What will you be doing?
- Providing key worker support to individuals living with mental illness, including one to one individual or group support, liaising with other mental health agencies on behalf of people who use our services
- Enabling individuals to self-manage their personal and domestic resources and to undertake activities of daily living
- Enabling and encouraging individuals to find out about local services and facilities within their community and facilitate access to alternative services where specific needs are identified
- Familiarising myself with individual Care/Recovery Plans and support people who use our services in achieving their goals
- Carrying out all need’s assessments, individual safety and support planning within organisational and legislative requirements.
- Contributing to the protection of individuals from the risk of abuse and harm to self and others who use our services
- Increasing choice and involvement of service users and carers

Do you have what we are looking for?
- Experience of providing support to people with a mental illness; or experience of providing care and/or support services within another social care setting in either a paid or voluntary capacity.
- Knowledge and understanding of recovery principles and embedding the ethos of recovery into your own practice.
- The ability to make a comprehensive assessment of an individual’s situation and needs; to develop, implement and review risk management plans and devise support plans to meet these needs.
- QCF Diploma or NVQ level 3 in Care or Certificate in Community Mental Healthcare (or willing to work towards) or an equivalent qualification.

You will generally be working Monday to Friday 08.00am until 16:00pm, however we will need you to be able to work flexibility to support the needs of the service.
Our Mental Health Care Navigators use a coaching framework, to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending. By preventing this decline in health, through robust reconnection with health services in the community we will reduce health inequalities for those leaving the criminal justice system and provide support to allow a seamless, and effective transition to independent community living.

- Taking a lead role in service user engagement, including facilitation of service user forums and support of peer mentors
- Supporting health promotion, planning and delivering health promotion events in collaboration with colleagues
- Completing detailed, accurate, professional and timely notes for each patient, and upload them to the relevant clinical systems in line with organizational processes and local, regional and national dataset requirements
- Working in partnership with other disciplines and to maintain links with statutory and non-statutory agencies as appropriate, in relation to continuing care on release from prison.
Do you have what we are looking for?
- I am educated to NVQ Level 3 or equivalent with qualifications or training relevant to the Mental Health Care Navigation role, e.g. Health and Social Care
- Experience of supporting vulnerable individuals to identify, set and meet their own goals using 1:1 and/or group interventions
- A working knowledge of health, debts, housing, court systems, mental health, criminal justice and substance misuses
- Experience of working to support individuals using structured, time limited support and am confident with this way of working
- I hold a full UK driving license and have access to my own transport for work purposes

This is an exciting opportunity to work in a service delivered by Touchstone, Womens Health Matters, Holbeck Together and Leeds Beckett University to deliver a Wellbeing Space and Support Space across LS8, 9, 10 and 11. Your Space provides a community health development service in South and East Leeds. The service is funded by Public Health at Leeds City Council.
We are looking for a proactive, outgoing individual who is confident in using coproduction and asset-based community development approaches to improve individual and community protective factors and reduce risk factors associated with mental health and wellbeing. You will design and deliver activities with targeted communities and the wider workforce to reduce risk factors associated with mental health and wellbeing. You will have experience of motivating others to have positive conversations about mental health, delivering both group and individual work, in the form of training sessions and wellbeing activity and be able to work with vulnerable or marginalised communities and volunteers successfully. Purpose of the Job
The Wokingham Primary Care Project is a new service to be delivered in Wokingham Borough, across 4 Primary Care Networks (PCNs) from Spring 2021. Under the supervision of a team manager, a team of Wellbeing Workers will deliver a time limited person-centred service. The offer to service users will include non-medical interventions such as social prescribing, development of self-management tools and techniques and active support to engage people with relevant activities and community support.
Wellbeing Workers will forge relationships with existing Voluntary and Community Services (VCS) and local health services and mental health resources.
This project will offer support to people who are:
- Experiencing mild or moderate mental health problems and would benefit from some support as to how to manage their own wellbeing.
- Receiving treatment from the GP for mental health issues and would benefit from support to consider accessing local services and opportunities
- Caring for someone with a mental health issue and would benefit from support for themselves or for the person they care for.
The key responsibilities of the Primary Care Wellbeing Worker are to:
- Ensure that people referred by practice staff or via self-referral receive an excellent person-centred service via an initial assessment and follow-up sessions as required.
- Work with individuals to identify their own hopes and goals, and support them to develop a personalised action plan to increase resilience and wellbeing.
- Keep up to date with appropriate information about relevant support and services within Wokingham borough and West Berkshire- and other local, regional and national services - to ensure service users can be offered appropriate information and signposting.
- Review and assess progress of individuals, supporting them to complete their personalised action plans.
- Carry out follow up contact with individuals to evaluate outcomes using designated measures such as the Short Warwickshire-Edinburgh Mental Wellbeing Scale (SWEMWBS) and patient satisfaction questions.
- Collect and accurately record service user data onto an online database.
- Provide advice and guidance to GP practice staff about community and voluntary sector services.
- Forge and maintain excellent relationships with Primary Care staff to help ensure the service is well understood, and that staff are aware of ongoing work with service users.
- Ensure that the service is inclusive, ethically and culturally relevant and able to meet the wellbeing/mental needs of a diverse range of residents in the Wokingham Borough, g. younger adults, women with children, black and minority ethnic communities, veterans and groups who may have difficulty in accessing services.
About you
- Experience of working within mental health, wellbeing or similar services and as part of a team
- Experience of working with individuals enabling them to identify and pursue their own goals to support personal wellbeing and recovery.
- Experience of working in an often-demanding environment and supporting people with complex needs
- Experience of data collection and entry to enable the measuring of outcomes and service impact
- Knowledge and understanding of mental health issues, mental health services and the needs of people living with mental health problems

Peer and Volunteer Development Worker – Leeds Community Pain Service
NJC Scale Points 18-22, starting at £24,982 pro rata

This role is fixed term for up to 18 months, with the possibly of extension.
Working Location: During lockdown the team are working both at home and at the LMWS office 179-181 Haslewood Drive, Leeds, LS9 7RE. The Hours: 22.5 hours per week. Currently these are during the day, between Monday and Friday. There may be a future need to provide support for volunteers on weekday evenings if a need is identified for peer support at these times in the future (e.g. an evening peer support group for people who work).
This is an exciting opportunity for someone who is passionate about integrating Touchstone’s values and ethos into an NHS statutory service.
Since 1st January 2019, Touchstone have been integrating peer support into the Leeds Community Pain Service (LCPS) as a subcontractor to NHS Leeds Community Healthcare. The LCPS service helps people experiencing chronic pain develop ways of living a better life with their long-term pain.
The focus for Touchstone over the last year has been to develop peer support options for patients during and at the end of their treatment with the Community Pain Service. The focus for the next year will be to support colleagues to identify patients who have direct experience of chronic pain who may want to volunteer as peer supporters, to provide the volunteers with training and supervision to deliver positive outcomes with the Community Pain Service.
Touchstone’s Peer and Volunteer Development Worker will work alongside NHS employed staff to develop further roles in the service for volunteers and obtain feedback from patients of the service in order to make further changes and integrate service user involvement into their service model.
The Team: You will be part of the Leeds Community Healthcare Community Pain Service Team, they are a multi-disciplinary team which consist of roles such as physiotherapists, nurses, occupational therapists. You will attend their meetings, and post lockdown you will also attend their clinics to integrate the culture of Touchstone into their way of working. You day to day line management will be through their Service Team Manager.
To ensure you are still integrated into the rest of Touchstone you will also be a member of the LMWS Peer Support Team and will have supervision with their Peer Support Co-ordinator once every 8-12 weeks. Their team also consists of a Peer and Volunteer Development Worker, 2 part time Peer Support Workers and currently 28 volunteers. You will work closely with their Peer and Volunteer Development Worker to share learning and resources. You will also access monthly continual professional development (CPD) learning sessions and wellbeing days in the LMWS team, and you will be able to attend any additional meetings you may find useful such as morning check in calls.

MHSTs work with educators on a Whole School Approach to improving emotional well-being/mental health.

As a Recovery Worker you will deliver specific components of prescribed care packages under the direct supervision of Care Coordinators and Clinical Leads. Liaison and Diversion Support Time Recovery Worker

This role will be based in Kirklees.
Any job offer will be dependent on the outcome of police vetting at level 3, and reference checks.
The Liaison and Diversion (L&D) Service works with people of all ages (including children and young people) with a wide range of vulnerabilities including poor mental health and learning disabilities, who have offended or are at risk of offending. We support service users who have contact with police stations, the courts, prisons and probation. The Liaison and Diversion service is delivered by Wakefield Council across the West Yorkshire Police force area. The team is a multi-agency, multi skilled team. Touchstone is one of the partners in the service.
The aim is to work together to reduce the risk factors for reoffending by providing short-term support and signposting service users into longer-term support services.
L&D service across West Yorkshire will operate 7 days per week from 08.00 to 18.00. Project Workers will be required to work shifts to ensure appropriate cover, this will include regular weekend and evening work.

Safe Haven Inclusion Worker

Background to the role
Safe Haven is an exciting new development for Leeds Survivor Led Crisis Service and Touchstone as the first of our crisis services to be open 24 / 7. Unlike LSLCS’ other services (which are all self-referral), Safe Haven will be accessed via Leeds NHS Crisis Resolution and Intensive Support Service, and the Older People’s Support and Intensive Home Treatment Team (for those aged over 65).
Leeds Safe Haven will be open 24/7 to provide an alternative to hospital admission for people in acute mental health crisis. The service will have space for 6 guests (who may stay for up to one week) and 4 day visitors (who will come only in the day time, again for up to 7 days). The service will be staffed by a manager and deputy manager, a team of approximately 18 crisis support workers, and an inclusion worker (with a focus on outreach to disadvantaged groups).
About the role
From a service perspective, the post-holder will promote engagement with local communities and organisations to ensure that all groups are aware of the service and to provide outreach to engage those people who are under-represented. They will be a resource for the team to identify gaps and needs and consider the wider mental health system and access to relevant advice and training.
The post-holder will be on the rota to deliver support at the Safe Haven and be required to offer person-centred support to people in crisis. The Crisis Support Worker element of the post provides a range of practical and emotional support to people in acute mental health crisis, face-to-face. The team will usually work independently, although the Manager and Deputy Manager will be available during office hours to provide, supervision, support and guidance.
- To deliver support to guests/visitors in the house, provide a resource in the team on diversity issues and to work with external stakeholders.
- Hours: 30 hours per week:2 x 8 hours support work shifts per week, the rest are outreach hours. 1 x 3 hour monthly reflective practice and team meeting, which occurs on the second Wednesday or third Monday 6:30pm-9:30pm.

Summerfield is a 6-bedded residential home in Abingdon, Oxfordshire which supports children with significant disabilities ranging from physical health needs to autistic spectrum condition.
We are seeking caring and fun filled staff who will be passionate about the children we care for, showing understanding and patience in your approach.
Ideally, you will hold an NVQ level 3 or equivalent qualification in a relevant field however substantial experience of working with children with disabilities in the community or in a care setting may be considered adequate.
Due to the needs of our children the role will include providing personal care and supporting those who experience challenging behaviour and advocating for their needs. We liaise daily with external agencies including Social Workers and Schools. A comprehensive Induction and Training program will be given to enable you to support them to a high standard.
Our staff team work flexibly around a 24 hour day. These shifts are mornings, afternoons, overnights and weekends. Long shifts will be considered reducing the number a week to three.

What will you be doing?
- Providing a high quality, responsive and effective advocacy service in accordance with the service specification, contract and legislative timescales.

Do you have what we are looking for?
- Completed or am working towards National Qualification in Independent Advocacy (Level 3 or new Level 4)
- Completed or working towards additional specialist units: IMCA, DoLS , IMHA, CYP, ICAA.
- Experience of working in one or more of the following specialist areas: learning disabilities; mental health/capacity; older people; physical disabilities; sensory impairment; children & young people; substance misuse; NHS Complaints.
- A good understanding of advocacy practice and principles of empowerment and knowledge of relevant legislation
- Ability to respect and promote a person’s right to make informed choices and decisions without influence
- Ability to manage a complex caseload covering a wide range of subjects ensuring independence and clarity of decision making.
- Ability to support clients who may have a range of particular communication and other needs.
- Ability to collate information and write formal reports to a high professional standard.
- Ability to travel throughout the Locality, including travel when required to Hub Office
