Hospitals need to free up beds by discharging patients who do not need to remain in hospital. Despite the pressures we want discharges to be safe, successful and as painless as possible for patients. We also want to ensure a patient is not readmitted into hospital. 

The Hospital Discharge Community Support Coordinator will match Age UK Lambeth (AUKL) volunteers and partner organisation volunteers to support a patient's discharge. Volunteers could provide support with transport, practical support such as food shopping or completing home safety checks. Upon discharge volunteers may continue supporting patients for up to 4 weeks, providing support in person (at the individual's home or in the community) and over the phone, linking them into support services and helping them access the community to prevent hospital readmission.

What you’ll be doing?

1. Oversee the development of a new, inclusive volunteer programme that offers a range of diverse and challenging volunteer roles to support patients leaving hospital and once settled home. 

2. Work in partnership with the VCS to create 100 volunteering opportunities to support hospital discharge.

3. Using the volunteer platform, Care for Me, track allocated tasks to ensure they’re completed. Monitor volunteer reports to review the support and work delivered under the project.

4. Develop and maintain strong working relationships with King’s College Hospital and Guy’s and St Thomas’ Hospital to ensure appropriate referrals are made into the service.

We are looking for an experienced commercial and visitor experience professional to join our friendly team in the new post of Head of Commercial and Visitor Experience, reporting into our trading board, Crystal Palace Park Events Ltd (CPPEL), and line managed by the Director of Park Management.

Responsible for achieving ambitious income targets, you will lead the  management, development and growth of key revenue generating activities including concessions, leases, retail, licensing, catering, markets and car parking activity (and excluding events and venue hire). Alongside this you will oversee the smooth running of the park as a visitor attraction, ensuring an excellent visitor experience and an accessible, warm welcome to all. 

You will be a creative and entrepreneurial team player with experience working in a visitor attraction, cultural, heritage or park setting and a proven track record in business development and income generation.

In this role, you will be managed by Laura Mertsching. Laura joined ClientEarth in 2022 and is based in London. For 2.5 years, Laura has been focussed on improving our policies, processes and tools to improve funder compliance and efficiency, and working in collaboration especially with fundraisers, internal legal council and finance staff. At the beginning of 2025, Laura was seconded to lead a cross-departmental project to align internal systems, processes and tools, which is why she is hiring a support role at the moment to drive forward key initiatives of her original role. Laura is an experienced project manager with experience working in the humanitarian and international development sector for more than five years before joining ClientEarth.

Main Duties

• Contributing to the delivery of the global Fundraising Strategy through driving improvements of relevant policies, processes and tools along the entire funding management cycle ensuring effective and efficient internal operations
• Effectively plan and deliver priority initiatives (including policy revision, risk matrix tool development, streamlining corporate income processes) collaborating with stakeholders across the organisation to ensure that relevant perspectives are incorporated and work with their line manager on effective prioritisation
• Enable effective decision-making across ClientEarth’s existing and continuously growing funding portfolio, and occasionally review new funding agreements and advise staff during the contracting process

Sound Minds is a charity, led by mental health service users, channelling the power of creativity and peer support to improve the lives of people living with long term mental ill health. Canerows offers peer support to those who are in hospital and those living in the community.

Main purposes of the job:

Sound Minds and Canerows are delivering an exciting project which is about offering peer support across Wandsworth to people who are experiencing mental health difficulties and living in the community. We are working in an alliance with Mushkil Aasaan and Wandsworth Carers Centre.

Mental Health Peer Support gives emotional support and promotes access to information and practical advice to people experiencing mental health difficulties. A key component is that these staff use their own direct lived experience of mental health difficulties to support other people. They work collaboratively with people within their service, focusing on the person’s strengths and personal resources to offer educational, emotional and practical support towards goals that are defined by the person.

The Peer Support Worker will be supported through peer support training and regular supervision.

Salary: £25,875 per annum FTE & 3% contribution to pension

Hours: 17.5 hours per week

Lead Disability Business Partner

Business Disability Forum is the leading business membership organisation in disability inclusion.

The role

To engage with and develop productive working relationships with the BDF membership helping to further their disability agenda over time using our knowledge, member services and paid for products where appropriate. The Lead Disability Business Partner has direct responsibility for their assigned Partners and indirect responsibility for the Partners managed by their direct reports plus overall responsibility for work allocation of the Senior Business Partner Team.

The Lead Disability Business Partner will be responsible for managing the workflow for the Senor Business Partners (SBP’s) allocating and quality assuring consultancy and Partner work as required.

The requirement

• Experience of providing business support/coaching to experienced professionals (A, I & T).
• Experience of providing consultancy, training and advice on disability related issues (A, I & T).
• Experience of line managing a sales/customer service team, a consultancy team would be an advantage (A & I)
• Experience of consultancy service business development (A, I & T).
• Knowledge of disability legislation and application in the workplace (A, I & T)

The role holder will be expected to have a knowledge of EDI legislation across the UK.
