Dementia Concern is a long-established local charity providing life changing care and support to people living with dementia and their families across West London.

After several years without a dedicated fundraising department, we are ready to rebuild and strengthen our income base, reconnect with the community we serve, and create sustainable funding for the future. This role will be central to that transformation, shaping how we tell our story, engage supporters, and grow our impact.

We’re looking for a Head of Fundraising, someone who can design, develop and deliver our fundraising stratergy. Someone who can innovate and who can combine stratergy, can lean in and has energy to connect with people, and who sees potential everywhere. If you’re motivated by the challenge of shaping something lasting and meaningful, this role offers the chance to make a visible difference, both to the charity and to the lives of people living with dementia.

Reporting directly to the CEO, you will have both the freedom and responsibility to build a thriving, sustainable fundraising function, securing income initially from trusts and foundations and then corporates, philanthropists, individuals, and community supporters.

You will play a key hands-on role, supported by a fundraiser, volunteers, and an experienced and engaged CEO and Board.

Key Responsibilities:

Strategy and Leadership

• Develop and deliver a clear fundraising strategy that diversifies income and builds long-term sustainability.
• Inspire a culture of fundraising across the organisation, empowering staff, volunteers, and trustees to become confident advocates and ambassadors.
• Use the compelling stories of our frontline team and clients to engage, motivate, and retain supporters.

Trusts, Foundations and Corporate Giving

• Lead the development of high-quality, strategic funding applications to trusts, foundations, and corporate partners (typically £20k+).
• Build and manage a sustainable pipeline of prospects, ensuring timely applications and stewardship.
• Line-manage and mentor the newly appointed Trust Fundraiser, supporting their development and success.

Individual Giving and Appeals

• Refresh and strengthen Dementia Concern’s case for support, ensuring a consistent, powerful message across all audiences.
• Design and deliver the charity’s first major appeals — including digital campaigns, local community drives, and postal appeals — to attract and retain individual donors.
• Develop engaging donor journeys and opportunities for people to give regularly, locally, and meaningfully.

Community and Partnership Development

• Reconnect Dementia Concern with its local community through relationship-building, events, and partnerships.
• Represent the charity externally with warmth and confidence, growing our network of local supporters, volunteers, and champions.
• Work closely with the CEO and Trustees to build mutually beneficial partnerships with local businesses, groups, and influencers.
• Oversee the charity’s website and digital presence, ensuring our communications are clear, inspiring, and supporter focussed.
• Lead on campaigns and public relations that raise awareness of dementia and the charity’s impact in West London.

About you:

You will have experience in:

• Trust Fundraising and at least one of the following: corporate partnerships, individual giving, or community fundraising.
• Proven ability to develop and deliver successful fundraising strategies.
• Excellent written and verbal communication skills, able to craft compelling cases for support and inspire others.
• A collaborative, ‘hands-on’ approach comfortable working in a small, dedicated team.
• Strong organisational and project management skills, with the ability to juggle multiple priorities.

Job Purpose 

To support the manager and deliver on the home and settle services across two hospitals, supporting the wider team, and provide leadership when necessary. You will work in collaboration with AUKEL, the hospitals, and other community health and social care colleagues.

The service supports people returning home after a stay in hospital, helping them to settle back at home, regain confidence and support with some practical tasks, e.g. shopping/cleaning.  You will manage referrals, carry out assessments, and with the Manager, allocate service users to Discharge and Settle Worker caseloads. You will provide direct frontline support and co-ordinate visits to the service users’ home in preparation for their return home, as necessary. You will work collaboratively to lead and manage your team to support the smooth transition from hospital to home for service users.  The service is time limited, usually up to 6 weeks, to adjust and settle back in their homes.

Key Tasks 

• Act as the first point of contact for all referrals across the hospital they are attached to. 

• Meet with patients in the community and/or hospital and carrying out assessments and risk assessments

• Enter referrals on to AUKEL’s case management system (currently Charity Log) in accordance with GDPR guidelines. 

• Allocate referrals to Discharge and Settle workers using tech available (currently the Call Round App), training will be available. 

• Collecting feedback from service users and uploading to case management system. 

• Prepare and keep stock of AUKEL Welcome Packs and service leaflets. 

• Deputise for the Manger at discharge planning and other meetings as directed. 

• Support the hospital to improve the patient’s experience of the discharge process 

• Monitor the case loads of each of the Discharge and Settle workers (DSW) to ensure they are evenly and fairly balanced. 

• Work collaboratively with other agencies providing support services. 

• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to preventing unnecessary admission to hospital. 

• Plan and develop person centred interventions to provide short term support for people after hospital discharge. 

• Deliver the front-line services as required e.g. when there is high demand, sickness or staff shortages. 

• Provide occasional weekend cover for the A/L of other Project Officers

• Work with people with a range of needs, dealing with issues ranging from social isolation and keeping people engaged in their community, to preventing unnecessary admission to hospital. 

• Supervise, support and develop DSWs and volunteers in your service. 

• Liaise with the handyperson service about referrals and jobs for service users. 

• Ensure DSW are resourced to sign post service users to other services when the home and settle service intervention ends. 

Administration 

• Keep AUKEL case management system up-to-date, input all data, and ensure all referrals, interventions, time spent with service users are recorded. 

• Allocate referrals to DSW using tech available (currently - Charity Log ‘Call Round’ App). 

• Ensure wards have good supply of service leaflets and referral forms. 

• Ensure data collected meets the requirements of the service’s contract monitoring and key performance indicators. 

• Ensure customer feedback questionnaires are sent at the end of each intervention and record returns. 

• Ensure admin tasks relating to service users, Discharge and Settle workers or general admin is carried out in a timely and efficient manner.

• Provide weekend cover as needed, i.e. annual leave and sickness, for the services you manage and other hospital settle services provided by AUKEL 

• Carry out monitoring of the service ensuring it meets the contractual requirements of commissioners. 

Quality  

• Be familiar with and to implement AUKEL policies and procedures.

• Support the design of research studies in collaboration with the lived experience team and academic partners.

About the Role

We are looking for an experienced Trusts and Philanthropy fundraiser, to manage a portfolio of high level funders, whose support is vital to the work of Become.

Working closely with the Trusts and Philanthropy Manager and the wider Fundraising team, the postholder will research potential funders, write compelling grant applications, and build strong relationships with funders to ensure continued and increased support. They will have a strong track record of securing five/six-figure funding from trusts and foundations, and other giving vehicles, excellent research and writing skills, and the ability to manage relationships with internal and external stakeholders effectively.

We are currently hybrid working: our team primarily works from home. In-person attendance at some meetings will be expected approximately once or twice every month, usually at our central hot-desking location in Old Street, London. For non-London based staff we will consider hot-desking options near you, if required. Job Purpose 

The weekend Home & Settle Project Officer is responsible for responding to referrals received over the weekend from both Royal London Hospital & Homerton Healthcare NHS Foundation Trust. You will work with the manager on Friday to deliver services and plan visits on the weekend for you to attend.  

The service supports people returning after a stay in hospital, helping them to settle back at home, regain confidence and support with some practical tasks, e.g. shopping/cleaning.  You will manage referrals, carry out assessments, and in liaison with the manager, allocate service users to Discharge and Settle Worker caseloads. You will provide direct frontline support and co-ordinate visits to the service users’ home in preparation for their return home. You will work collaboratively to lead and manage the weekend service to support the smooth transition from hospital to home for service users.  The service is time limited, up to 4 or 6 weeks, to adjust, build confidence, and settle back in their homes. 

Key Tasks 

• Act as the first point of contact for all referrals over the weekend across both hospitals. 

• Meet with patients in the community and/or hospital and carrying out assessments and risk assessments.

• Enter referrals on to AUKEL’s case management system (currently Charity Log) in accordance with GDPR guidelines. 

• Allocate referrals to Discharge and Settle workers (in the week) using tech available (currently the Call Round App), training will be available. 

• Collect feedback from service users and upload to case management system. 

• Prepare and keep stock of AUKEL Welcome Packs and service leaflets.

• Ensure admin tasks relating to service users, Discharge and Settle workers or general admin is carried out in a timely and efficient manner.

We’re looking for an Involvement Coordinator to join our Volunteering and Involvement team within the People and Culture Directorate. You’ll play a key role in coordinating our Involvement Network and Community Involvement Network, helping ensure activities are accessible, inclusive, and impactful for everyone taking part.

About the role

As an Involvement Coordinator, you’ll play a vital role in supporting two key communities: our Involvement Network and Community Involvement Network. 

You will work closely with colleagues across the organisation to help coordinate and deliver meaningful involvement opportunities, making sure they are accessible, inclusive, and delivered smoothly. Whether you’re managing day-to-day communications, responding to enquiries, organising activities, or maintaining accurate records, your efforts will ensure our involvement network members feel valued, supported, and engaged.

You will also help improve our systems and processes, advocate for more diverse representation in our networks, and contribute to our continuous improvement approach. A key part of your role will involve supporting and strengthening our Community Involvement Network, working directly with people and communities who are often underrepresented, to ensure their voices are heard and their experiences help shape the direction of our work. 

This role offers the chance to work across teams, build strong relationships, and make a real difference in how people with arthritis influence the services, campaigns, and decisions that impact their lives.

*As a hybrid worker the expectation is that you will spend around 40% of your working time in our office spaces or working in community settings.
