StreetLink London is a bespoke assessment and response telephone service for people who are rough sleeping in London, supported by volunteers. Building on St Mungo’s extensive experience of supporting people sleeping rough through a range of face-to-face services, this innovative service aims to reduce the incidences of subsequent rough sleeping amongst people who have not slept rough before and increase the speed with which those who have a history of rough sleeping can access support and accommodation.
As an Assessment and Response Worker you will:

Answer phone calls from people experiencing homelessness in London and deliver a high quality of customer service

Triage online referrals from the website and the app, to connect people who are rough sleeping with local outreach teams as quickly as possible.

Carry out comprehensive telephone assessments, identifying immediate options for accommodation and support while confidently responding to emergency situations and safeguarding concerns.

Advocate on behalf of people with relevant statutory and non-statutory services to ensure clients can access the support and accommodation they are entitled to as quickly as possible.

Train, motivate and support volunteers so they can provide a high quality, responsive service to everyone contacting StreetLink London.

The Specialist Multiple Disadvantage Worker will have skills in effectively liaising and building good relationships, with external and internal stakeholders to keep a satisfactory level of appropriate referrals.
About you:

Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
You will:
- Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
- Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this
- Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation

As a Resettlement Worker and Domestic Abuse Specialist, you will work in person and remotely with clients who have support needs, in particular, those who are survivors of domestic abuse. In conjunction with other members of the Partnership, you will support our clients to engage with services and find accommodation solutions. You will work in a person-centered way to get the best prevention, relief and support outcomes for the client.
Main duties:
- To work with clients with support needs, to engage with their prevention, relief or advice goals by; Agreeing with them a support plan to deliver the actions identified in the Personalised Housing Plan.
