The Role

We have a fantastic opportunity for a staff nurse to join our existing CHAS at Home team. With a base in Inverness the team works in families’ homes providing the highest level of family centred care to support the needs of babies, children and young people with life shortening conditions and their families.

As an experienced nurse you will be supported with an induction to palliative and end of life care, exploring areas such as advanced communication, symptom management, pain management and development of advanced clinical skills such as PN, PD and ventilation.

Key Responsibilities include but are not limited to:

• Provide a high standard of palliative nursing care to children and young people who have life-shortening conditions, through assessment, planning, implementation and evaluation of care plans, ensuring that the care provision is adapted to meet the complex needs of the individual.
• Provide nursing care at end of life and bereavement support including last office procedures, for the child, young person and their family, with support as required.
• As necessary, provide care for the child, young person and their family in a variety of settings, ensuring the care plan meets the needs of the individual.
• Collaborate and liaise with a range of professionals to meet the needs of the child, young person and their family taking opportunities to promote and raise awareness of the services provided by CHAS
• Maintain and accurately updates all written and electronic records in accordance with appropriate legislation and organisational standards.

About the role:

ABOUT THE ROLE

We are hiring a Mental Health Floating Support Coordinator to support our Luton Service which delivers support to individuals living within the community who have support needs pertaining to their mental health and wellbeing. The service works alongside statutory services, with the aim of moving people on to independent living within two years. You will empower and motivate our residents and participants to achieve their personal goals and gain greater independence, which enables them to reintegrate into the community and lead fulfilling lives. The support offered is tailored to the individual needs of the participants and residents through a recovery based model.

KEY CRITERIA

What we are looking for:

• IT Proficiency, ability to learn new software programs, basic Microsoft experience is required
• Experience of working with people of complex backgrounds, including in the criminal justice field, mental health, and/or substance misuse or a good understanding of the sector
• Understanding of the housing and social needs of people with multiple and complex needs
• Able to influence and negotiate positive outcomes with others

What we would like, but not essential:

• Appropriate qualification: NVQ/Diploma Level 2 in Health & Social Care/Community Justice, or professional equivalent
• Experience of providing housing support and a practical assistance within a residential or outreach support role
• Experience creating co-produced support plans and providing appropriate interventions for service user and liaising with other professionals

We require a Social Worker to provide high quality social work in our schools across the Diocese of Salford according to each school’s formal agreement.

You will work directly with children and young people, their parents, carers and schools in order to further the child’s welfare and provide the interventions necessary to enable them to use their learning opportunities to the full. You must be able to provide a non-judgemental, caring and confidential environment throughout this work to ensure the issues are addressed in an appropriate manner for all those involved.

To apply you will have experience of working with children and families and experience of direct work with children which includes supporting school staff with safeguarding. You must have competent IT skills which includes providing regular reports to the individual schools and Caritas and feedback to school staff as required. Opening in April 2021, Crawley Crisis Café is a partnership between Richmond Fellowship and Sussex Partnership NHS Foundation Trust (SPFT). The café will provide support for individuals experiencing a mental health crisis and will be open Monday to Friday 5pm – 11pm and Saturday and Sunday 3pm to 11pm. As Peer Support Coordinator your challenge will be to recruit and induct, with the Service Manager, the volunteers.

The volunteers will work alongside Recovery Support Workers and the Community Psychiatric Nurse to ensure positive outcomes for individuals that have come to the crisis café for support. And, when it comes to contributing to the development of organisational policies and procedures or attending networking events and community meetings to promote the service, recruiting volunteers and compiling a monthly rota for the volunteers again, we’ll rely on you.

To succeed, you’ll need a good understanding of the recovery and personalisation agenda and the principles and practice on which mental health services are run. You must be able to, liaise and work collaboratively with a team of colleagues. You will be well-organised and self-motivated; good problem-solving and decision-making skills are essential, as is an innate ability to cope with personal stress.

Family Support Worker (18351)

We have an exciting opportunity here at Barnardo's for experienced, enthusiastic and assertive Family Support Workers to join a brand-new team who will deliver the In Reach Hospital Emergency Department Family Support Programme.

The role is full time , 37 hours per week, permanent fixed term funded until March 2025 and will be based at the Lincoln County Hospital, Greetwell Road, Lincoln, Lincolnshire, LN2 5QY

Successful candidates will work in partnership with the clinical team to facilitate discharge of children aged 0-12 years from the Emergency Department where the clinical presentation is not acute and can be managed at home by local services.

“This role requires workers to have experience of holistic ways of working with families and young people. Including assessment, support, signposting and making referrals to appropriate services where needed. The successful applicant will be able to work as part of a team as well as on their own initiative and manage the pressures of working in both a busy emergency department and the community”

Some but not all of the key responsibilities are:

• Initial face-to face conversation or triage call to identify presenting issues and the level of intervention required.
• Delivering support flexibly and tailored to the needs of the family unit including telephone, virtual and face-to-face support and home visits where necessary. Contact with families could be a one-off or over weekly sessions based on level of need targeted. 
• Signposting to partner agencies, based on needs identified.
• Supporting families to navigate their way through referral pathways and enable them to access universal & targeted services.
• Where required advocating on behalf of families, supporting parents and carers to communicate with professional/Services.

Ideal Candidates must have's

• Experience of working with families with complex needs and the ability to build and maintain rapport through effective communication and listening skills, using a whole family approach.
• Ability to be clear and realistic about what support the service and you can offer to families.
• Ability to maintain a visible presence and sustain relationships with clinical staff, being mindful of the various working patterns/ professionals within the department.  
• Ability to navigate a high pressure, fast paced acute healthcare setting.

More about the Programme

The In Reach Hospital Emergency Department Family Support Programme model offers a robust way of supporting often vulnerable families to uncover and address unmet need. Through identifying the underlying factors causing the clinical presentation and attendance, and addressing holistic needs of the family unit, we hope activity to unscheduled, primary and emergency care is dramatically and sustainably reduced.

The model is based on Family Support Workers based within Children's Emergency Departments, offering holistic, family-oriented support in a flexible manner either in-person, telephone- based and home visits to be able to address the underlying causes of repeated / avoidable ED attendances.

*Although this contract has a permanent status, please be aware that this post is subject to funding currently to March 2025 and therefore should this funding not be extended further, you may be subject to a redundancy consultation or a TUPE arrangement.

** Please note the hours for this role will be required over a 5-day week and may be done in longer day shifts or with set times i.e. 8am to 4pm (to suit Emergency Department and community/family needs). The role can adopt flexible working arrangements to meet service and personal need but will NOT provide 24 hour provision in the ED settings. This can be discussed further at interview.

Contract Managers are responsible for the effective management of all contracts within their designated patch, including line managing the relevant front line staff. Leadership and competence development in direct reports is a key responsibility of the role. We are looking for a Complex Caseworker to join our Rough Sleeping Navigator team in Bristol who provide intensive support to male clients with complex needs experiencing homelessness. This passionate team offer person centred support service to help people who require a specialist intervention approach to support them through the transition from rough sleeping into sustainable accommodation.

In this role you will work collaboratively with the Rough Sleeper Navigator Coordinator and Outreach team as well as relevant partner agencies in Bristol to deliver sustainable outcomes to a male cohort of clients.

• You will advocate for clients who have been some of the most marginalised in the local area, to make a real and long lasting difference to their recovery.
• You will be responsible for overseeing a caseload of clients and providing day to day support to clients with their transition from the streets into accommodation and independent living with a strong focus on tenancy sustainment and rescue.
• This includes working with people on health, housing, employment and training support. You will be given support to build strong relationships with a range of other services to find creative and sustainable routes to recovery for clients.

We have an exciting opportunity for a Project Worker 2 to join our Kickstart Future service on a fixed term basis until 31st March 2025.

The Supported Accommodation Kickstart Futures service. currently supports young people across the London Boroughs of Sutton and Croydon, who are living either in specified semi-independent properties or elsewhere in the community.

Location of the post:

 You will be community based, so you would be expected to support and travel where the young people live covering Croydon and Sutton.

Hours

Full time – 37 hours.

The Ideal candidate will need:

• Experience of working with looked after children / care leavers and understanding of the accommodation needs of care leavers.
• Knowledge and understanding of the specific needs of young people leaving the care system.
• Ability to listen to and communicate effectively with children and young people.
• Knowledge/understanding of the needs of Unaccompanied Asylum-Seeking children and young people.
• Understanding benefits processes, including Universal Credit and Housing Benefit.
• You will need to be a confident writer with analytical skills and experience in completing reports.

Some of the key responsibilities include but are not limited to:

• The delivery of 1:1 keywork support to care leavers that meets their individual needs. Frequency of key work will be dependent on the support package of each young person.
• Development and delivery of weekly group work programmes for care leavers.
• Assessing young people's needs and agreeing a support plan for each young person in partnership with the Personal Advisor. Each plan will have clear milestones and outcomes to be.
• Taking appropriate referrals for the service.
• Producing quarterly monitoring reports and attending quarterly contract meetings as part of the quality assurance framework.

History of Service:

The Kickstart Futures service has been established to provide support to Unaccompanied Asylum Seekers and young people who have been trafficked, aged 18-25. We also support care leavers who are not from a forced migration background. The service launched in June 2020, initially supporting young people living in 1 semi-independent property in Sutton. We are currently providing support in 6 semi-independent properties in Sutton and Croydon, as well as within different community settings. We expect this service to continue to grow, and to provide support across an increasing number of properties.
