What you'll do:
* Undertake key-working responsibilities for a caseload assigned by the Manager
* Provide support 9am-5pm 7 days a week
* Assess prospective discharges within 3-5 hours of receiving referral information.
* Support the customer to settle in and meet their immediate social care needs.
* Update the Discharge/Patient Flow Team on patient progress on a weekly basis.
* Escalate concerns to the Discharge Team and urgent concerns via the Crisis Line.
* Proactively monitor customer's mental state, ADLs and move-on plans.
* Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions.
* Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
* Support customers to access activities and services in order to maintain and improve well-being, access benefits and reduce isolation.
* Maintain regular communication with Hackney Council (Local Authority) to facilitate re-housing.
* Develop and maintain links with all key agencies and service providers in the local community.
* Empower customers to ensure they receive the service and benefits they are entitled to
* Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards.
* Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.
* Adhere to Look Ahead's Policies and Procedures
* Engage in learning and development activity to increase knowledge and skills.
* Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned

Creative Support is looking for warm, caring, and proactive Relief Support Workers to provide person-centred care at our respite service in St Helens. The service provides planned respite care for adults aged 18 and over with learning disabilities, mental health needs, and physical and sensory disabilities.
Thyme Lodge, a modern detached house in Clockface, offers a relaxed, homely environment with 5 en-suite bedrooms, a communal lounge, conservatory, sensory room, and an open-plan kitchen and diner. As a Support Worker, you will provide personalised care, helping service users develop friendships and learn new skills. Activities include day trips to Blackpool, local safari parks, and picnics, as well as annual events such as summer fairs, BBQs, and Christmas parties.
Your duties will include developing trusting relationships with service users, carrying out administrative and housing management tasks, and supporting individuals with domestic tasks and daily living skills. While some shifts may involve lone working, we generally work as part of a small team with on-site management support.
A minimum of twelve months of care experience is essential, and a passion for providing person-centred care in a friendly environment is key.
The service is within walking distance of Lea Green train station, with several bus routes nearby and on-site parking available. A valid driver’s license is essential.
As a Creative Support Bank Staff member, you will have access to our free app, allowing you to pick up shifts nationwide and providing service information, including directions. You will be paid weekly.

We are looking for compassionate individuals to join our dedicated supported living team. If you would like a rewarding role supporting individuals who have learning disabilities and autism to achieve their life goals and join the local community then we would love to hear from you.
Your Support Worker role would include:
- Supporting service users inside their home and in the Hartlepool community
- Supporting service users to retain their independence
- Engaging in activities with the tenants using both verbal and non-verbal communication
- Collaborating with service users, family members and other involved professionals to provide excellent person-centred care for service users
- Working as part of a team and independently across our supported living services
- Administering medication, providing respectful personal care and helping service users retain their dignity at all times
- Some service administration tasks which will require basic computer knowledge

Experience of working with people who have learning disabilities and/or autism is beneficial but not essential for this Support Worker role as you will be provided with full training for the role, with excellent staff teams that support each other across the Hartlepool services. A car driver is preferred for this role but not essential as the services are located within walking distance of each other and are on a good bus route.
You will be supporting service users in their own homes. Our service users enjoy a variety of activities, such as going for drives, movie nights and going to the pub! You will support our service users to access the community, maintain housework and live an enjoyable and independent life.

About the Service
Ella House is an Accommodation Service that provides support to women who have experience Rough Sleeping, Trauma and Multiple Disadvantage. The aim of the project is to provide support to women for around 18 months to build skills to be able to live independently.
The service is a 24 hour service that runs on a shift pattern over the week and has a concierge service covering nights. Staff will provide holistic support to service users looking at all their needs and putting a robust plan in place.
About the Role
The Activities and Wellbeing worker will provide both 121 and group sessions. Speaking with women to understand their needs and wants and developing appropriate session plans. These can be wellbeing activities, skills based or creative depending on what the women require. There will be a small budget to accommodate activities.
About You
The ideal candidate will be passionate about working with vulnerable people from a strengths based approach. An understanding of Trauma and how to work in psychologiacally informed environments is essential in this role. Someone who is motivated and creative and able to build relationships with women and get them to engage in a variety of activities.
