Location: Home based - within a commutable distance of London as there will be a requirement for fortnightly travel to the Chelsea & Westminster Clinic and with additional occasional travel across England, Wales and Northern Ireland. 

Shine’s Health Team 

Shine’s Health Team provides condition-specific health advice and support to people of all ages with spina bifida, hydrocephalus, NPH or IIH, promoting good self-management and enabling our members to live in optimum health.  The Team also aims to reduce the numbers of pregnancies affected by spina bifida and anencephaly through education around supplementation and fortification with folic acid, and other measures. 

Purpose of Job 

The Shine Nurse Specialist will deliver services and create resources to improve the health of Shine’s members, by informing members, their carers and professionals 

Principal Duties 

• To provide ad hoc advice to Shine’s members, their carers  and professionals on complex health issues, and signpost to NHS services 

• To attend and represent Shine at the Chelsea & Westminster Spina Bifida Clinic

• To collect information, statistics and research from a variety of sources to inform Shine’s health projects 

• To create resources to inform and support members’ health 

• To develop training/information resources for professionals 

• To plan and deliver projects, events, presentations and information sessions for members and professionals 

• To provide an information resource for Shine staff 

• To form and engage with networks of health professionals 

• To represent Shine at events and meetings 

• To work closely with the Health Research & Information Coordinator to produce and disseminate health information to Shine members and the general public 

Skills and Experience 

Essential 

• Professional/Registered Health Qualification, for example:

  • Adult Nursing 

  • Registered General Nurse 

• Experience of searching scientific and medical literature, collating information and writing reports 

• Excellent communication skills, written and verbal

Job Purpose

To manage and develop the home from hospital settle service in the hospitals that you are responsible for, to support timely and successful patient discharges from hospital and prevent re-admissions.  You will work in collaboration with hospital teams and community teams to lead and manage your team to support the smooth transition from hospital to home for service users.  The service works with people for time limited support (usually up to 6 weeks) to adjust and settle back in their homes, and to prevent readmission into hospital. 

Key Tasks

• Work in partnership with the hospitals and other health and social care professionals to coordinate safe and timely discharges for referred clients.  This involves attending meetings with hospital personnel including Discharge Co-ordinators and Ward Managers to discuss discharges and plan who and how individuals will be supported as appropriate. 
• Manage a team working across two hospitals and ensuring the service is available between the contracted hours.
• Work collaboratively with other agencies providing support services.
• Conduct initial assessments of people referred to the services as necessary.
• Plan and develop person centred interventions to provide short term support for people after hospital discharge.
• Work with GPs as appropriate in identifying and supporting patients to reduce unnecessary hospital admissions.
• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to preventing unnecessary admission to hospital.
• Support the hospitals with delayed transfer of care (DTOC).
• Coordinate and monitor the caseload of referrals allocated to team members to ensure a fair and manageable distribution of work.
• Rota planning to ensure staff flexibility and availability, enabling planned rosters that best suit both hospital and patient needs.
• Develop your team which includes; supervision, appraisal, planning of continued professional development opportunities and training needs.

• On a rota basis, provide weekend on call cover for the services you manage, and other hospital settle services provided by AUKEL.

Administration

• Lead on contract reporting, this includes quarterly, annual and ad hoc reports as necessary.
• Carry out annual customer service surveys.
• Enable the impact of the service to be assessed and inform the improvement of other local services by keeping electronic records of all referrals and interventions, and producing reports on activity and outcomes, in accordance with KPIs.
• Meet and produce reports as required by the Head of Service.
• Ensure patient records are regularly updated through AUKEL database, and support staff to complete their administration via training etc.
• Work with the Communications team and the Head of Service, market and promote the service.
• Build and circulate a resource pack for the wider AUKEL team to generate referrals between services delivered by the charity.

Quality

• Be familiar with and implement AUKEL policies and procedures.
• Ensure the service is delivered in accordance with Care Quality Commission principles.

We are recruiting a General Advice Officer to support with the delivery of advice and welfare support to vulnerable guests who access the Community Wellbeing Service, a local support hub offering relief from the rising cost of living for local families. The General Advice Officer will support our guests on a range of areas including benefits, immigration, housing, money management and access to training and employment services. The postholder will provide high quality welfare advice and guidance through weekly one-to-one advice sessions, undertake case work, work with partners to facilitate referrals, and provide support to advice volunteers.

Advice Work

What You’ll Do

We are seeking a compassionate and highly organised individual to work within the Welfare Benefits Team. You will be required to undertake welfare benefit checks and provide tailored advice on missing benefit entitlements and on wider income maximisation areas. You will also be required to provide advocacy and representation at appeal tribunals and with challenging more complex benefit decisions.

The successful candidate will need to have experience of providing welfare benefits advice with skills in advocacy, negotiation and communication. They will be an articulate communicator with excellent IT skills suited to working in a paperless environment. You must be able to demonstrate an ability to relate to people of all ages especially older beneficiaries, in a manner which is conducive to relieving stresses and concerns by giving sound and impartial information, guidance and assistance. 

The role will be office based at our London HQ, currently with hybrid working of 2 days in the office and 2 days working from home. The successful candidate for this role will need to be Standard DBS checked and prove they have the right to work in the UK.

The Western Forest Director will drive, direct and manage the delivery and strategic direction of this new national forest. The Director will inspire and work alongside partners and the core Western Forest team to achieve the goals and targets set for the forest, ensuring it develops sustainable income streams and models of delivery by 2030.

The Western Forest Director will head up the core team in supporting and directing resource and expertise throughout the region. Some key duties include:

• Support the shaping and organisational effectiveness of the Western Forest team and partnership, with a focus on collaboration, learning and knowledge sharing.

• Manage and ensure delivery of the programme KPI’s and partnership delivery model.

To meet the needs of this role you will need:

• Demonstrable experience of impactful leadership, working across organisations and a wide range of sectors and stakeholders.

• Proven track record of securing significant financial resource for an organisation through large funding bids or other sources of income generation, ideally with a good knowledge of the tree and woodland funding sector.

We are the largest sexual violence support provider in Wales, with 30 years’ experience of delivering specialist therapeutic support to adults and children affected by the trauma of rape, sexual assault or sexual abuse.

PURPOSE OF THE POST

To provide administrative support and to ensure the efficient day to day running of the office. This post has a strong emphasis on client support and engagement.

GENERAL

MAIN TASKS AND DUTIES:

• Offer a welcoming and non-judgemental environment to all service users.
• Act as first point of contact with reception duties, including answering telephone enquiries, welcoming clients, dealing with incoming and outgoing mail etc.
• Record initial referral information from clients.
• Liaise with clients, counsellors and volunteers by telephone, Microsoft teams etc.
• Schedule client appointments.
• Co-ordinate counselling timetables.
• Maintain records, both written and computerised.
• Ensure an effective system of communication exists between clients, counsellors and volunteers.
• Word processing of letters, reports and other documents.
• Undertake relevant training, as required.
• General clerical tasks including photocopying, filing, etc.
• Complete routine health& safety checks.
• Order stationery and other supplies.
