About the role
The role provides residents with complex needs a personalised case management support to help them be reconnected, turning assessments into a practical action plan which fully involves the resident. They will ensure that the work they do is properly recorded and as appropriate is entered clearly onto CHAIN.
About you
To be successful in this role you will need to have a demonstrable level of experience and understanding of the range of approaches appropriate to working with challenging behaviour including current drug /or alcohol use, antisocial behaviour, offending, substance misuse, and complex needs. You will also need good IT skills (particularly Microsoft Office packages) and an awareness of issues facing single people that experience homelessness or rough sleeping. A proven track record working within local authority, volunteering, independent, charity or a social housing association is also a real advantage.
It would be an advantage if you had a full current driving license and a successful track record in developing services in response to changing needs and demands.

Wandsworth Community Empowerment Network (WCEN) and Citizens Advice Wandsworth (CAW) work in partnership to deliver the Help Through Hardship Crisis Project. This award-winning project has been funded by the National Lottery Community Fund since 2016, three years’ continuation funding has now been agreed from 2020 to 2023.
As part of this project CAW and WCEN have developed an Advice First Aid training programme which provides a two-day course for community and faith groups, alongside voluntary sector and statutory agencies. The sessions provide an introduction to key advice issues; how to spot problems, and signpost and refer people to local services for support. Following the training programme participants are encouraged to use their new skills within their community setting. This exciting new role will support community and faith groups to do this.
We are recruiting an Advice First Aid Community Worker to support key community and faith partners to co-produce Advice First Aid delivery within their community setting. When Covid-19 allows, the postholder will be based mainly in community settings and the WCEN offices, and 1 days a week based at CAW offices and training as a telephone assessor.

Background:
The vision of St Alban’s Fulham is to see a great awakening of faith in Jesus, transforming Fulham and beyond and ever since the church was re-opened 10 years ago as a church plant from
Holy Trinity Brompton, young people have been one of the key priority areas for us as we work towards that vision.
To give you a snapshot, over the last year, St Alban’s Fulham has supported 135 young people (60% male) through weekly youth clubs, sports activities, holiday programmes, mentoring and support. We have built strong partnerships and trust in the local community. We currently work in collaboration with the council, police, local schools, Youth Offending Team and other key community stakeholders. As result we have seen our youth provision make a considerable difference in the lives of local young people, seeing them grow in confidence, and build new friendships. The majority of young people live on the council estates adjacent to the church and are a focus for our projects.
We deliver a weekly discipleship group and offer termly events for young people to explore faith. Throughout the year St Alban’s Fulham supports three local schools by providing assemblies and offers courses in ‘healthy relationships’.
The projects are led and co-ordinated by a full time female Youth and Community Worker along with a team of local volunteers. Despite the challenges of 2020 we were still able to fulfil many of our objectives and have continued supporting young people online, outdoors, and in small groups.
Overall Aim of this post:
We are looking to recruit a dynamic Male* Assistant Youth and Community Worker who can help us expand the success of existing youth work, enabling us to take it to the next level in terms of depth and relational support, scope of programme, and scale of reach.

Look Ahead's Shirland West Mental Health service is commissioned by Westminster City Council. The service comprises 3 complex needs sites which have staff cover 24/7, as well as 7 step down sites. Support staff will benefit from working in a dynamic service across multiple sites, and will support a range of customers with varying support levels.
Enhanced Support Workers will help enable vulnerable people living in a variety of different tenures to manage their tenancies/licences and move towards greater independence within the community by providing a flexible and individually tailored support package. This role requires working on a rota basis including weekends, shift pattern - 8am-4pm, 4pm-10:30pm.

Tower Hamlets building Bridges is a family support service that has been present in the borough for several years, with a particular focus. The service exists to provide distinct support to families where a parent has a mental health need. Many of the families referred to the service have a high level of need and are living with a mental illness that has endured for many years. For some families this translates into a lack of prioritisation of children’s needs by the parent/s along with a range of parenting skills deficits. We provide a preventative home visiting service, which seeks to maintain the care of children in their own families and give emotional support and promote positive attachments between family members.
We are looking for an experienced Family Support Worker for our Building Bridges Service in Tower Hamlets. Your role will be to provide focussed intensive support to families where the parent / main carer have enduring diagnosed mental health difficulties. You will work alongside parents and their children mostly in their homes and offer practical and emotional support and creative responses to their needs (this will include help with developing household routines, parenting skills and behaviour management strategies). We are recruiting a Ministry Leader for Webber Street Day Centre and Hope Community Homes; Webber Street is a homeless Day Centre providing primary services to 50-70 homeless guests per day while Hope Community Homes is a mid-term supported housing project working to support formerly homeless men off the street. The role holder will lead and develop these ministries and train and equip their teams. They will also contribute to the training of other LCM outreach staff in homeless outreach ministry. They will collaborate with local churches to encourage their engagement in the ministry at Webber Street and Hope Community Homes, as well as support them in developing their own ministry to the homeless in their local community. This role will include the management of two teams of staff and volunteers.

NAZ is looking to recruit an enthusiastic and dynamic individual to take up the role of HIV Peer Mentor Coordinator with people of Black, Asian and Minority Ethnicity (BAME).
Your role will be supporting predominantly BAME people living with HIV in Central and West London. You will provide direct case management for clients as well as delivering workshops to target groups. - Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities
- Report any observations relating to customers welfare
- Adhere to all the policies and procedures of Look Ahead Care, Support & Housing plus those specific to the project including any statutory requirements
- Ensure Look Ahead Health and Safety policies and local protocols are adhered to at all times.
- Maintain records as required at the project under the direction of the support workers / management
- Participate in team meetings/reviews and the general development of the service.

We are now hiring a Support Worker to work closely and collaboratively with our Treatment Team, delivering treatment interventions face to face, online and over the phone.
As part of this role you will work directly with our service users, primarily working with those identified as requiring low-intensity interventions. This includes providing 1-2-1 interventions, conducting triage assessments and working with other support agencies to provide a wrap-around support to our service users.
This role will also provide administrative support, such as data entry to the Treatment Team, as well as assisting with raising GamCare’s profile within the local communities.
If you have previous experience in the addictions field or working with vulnerable people and wish to help those struggling with problem gambling, GamCare maybe the organisation for you.
The post will be full time and based in the London office. During the coronavirus period, temporary home working will be taking place. You will need to ensure that you have a confidential space within your home to work from, a home working risk assessment will be carried out by your line manager.

We have an opportunity for an Intervention Worker to join our Home For Good Team. You will join us on a full time, permanent basis and in return, you will receive a competitive salary of £28,981 per annum plus benefits.
About the role:
Home for Good is a community integration scheme, established in response to the evidence that people who have experienced homelessness often feel isolated and struggle to integrate into the local community once they have moved into accommodation.
As our Intervention Worker, you will provide ongoing encouragement and practical support to clients of the Home for Good Community Volunteer Scheme and Social Club and identify new sources of client referrals and promote the scheme.
Key responsibilities will include:
- To conduct meetings with clients ensuring they are carried out in a psychologically informed way with both new and returning clients.
- To establish positive and constructive working relationships with a range of homeless service providers and other agencies, proactively seeking new referrals, and liaise with these parties regarding clientrelated matters to ensure the best possible outcomes for our clients.
- To support clients to find solutions to their housing, benefits, health and other issues, including identifying appropriate agencies, referring clients to and liaising with these, and following up in a timely manner.
- To draw up psychologically informed action plans for clients ensuring they are person centered and taking into consideration the clients’ views.
- To lead on other group sessions for Home for Good clients and establish a focus group, to enable clients’ authentic input into the evolvement of the scheme.
- To share responsibility for covering the Home for Good duty phone on a rota basis, responding to outof-hours contact from befriending volunteers.
- To ensure that any health and safety and safeguarding concerns are reported appropriately using the correct systems.
- To assess any risk to clients, the environment and staff, record and communicate any risks identified using appropriate channels in the Passage.
Our ideal Candidate will have the following:
Experience
- Experience of applying a client focused approach to all areas of work
- Experience of working with and advocating for vulnerable people who present with complex support needs in a person centered and psychologically informed manner
- Experience of establishing and/or leading a focus group or discussion group, to promote and enable client involvement
- Experience of establishing relationships and working with a wide range of stakeholders and liaising with support agencies
- Experience of record keeping using databases
- Experience of managing conflicting priorities
Knowledge
- Knowledge of how to work with clients within a psychologically informed framework
- Knowledge of the issues which contribute to homelessness and challenges with maintaining a tenancy
- Knowledge of applying trauma informed care solutions
- Knowledge of risk and incident management in a psychologically informed manner
- Knowledge of welfare rights and how they impact on our client group
- Knowledge of homelessness resources, and other services supporting vulnerable people, and how to access them to obtain a positive outcome for our clients
