The Croydon Health and Wellbeing Space Support Workers will provide advice, information, support and representation to Croydon residents who are referred to the Space through the local Integrated Care Network Plus multi-disciplinary clinical and community mental health support teams (MDTs) based in the local Mental Health and Wellbeing Spaces in North, Central/East Croydon and/or South Croydon.
In addition to casework, the role will be expected to assist with reception and initial referrals in the Space when required and will also support Space volunteers. The Health and Wellbeing Space team will ensure that support is person-centred; focussed on supporting recovery; that needs, and wishes are identified by people themselves; that they have a stronger voice in relation to issues that affect their lives; and that they are supported and empowered to improve and/or maintain their mental health and wellbeing and to live their best independent lives.
The multi-disciplinary team during the initial one-year test period will inform the development and growth of the Health and Wellbeing Space to meet local needs and to ensure that it can support community-based mental health and wellbeing across the whole population and in all 6 Integrated Care Network Plus localities by 2023.
The role is an integral part of the ICN+ which brings together clinical and community-based support and services in co-located teams based in the Health and Wellbeing Spaces. We are recruiting an Assistant Support Worker to join our Hillingdon outreach team. This is an exciting opportunity to join a new service working in Hillingdon and Heathrow airport. Working in this team will give you an opportunity to make a real difference to rough sleeping. This role will enable you to work towards quick outcomes and to support rough sleepers away from rough sleeping. This team will work alongside other agencies to support people to access suitable crisis and longer term accommodation and reduce the number of people sleeping in Hillingdon and airport.
Our Assistant Support Workers play a pivotal part in Thames Reach’s inspiring work in Hillingdon, helping homeless people get off the streets, back on their feet and looking forward to a brighter future.
With the help of our experienced senior staff and management, you will be supported and trained to use your strong communication and creative problem-solving skills. You will receive regular supervision from experienced staff who will both guide your work and encourage and facilitate your personal development.
You will have over-see the progress of a group of clients, some of whom will be on the streets of Hillingdon and some newly moved into accommodation, you’ll work with your clients to agree and complete support plans ensuring actions from these are carried out. This work will be monitored by experienced senior staff.
This is a challenging but extremely rewarding role that requires you to assertively reinforce our message that rough sleeping is an unsafe choice and give individuals you meet clear information about their available options and the necessary encouragement and support so they can make an informed choice to move away from the street.
As Assistant Support Worker you will:
- Provide a rapid response to referrals we receive ensuring people sleeping rough in Hillingdon are looked for within tight time scales.
- Support our clients with practical tasks that will enable them to move off of the streets or sustain accommodation.
- Directly support clients, identifying activities that will assist in their journey towards recovery and away from life on the streets
- Maintain and support links with community programs and resources.
- Ensure that all paperwork is completed and work is recorded and monitored.

Ideally, you will have some experience of selling products or services that involves handling cash/card transactions but we are happy to provide training for the right person. Good verbal and written communication skills and an ability to use office IT systems is needed as is the ability to work flexibly including weekend cover as part of a rota. An NVQ 2 in Catering or related vocational qualification would be an advantage.

Are you a Support Worker looking for your next career step?
We are looking for Support Workers at Bridge College in Openshaw. Support Workers play a vital role in ensuring the students are supported in their learning programs, including extended curriculum, external placements and visits.
This is a great opportunity for Teaching Assistants looking to broaden their experience within SEN or Support Workers/Care Assistants who are unable to meet the 24/7 flexibility required within the care sector.
Bridge College is the Together Trust’s specialist education college for students aged 16 to 25 years with disabilities, complex needs and autism. Promoting a multidisciplinary approach to learning the college has expertise in supporting students with a wide range of needs.
Under direction of a session leader you will provide support and guidance to small groups of students providing intervention support with their learning programmes, communication systems and learning environments. You will provide 1-1 support for SEND students.
Proactive in completing assessments of students’ progress and contributing to planning and review of processes you will communicate effectively with parents, carers and others ensuring confidentiality is maintained at all times.
Supporting teaching staff to plan lessons and evaluate learning, you will write reports and maintain records using college information and recording systems as required. Working alongside therapists to embed strategies to support engagement in learning you will undertake further training and communication, technology, therapeutic services and personal care intervention to enhance student’s learning opportunities by removing barriers to learning, you will participate in supervision and appraisal processes as well as contribute to college self-assessment reviews throughout the year.
As part of your role will involve driving groups of students and staff when required using college and/or own vehicles candidates with a full clean driving licence are encouraged to apply.
About You
You must have experience of supporting young people with a variety of complex learning needs as well as some experience of working with young adults with SEND in a learning environment. Passionate about advocating for the rights of young people with SEN you will understand the complexities of a variety of disabilities and adapt your approach to support students.
Understanding safeguarding concepts you will have knowledge of the needs of students with learning difficulties and complex needs and how these can be addressed. A thorough understanding of what constitutes support for students with learning difficulties you will be familiar with alternative methods of communication e.g. Makaton, PECs.
Creativity for stimulating and engaging pupils in their learning programmes you will have a real passion for education and improving the attainment of students, you will be willing to support with all aspects of personal care and meet individual medical needs.
A reflective practitioner you will be self-driven to develop personal skills through CPD opportunities as well as have resilience and work dynamically in pressured situations.
Level 2 in Literacy and Numeracy is essential for this role and you must have good general education and good written English. Those who have Level 2 or Level 3 in Care and Learning Support are highly desirable however this is not essential.

What will you be doing?
The purpose of your role is to give the highest possible standards of support to individuals with moderate to severe learning disabilities
This can range from supporting them to discover new interests, reaching goals like travelling or holidays and working positively with their families and friends. This will help individuals to engage in different leisure, home or work-related activities, all according to their personal choice
As a support worker, your responsibilities are varied and no two days are the same. As well as providing physical and emotional support you may be needed to help with household tasks and personal care as well as supporting individuals with healthcare needs, e.g. taking medication.
Everything you do will enable people to have choices about their own lives, providing flexible services that meet people’s needs, and always looking for the best way of doing things.

WCN is a 15-bed accommodation service, for single homeless women with complex needs.
The successful candidate will have an understanding and awareness of working with people with challenging behaviour and with knowledge of the support needs the client group presents with.
We are looking for reliable and self-motivated candidates who have an understanding of the issues facing women affected by homelessness, and an awareness of the support needs the client group presents with. You will be updating documented support plans to help service users achieve objectives.
The main functions of this role will be;
- to build the skills of service users to empower them to maximise their independence
- to contribute to the achievement of contractual service obligations
- to work in partnership to maximise service delivery
- to update documented assessments and support plans with service users
- to support service users to engage with the move on process
- as a member of the team to engage with the management of the building
You will be required to cover the following on a rota basis
Morning shifts 8am – 4pm
Afternoon Shifts 2pm – 10pm
Includes occasional weekends / bank holidays
Part time positions also required hours to be discussed
