As a Family Support Co-ordinator you will join our team of compassionate, experienced Family Support Co-ordinators, working to build strong relationships with local families facing complex challenges;

You will work one-to-one with families to identify their needs, match them to a trained volunteer and regularly review their progress.

You will provide on-going support and supervision to skilled, diverse volunteers, and work hard to build and maintain links with referring partners throughout our communities, ensuring holistic support for families.

As needs are identified, you will work together with our team of Family Support Co-ordinators to develop and facilitate supportive groups and activities for families.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.
We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.
Main Responsibilities:
· Undertake 1:1, group, detached and outreach work with young people
· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.
· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.
· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.
· To maintain confidential, up to date and accurate records of all sessions and plans.
· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:
o mental health and wellbeing of children and young people.

· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting

Make a difference
This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.
The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.
You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.
Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. Experience of working in mental health or dementia services would be an advantage.

Bonny Downs Community Association has been working together to change lives and transform our community since 1998. Over the past 25 years our team has grown to provide a wide array of services that benefit Newham residents from all walks of life.
This role is specifically funded through the Youth Endowment fund as a pilot project ‘Thriving Communities’. You will be required to work alongside Thriving Communities commissioners, as well as the project’s Youth Justice Worker’s, Educational Psychologist, and/or professionals assigned to individual young people’s cases- such as teachers, social workers and mentors. The Thriving Communities project is a small part of a wider collaborative project, EMPOWER YOUTH, delivered by two partner charities so, although this position is based at BDCA, you will be working closely with a second East Ham Youth Partnership Project lead and team of Sessional Assistants based at nearby charity The Renewal Programme (TRP). Staff from each organisation will work together to deliver the programme and cross-refer young people to activities led by both teams. You will also be required to work with BDCA’s own Children & Families team during the summer scheme and liaise with our marketing team all year round. The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.
South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.
The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.
You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.

We are seeking an Advanced Recovery Practitioner to support in the Women and Families team, within Mind in the City, Hackney and Waltham Forest, working in partnership with Turning Point and Antidote. The successful candidate must have demonstrable experience in working within a substance misuse treatment and recovery setting, must have specialist training and supervision in one or more complexities alongside substance misuse. This may be mental health, long term conditions, criminal justice, offending behaviour, homelessness, motivational interviewing, safeguarding, family work. Have an understanding of risk and risk management, as it applies to substance misuse and complexity. Have a working knowledge of Safeguarding of Vulnerable Adults framework, the ability to follow procedure and development of a culture of safe practice. Ability to work under pressure and have significant experience of working in health and social care.

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing.
You will support patients within Primary Care GP practices in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.
