dementia, to live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love.

• Offer a holistic and person-centered assessment for people living with mild cognitive impairment or Dementia to ensure meaningful interventions.

• Manage a case load of clients, providing practical and emotional support to the most isolated and vulnerable in our community.

Snow Camp is a ground-breaking national youth charity, and we are looking to recruit a fantastic new North West Wellbeing Manager and Programme Support to join our North West team on a 4 day a week contract (30 hrs).

Our accredited programmes are unique and attractive to young people, taking them from beginner to qualified snowsports instructors in just one year with volunteering and apprenticeship progression routes available at the end. Alongside learning to ski or snowboard, young people also have access to wellbeing support with the emphasis on improving young people’s mental health and wellbeing.

You will report to the North West Programme Manager & Head of Snowsports and will be responsible for the delivery of our wellbeing workshops and providing 1to1 wellbeing support sessions to young people. You will also work as part of the North West team to assist with the planning and delivery of snowsports and life-skills focused youth activities run by the Programme Manager and assist with the apprenticeship programme throughout each year.

Experience of working successfully with under-served young people in a range of settings, with a focus on supporting young people’s wellbeing will be essential to the post. Passion for snowsports and supporting young people generally will be a huge advantage!

Youth work, wellbeing support experience, project management, logistical skills, managing relationships, communication and presentation skills will make up the role. Along with experience of delivering group workshops to help young people work towards meaningful goals to improve their emotional wellbeing.

Job Description

• You will spend roughly 2.5 days will be in the Wellbeing Manager role and 1.5 days will be in the Programme Support role each week.
• In consultation with the Programme Manager, to manage the delivery of the Wellbeing Workshops and 1to1 mentoring support needed for young people and apprentices.
• Work with the programme manager to gain the young people’s trust and confidence.
• To assist with delivery and development of The North West’s full snowsports focused journey of youth programmes which take place at Chill Factore.
• Build a good relationship with the North West Programme Manager to help provide support and guidance to young people.
• Support with the management of (currently) 3 x North West apprentices on their 1-year Snow Camp Apprenticeship programme. particularly helping to manage their wellbeing and signpost to other services where needed.
• Ensuring the apprentices have clear personal development plans and objectives.
• Provide effective and inspiring leadership to apprentices and young people.
• Providing weekly 1to1 support sessions for each apprentice.
• Having an understanding and an involvement in the development of their weekly working rotas.
• Supporting with NVQ work & assisting with any challenges that may be faced.
• Ensuring apprentices are involved and supported working on all Snow Camp programmes as the North West staff team.
• Complete Personal Development Plans and Wellbeing Assessments as required with apprentices.
• Attend regular meetings with the other regional Wellbeing Managers to provide a consistent approach and building resources focused on improving young people’s wellbeing.
• Outside of programme days, provide one-to-one mentoring sessions to young people, in-person or remotely, focusing on their well-being and offering support so they can cope with the Snow Camp programme and improve their general wellbeing.
• Be a key team member on the overseas residential for the Excel programme during the Easter holidays. *Valid passport for travel in Europe & Andorra required.
• Use our UPSHOT online database system to keep records and ensure all monitoring and evaluation is carried out.
• Attend external training as required (e.g. Safeguarding and Mental Health Awareness tutor training courses).
• Ensure that all activities and provision you deliver is done safely and in line with organisational policies.
• Support the Programme Manager to ensure all programmes run smoothly and effectively by supporting the delivery of each programme – managing venue instructors, guest speakers, apprentices, youth staff, sessional staff and volunteers as required.
• Support the Programme Manager with recruiting young people for each programme through visits to youth projects, promotion through email to all relevant youth programmes and services and presenting at key stakeholder meetings on youth service provision. Finding new young people in the North West area to support by Snow-Camp will be key.
• Support the Programme Manager with programme administration including finances, SLAs, attendance records, risk assessments, bookings and young people’s records on Upshot.
• Support with management and delivery of trips and overseas residential’s (Snow-Camp Excel, Apprenticeship & Youth Forum Residential’s) as part of Snow-Camp programmes. Including support with all the necessary risk assessments.
• Additional tasks may vary depending on different opportunities throughout the year.

Personal Specification

• Sound knowledge of MS Office tools including Word, Excel, PowerPoint and Outlook.

Management and support

The North West Wellbeing Manager & Programme Support will report to the North West Programme Manager & Head of Snowsports Morv Bett who will provide support and guidance.

The Trustee board will determine the overall direction of Snow Camp and will set the overall priorities in consultation with the Director and Senior Team.

The Junction is an out of hours “calming café” service which is open 365 days of the year to support people experiencing pre and/or mental health crisis. The aim of the project is to alleviate the need for people within the Swindon locality to access A&E and/or acute mental health services.

As a Wellbeing Practitioner, you will be providing a preventative, person centred approach to people accessing the service who may be experiencing a mental health crisis.

What you’ll be doing? 

You will be part of the MYcommunity Linkworkers team. Your main role will be to:

• Provide personalised brief intervention and social prescribing support to individuals, their families and carers.

• Meet clients on a one-to-one basis, making home visits where appropriate and allowing clients the time and space to discuss ‘what matters to me.

• Provide person-centred support to enable clients to identify their health and wellbeing needs and goals. Co-produce support plans with clients to discuss what’s important, where they would like to be in the future and how we can support them to achieve this. 

• Manage and prioritise a caseload to ensure clients' needs, priorities and urgent support are met.

• MYcommunity Lambeth works in partnership with a large number of statutory and community services and organisations across the borough. You will link clients with appropriate services to meet their needs. It is vital to have a strong understanding of when it is necessary to refer someone back to health care professionals/ agencies when the client’s needs are beyond the scope of the MYcommunity team.

We have an exciting new opportunity for a Social Prescribing Link Worker to join our team in Merton.

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing. 

You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.
