We are seeking experienced and highly motivated staff members for our Complex Needs Accommodation Service. The role will require you to work alongside the Regional Services Manager , to provide a high level of support to these clients at our Ashley Road and Milton House projects.
You will be in a specialist role designed to work with people with complex support needs relating to: mental ill health, physical health & substance use needs. You will also play a key part in reducing the risk of re-offending, liaising with external professionals, preventing homelessness and promoting the independence and social inclusion of these clients.
The working rota for this post is Monday - Friday. (no weekend shifts) and a mixture of earlies (8am - 3.30pm) & lates (3pm - 10.30pm)
About you:
- Previous experience of working with people with complex support needs.
- Ability to manage a caseload and deliver positive outcomes utilising your knowledge of the health, social care and criminal justice sector.

Wandsworth WellFamily and Foodbank Service is an adult mental health service serving the Wandsworth community and based in foodbanks and GP surgeries across the London Borough of Wandsworth. We offer short term, holistic support focusing on the needs of the whole family whilst directly supporting the adult.

New Horizon Youth Centre has been commissioned by the Mayor’s Office for Policing and Crime (MOPAC) as part of an Alliance with Safer London, St Giles Trust and the Anna Freud Centre to deliver on their Children and Young People’s Violence and Exploitation and Reduction support service. The commission will run from June 2024 for three years. As part of this, an additional service will be set up within the NHYC Youth Justice team specifically to deliver on the Alliance contract. The team works pan London to deliver a unique, youth-specific outreach service to young people currently or at risk of rough sleeping. We use our extensive housing expertise to place those young people into accommodation that is appropriate and safe whilst encouraging them to access the day centre to benefit from the wide range of services we offer. The Outreach Worker will work as part of Rough Sleeping Team to deliver at least two street outreach shifts per week and transitional casework to young people facing rough sleeping. Carers, who provide care for a family member, partner or friend who has a disability or long-term illness, play a vital role in our community. Many people who have someone who depends on them for support don’t consider themselves a carer yet they are fundamental to the effort to keep people cared for and safe, whilst saving the NHS billions.

As a part of the Support & Wellbeing Team, you will be required to identify informal carers from all communities in the London Borough of Camden, who care for a family member, partner or friend living with a Mental Health condition. You will provide relevant and appropriate support, information and advocacy to informal carers.
You will need proven experience of delivering provision within a related field and working with people and/or community groups; in particular we are keen to find someone with knowledge and experience in Mental Health. More details can be found in the job application pack on our website.

Please note, all shortlisted candidates will have two interviews, one with the Support & Wellbeing management team and one with a group of carers. For the carers’ group interview, it will take place face-to-face at The Greenwood Centre.
