Changing Faces is seeking a motivated Health Professional Engagement Officer to join our Services Team. This exciting part-time role takes the lead in our professional liaison work to educate health and social care professionals regarding the impact of visible difference and raising their awareness of the important work of the charity.

Main purpose of the role

The main purpose is to develop and maintain relationships with HCPs to:

• increase awareness of the impact of visible difference and educate them in how to effectively support patients
• ensure understanding of the Changing Faces service offers and how to access these
• encourage them to sign up to regular updates and news and stay engaged with us.

This will be done by building and retaining relationships with HCPs via NHS departments, organisations and societies within the field of visible difference; finding ways to keep HCPs on our mailing list engaged and interested in our news; and by providing information, meetings, education and talks both in person and online.

We are looking for

• A proven track record of engaging with Health Care Professionals (HCPs) at all levels, in a range of disciplines and building relationships
• Confident, engaging and experienced presenter to a range of audiences – clear, persuasive and authoritative in spoken communication, both online and in person
• High standard of written English, able to write information and presentations clearly, with an understanding of the needs of the HCP audience.
• Proactive, dynamic and innovative in approach, with an ability to be creative, self-supporting, meet deadlines and handle multiple priorities.
• Ability to work effectively, collaboratively, and sensitively with a wide range of senior stakeholders and service users.

Changing Faces is the UK’s leading charity for everyone who has a mark, scar or condition that makes them look different (a visible difference). Being different in a society where there is such pressure to look a certain way is extremely difficult.

People with visible differences are vulnerable to isolation, loneliness, social anxiety and low self-esteem. They face staring, unwanted attention and even bullying and hate crime. PRIMARY OUTPUTS

Key Responsibilities 

• Contribute to the writing of consistently high-quality funding applications and bid documents which are accurate, substantiated, and contain compelling content.  Ensure these are submitted to deadline and in line with the funders criteria as well as DMWS aims. 

• Write, edit and submit evaluation reports as required by the funder – working with the Grant Manager and operational colleagues for support. 

• Develop, update, and maintain an electronic project bid library resource, including written templates and application responses. 

• Maintain an up to date register of all grant and trust fund providers applied to for projects, submission deadlines, and other relevant information, including timeframes for re-applying. 

• Research and assess potential opportunities to obtain funding from grant making trusts and foundations, organisations, in order to sustain and develop DMWS services against strategic aims. 

• Contribute to the development and updating of DMWS Website 

• Develop and promote content for DMWS Social Media channels and other communication mechanism including print and online. 

• Identify and build strong internal and external relationships. 
   

As and when required, attend at various meetings and events at headquarters or other locations. 

Work with other head office and operational colleagues to share intelligence and contribute to the delivery of DMWS Strategy and organisational sustainability.

Debt Advisor

Scope of role

The Debt Advisor is responsible for providing comprehensive, video debt advice to prisoners across thirteen (13) HMPPS sites, tailored to their specific circumstances. The advisor ensures income maximisation and provides ongoing casework support, acting on behalf of clients where necessary. We currently have an immediate excellent opportunity for flexible and proactive individual to join our Nightingale Home from Hospital service, which provides support to older people over the age of 65 living in the London Borough of Richmond upon Thames on their return home from hospital or when they have become unwell in the community.

The team is involved in organising and delivering tasks which facilitate a speedy hospital discharge / make the home safer to avoid readmission and avoid admission in the first place. This includes preparing the home for discharge; fitting keysafes; moving furniture; shopping; arranging travel and providing advice.

- Developing a holistic plan of support and directly delivering aspects of it.

- Liaising with hospitals, social workers, volunteers, family members, carers and other voluntary sector organisations and services.

akt is the UK’s national LGBTQ Youth Homelessness charity. We provide safe homes, mentoring, and support to ensure that no young person needs to choose between a safe home and being who they are. 24% of young people facing homelessness in the UK identify as LGBT (National Scoping of LGBT Youth Homelessness, akt, 2015) and 78% of LGBTQ+ young people cited an absence of family support (The LGBTQ+ Youth Homelessness Report, akt, 2021). Overview

This is an exciting opportunity for a Housing Support Worker to join our new Supported Housing Scheme in Lambeth, an LGBTQ+ supported housing service with six units for young people aged 18-21 years. The service is due to open in late May 2025 and is part of a three-year funded joint project with Metropolitan Thames Valley Housing Association and Lambeth Council. The role is part of the Services team and is supervised by the Supported Housing Manager. Other members of the team include another part time Housing Support Worker and a part-time Youth Engagement Coordinator. 

Please note this is a part time vacancy working 17.5 hours per week. The pro rata salary for hours worked is £15,257.50 (£30,515 full time equivalent). Hours will be worked on a Week A / Week B rota basis, Monday to Sunday between 10am and 8pm. 

Job Summary 

In this role you will be responsible for supporting residents to reach their goals and aspirations and move on to independent living. This will involve:

• Supporting young people – building positive and affirming relationships, co-producing holistic strengths-based support plans, coaching and supporting young people with independent living skills and providing them with advice, guidance and support.
• Safeguarding and risk management – working in partnership with Social Services to support young people to keep themselves safe, attending strategy meetings and implementing associated action plans, creating and upkeeping evidence-based risk management plans.
• Service delivery and development – record keeping contributing to KPI tracking, contributing to the development of best practice around supporting LGBTQ+ young people across Lambeth, ensuring young people’s experiences and voices are centred with an emphasis on co-production.

Project Background

Come and join our award-winning place-based volunteering programme that works with residents living across Hackney and the City of London.

You will be based out in the community - the programme is based at GP practices - to engage, support and enable local residents and patients to deliver local services for themselves, to meet local needs. We support residents to come together, to share their skills and experience for the benefit of each other, to deliver local groups and activities such as a book group, film club, exercise classes or cooking group. Residents from diverse backgrounds and demographics attend these activities together and make new connections, building strong local communities, all working together to make their area a better place to live. People, many of whom are isolated across all ages, and who have difficult life circumstances, are brought together, providing each other with mutual support and encouragement to develop new skills and pursue life goals, and to feel better able to voice their concerns to services.

You will be joining a nationally recognised project - having won an award from the Royal Society of Public Health in November 2024 - to support residents to fulfil their personal goals, make connections with others, and design activities and services that meet local needs. You will also be supporting people to improve their mental and physical health through signposting them to appropriate services and encouraging them to talk to their GP if they have concerns; and will be working in partnership with GP practice staff to co-create volunteer roles to make the practice more efficient and a better place to visit.

Who are we?

Volunteer Centre Hackney is a medium sized charity (currently 30 staff) based in the London borough of Hackney, working to support local residents to realise their skills and passions through volunteering and social action, and to share these for the benefit of others.

Our mission is to build and strengthen communities across Hackney through supporting residents of all ages and backgrounds to volunteer and build projects together.

In 2021, we were awarded the Freedom of the Borough Award by Hackney Council for our efforts to support residents during COVID, which included mobilising over 2000 volunteers to deliver food, prescriptions and essential items, provide emotional support to isolated residents and support the vaccine rollout. 

We work with hundreds of diverse community organisations and deliver various programmes to help residents share their skills and deliver their own social action projects to improve people’s lives. These include a befriending programme, volunteering brokerage matching local people to volunteer roles based on their skills, interests and goals, a specialist mental health programme, peer research and multiple resident engagement programmes at GP surgeries and estates across Hackney.

Post holders need to be enthusiastic and engaging, bringing energy to the role, be self-motivating and confident to work primarily alone, albeit with some support from the team members based at other sites.

About this key role

Your role will be to provide young person centred holistic support – including everything from helping clients to understand their own behaviour and its consequences, and promoting change, to helping clients’ families to support them and providing practical help with attending appointments, education, training and employment options, housing, benefits, debt and other aspects of day-to-day living. You’ll be part of a client focused SOS Team delivering casework services, predominantly Monday-Friday during office hours, and within Islington (although other hours and deployments may be required). You’ll also work closely with a range of partner agencies to ensure clients access the appropriate services and get the best outcomes.

Working flexibly within the community, with responsibility for one or more volunteers, you’ll be building effective and engaging relationships with young people involved in or at risk of being involved in the criminal justice system, with the aim of improving their life chances. You’ll conduct robust risk assessments and strengths-based needs assessments, with safeguarding as the priority, and ensure that all young people work towards an agreed support plan which is regularly reviewed and adapted. You’ll also build effective relationships with agencies providing services to the client group, and enable clients to engage in positive activity within the community (e.g. boxing, football, etc).

What we are looking for

• Significant experience of working with children, young people and/or vulnerable adults and delivering interventions that have had a positive impact – preferably in a health and wellbeing context and/or on an offender led support project.

• Experience of providing support, advice and advocacy, with the ability to assess clients’ needs. 

• Extensive experience of managing complex safeguarding issues with children, young people and adults who are at risk of violence or exploitation, whilst working alone.

• Understanding of the physical, social, emotional and developmental needs of children and young people, their specific needs as they transition to legal adulthood, and the issues they face, e.g. exploitation, victimisation, offending, school exclusion, unemployment, trauma.

• Extensive knowledge of the impact of context – with a clear understanding of best practice around contextual safeguarding and those experiencing harm outside the home.

• Knowledge of trauma-informed practice in the context of working with children, young people and parents/carers impacted by violence and exploitation, and of how trauma –including from their own lives – can impact on how practitioners manage cases.

• Working knowledge of child protection and safeguarding legislation/policy, with experience of providing support, advice and advocacy to staff with a safeguarding responsibility.

• Understanding of the importance of good quality case recording quality assurance principles. 

• Ability to use electronic case management systems to record all aspects of the role, including action plans, outcomes and session data on a day-to-day basis.

• Recognition of the importance of resilience in coping with the emotional demands of the role and demonstrable experience of managing your own wellbeing.

• Relevant qualification to a good standard or equivalent experience – ideally with relevant accredited training such as safeguarding, counselling or mental health first aid.

The successful candidate will need to have experience of providing welfare benefits advice with skills in advocacy, negotiation and communication. They will be an articulate communicator with excellent IT skills suited to working in a paperless environment. You must be able to demonstrate an ability to relate to people of all ages especially older beneficiaries, in a manner which is conducive to relieving stresses and concerns by giving sound and impartial information, guidance and assistance.
