Support Workers will provide support to customers to help them develop the life skills they require to meet the assessed needs of the customers of the service. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices.

What you'll do:

Look Ahead Support Workers have the personal qualities required to manage these customer expectations alongside the responsibilities of their role. Day to day Support Worker activities include:

Building supportive, trusting relationships with customers and creating a positive atmosphere.

As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.

As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.

You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.

There will be a rota for shifts, with working patterns of 7:30-15:30 and 12:30-20:30. There will be some weekend work.

What you'll do:

Work proactively with Slough Hostel team to handle the service caseload and support an effective team approach to meeting each customer's identified needs.
Motivate the team and champion a positive local culture within the service.
Contribute to a positive service environment, ensuring the service is supportive and a place of safety and stability for customers. This includes working flexibly and being proactively in delivering, reviewing and arranging effective support for emerging needs and risks in order to prevent escalating concerns.
Deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. This may include practical tasks, such a facilitating group actives
Support customers to undertake domestic tasks, including practical assistance to ensure a high quality living environment that promotes customers' dignity, respect and allows customers to feel safe and welcome in their own home.
Participate in support planning, risk management and assessment of needs in line with support planning.
Work with customers to promote social inclusion to promote independent living, encouraging customers to maximise their skills and choices.
Involve customers in the design, development and delivery of the service through co-production.
Report and react to any observations relating to customers welfare.
Proactive development of links with local statutory and voluntary organizations to provide a range of engagement opportunities for customers in the community.

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.
This is a great opportunity for any support worker to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT) We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.

The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.

The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.

The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. Working in partnership with the London Borough of Newham and East London Foundation Trust, IBIS House supports people who are medically fit for discharge from mental health inpatient stays but have other barriers to discharge. These may include housing, living skills, accessing benefits or the community, or other interventions and will be on a person-centred basis. The main aim of the service is to provide short-term support to customers to link with longer-term services to reduce escalations in need, and reduce inpatient readmission and reduce pressure on inpatient beds.

Waking Night Support Workers are responsible for ensuring continuity of this support service at night and for keeping service-users and the building safe. More specifically, you will:

Understand the concept of recovery and apply those principles to develop an environment and service that enhances recovery.
Put the customer at the centre of their support and promote optimism and hope for their increased independence
Provide intensive support (both practical and emotional) to customers as required

About the role:

This role is home working based with travel in the London Area.

As a Homicide Service Children's and Young People caseworker you will be:

• Delivering a specialist service to children and young people (CYP) bereaved by homicide, and for those CYP who have witnessed a homicide.
• Working collaboratively and jointly within the Homicide Service and other relevant organisations that can help to progress identified needs and ensure that young people are central to all processes that involve them.
• Acting as the designated caseworker and manage a caseload
• Implementing the CYP Homicide Service Delivery Model
• Work closely and support colleagues to provide a trauma informed service to children and young people.

You will be responsible for providing a high-quality, front-line service to bereaved young people following murder or manslaughter. Teresa House and Hamlets Way are two supported housing accommodations for adults with enduring mental health issues. Both services provide a high level of support to 16 customers on each service. Teresa House is a fast stream service where customers are supported up to two years while Hamlets Way is a slow stream service where customers can expect support for a minimum of two to a maximum of 4 years.
The service is under Look Ahead management since April 2015 and the buildings are owed by Notting Hill Genesis. As a support worker you will play an integral role in developing effective support and risk management plans. You will also support customers with aspects of their day-to-day living including but not excluded to; social and physical activities, medication, benefit entitlements, maintaining living standards, exploring employment, education and training opportunities.

This role requires a valid Right to Work for the UK as Look Ahead are not a sponsoring organisation.

- To work on a rota based shift pattern which includes working early (8-4pm) and late (2-10pm) shifts 5 days a week.

- Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
