About the role:

You will be a Social Worker supporting our Hospice at Home team, working within a strong multi-professional organisation that is committed to providing holistic care for our patients.  You’ll be part of a great team of experienced professionals from who you can learn from and learn with, delivering support you can take pride in. The role also provides the opportunity to contribute more widely to Hospice clinical services including teaching, audit, and research.

About You

You will have well developed counselling and advocacy skills and be able to develop effective working relationships with other health and social care professionals in the area.

A knowledge of and experience of statutory social work in health and social care, safeguarding procedures and multi-disciplinary working are essential. You will have an excellent understanding of the social service system, and the ability to work with diverse communities.

Strong communication and people skills are fundamental as you will be required to work in partnership with the multi-disciplinary team, ensuring every patient and their needs are always at the heart of what you do.

YMCA DownsLink Group’s WiSE (What is Sexual Exploitation) Project is an innovative, award winning project working across East Sussex, Brighton & Hove and Surrey, with children and young people experiencing or at risk of sexual exploitation; as well as raising awareness of Child Sexual Exploitation and other forms of Child Criminal Exploitation.

We are looking for someone with the confidence to develop a new support offer for Surrey. This new post offers the opportunity to work in the WiSE team in Surrey in an exciting new role which has two strands to it; providing targeted early intervention peer group sessions to young people (up to 25 years,) who have been identified by their educational placement as being vulnerable too sexual or criminal exploitation. The role also entails working alongside Education and other agencies to prevent Violence Against Women and Girls by creating and facilitating training to teachers of the PSHE curriculum in both Primary and Secondary schools in Surrey. The successful applicant will be supported in the role through Reflective Supervision Practice, along with regular one-to-ones. 

This role will be based out of a Police Station in Surrey, which can be discussed at interview. There will be frequent travel and it is essential you hold a current driving license and have access to your own car.

You will receive support and training and a competitive starting salary of £20,000 for a 28 hour week. Reduced hours may be possible by agreement. This is a two-year fixed term post with the possibility of becoming permanent after two years.

We're looking for a Children and Family Worker to join the team in our West London Women's refuge. The service is a refuge accommodation for women and children surviving domestic abuse many of whom are living with trauma. we are looking for someone who really cares about the rights of women and their children.

As a Children and Family worker, you will support in providing a much needed safe and nurturing environment for children and their mother's, providing pathways, support and appropriate interventions to enable resettlement. You will support various teams to co-produce a tailored support plan which is altered to the individual's needs through a recovery model providing trauma informed and enabling environments to achieve this. 

Shift Pattern: 37.5 hours per week, working on a rota which is over 7 days including evenings. You will be based on site within the service.

Children and Family Relationships

• Provide parenting advice to support mothers and their families.
• Provide stimulating play opportunities to encourage children from all backgrounds to develop and express themselves, as well as support the children with their general learning, homework, and literacy.

Additional Responsibilities

• Attend appointments as and when required in support of residents.
• Undertake shift lead responsibilities on rotation, taking responsibility for ensuring day-to-day support delivery is effective, with staff carrying out their designated responsibilities, alongside ensuring resident and service-specific follow up actions are completed.
• Housing Management responsibilities, ensure residents and staff are safe, and the surroundings are kept clean and well maintained.

Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position. 

KEY CRITERIA

What we are looking for:

• Understanding and/or experience of working with people of complex backgrounds
• Previous experience and/or in-depth knowledge about working with children who have experienced trauma
• IT Proficiency, ability to learn new software programs, basic Microsoft experience
• Understanding of the trauma children and families may go through due to domestic violence and other previous experiences
• Understanding of the social needs and support required for people with multiple and complex needs
• Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with various challenging backgrounds, mental health, addiction, exploitation, homelessness, and previous convictions

What we would like, but not essential:

• Previous experience creating co-produced support plans, key working, and care planning, providing appropriate interventions for service users
• Previous experience providing housing support and practical assistance within a residential support role
• Appropriate Professional Qualification in: Social Care, Early years Education, Certificate in Childcare or other relevant
• Understanding and/or practical application of key legislation regarding social care, housing, criminal justice, and mental health
• Ability to Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent crisis

About the Role

Chance UK has been delivering early-intervention services across London since 1995. Our longest running programme is our mentoring service where Mentors pick the child up from home and run a 1:1 session in the community, once a week for 2 hours. They use the solution-focused approach to work on building the child’s self-esteem and confidence, set goals and explore new places.

In addition, we are growing a range of community based groups running after school as well as some workshops delivered in schools which will support children with developing social and emotional skills and resilience through activities or skills development.

Invisible Palace is currently looking for a Youth Support Worker to join an established circus project that works with local a primary school and youth partners in the Bromley and Lewisham areas of London.

Your experience does not necessarily need to be in a school or youth work setting, but could be working or volunteering in youth groups, sports coaching, after school clubs or the care sector.

Job Description - Freelance Youth Support Worker

• Freelance contract of up to 8 hours per week at £18 per hour for an estimated 44 weeks of the year.

Support Worker

We're looking for a kind, compassionate and resilient Support Worker to join our Hope House service in Slough.

£24,960.00 per annum, working 40 hours per week. Benefits include 25 days Annual Leave, pension schemes, staff discounts, healthcare and career progression.

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.
This is a great opportunity for any support worker to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT) We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.

What you'll do:

- Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
- Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
- Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
- Develop and maintain links with all key agencies and service providers in the local community
- Empower customers to ensure they receive the service and benefits they are entitled to
- Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards
- Encourage and enable tenants to pay their rent and to ensure that rent accounts are managed effectively in conjunction with the Housing Management team
- Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible

Young People Support Worker - West London (Ealing)

£29,750 to £31,401 per annum depending on experience

Working in partnership with the London Borough of Newham and East London Foundation Trust, IBIS House supports people who are medically fit for discharge from mental health inpatient stays but have other barriers to discharge. These may include housing, living skills, accessing benefits or the community, or other interventions and will be on a person-centred basis. The main aim of the service is to provide short-term support to customers to link with longer-term services to reduce escalations in need, and reduce inpatient readmission and reduce pressure on inpatient beds.

What you'll do:

* Assessing referrals received by the service, within the 4-hour target, and providing rapid response to these.
* Inducting new customers, and helping orient them to the community and local area
* Completing/supporting housing referrals as required, supporting customers with budgeting tools and providing support to customers in accessing any benefits or addressing any issues such as benefits.
* Building a relationship with the local borough and any key agencies. For example: no recourse to public funds teams, homelessness teams, community mental health teams etc.
* Support planning, including ward management liaison, medication profiles and medication reviews as required, identifying the goals and steps to support customers to manage their own support and risk and ensuring these are correctly documented within Look Ahead's paperwork.
