Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. We also work with several agencies and organisations in the local area to meet individual customers' needs. We support customers to build confidence in order to empower them to use community resources; we promote healthy lifestyles and wellbeing and offer customers choice and control over the activities they would like to participate in.
The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.
The Post holder will be expected to work on a rota basis (Monday - Sunday) and two sleep-ins per week which attracts £30.00 extra pay.The shift patterns are 8.00 am - 4.00pm, 2.00 pm - 10.00 pm and sleep ins.

Build supportive, trusting relationships with customers and creating a positive atmosphere
Undertake key-working responsibilities for a caseload assigned by the Contract Manager
Provide customer medication support and ensuring that medication procedures are followed
Conduct regular key work sessions that are innovative and engaging in order to achieve Support Plan goals
Ensure ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
Provide support with daily living activities, as well as practical assistance to support customers to independent living e.g. budgeting, cooking, community activities
Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions.
Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.

About the Role:
Oaklands & St Kilda is a pair of 6-unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.
The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. About this role…
Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
You will:
- Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
- Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this
- Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation
- Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation
- Accurately record all work completed within given timeframes
To succeed as the Lead Worker, you will have:
- A strong commitment to providing a high standard of service
- Experience working with people who have rough slept or vulnerable adults and supporting them to access services
- Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently
- Good communication skills and confidence in public speaking when representing Thames Reach externally
- Experience in forming strong working relationships with partner agencies, being able to challenge them when required
- Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone
