With your social work experience with children and families, you will support birth relatives whose children have a plan for adoption or have been placed for adoption. Working closely with colleagues in Buckinghamshire Council who will refer the relatives to PACT, you will provide support through individual sessions, support groups and drop-in sessions.
This role will include supporting birth relatives to meet with adoptive parents and to help birth relatives understand the importance for children, to keep connected to their relatives and significant people from their past.
Ideally, you will have previous experience of working with birth relatives whose children have been placed for adoption and you will have a good understanding of the adoption process. You will work with relatives from a range of diverse backgrounds, lived experiences and those who may be neuro diverse and you will need to demonstrate understanding, respectfulness and adaptability to your social work practice.

What you'll do:
If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. In some services, this will include delivering personal and physical care as appropriate
Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
Participate in the support planning and risk management, as facilitated by the lead Support Worker
Carry out security duties to ensure the safety of the customers and premises
Monitor the CCTV throughout the shift
Enable customers to make full use of community facilities by providing support as directed
Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities
Report any observations relating to customers welfare
Adhere to all the policies and procedures of Look Ahead Care, Support & Housing plus those specific to the project including any statutory requirements
Ensure Look Ahead Health and Safety policies and local protocols are adhered to at all times
Maintain records as required at the project under the direction of the Support Workers/ management
Participate in team meetings/reviews and the general development of the service

This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.
The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.
You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.
Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. Experience of working in mental health or dementia services would be an advantage.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. The service provides a 24/7 support service, with an emphasis on choice and control, enabling customers to achieve greater independence, improved quality of life and integration into the wider community.

Emmaus Greenwich is a unique charity that offers a home, work and individual support to people who have experienced homelessness and social exclusion. We don’t just give people a bed for the night; we offer a home, meaningful work and a sense of belonging.
At the heart of Emmaus Greenwich is our companions. Companion is the name given to those who live in an Emmaus community and contributes to the social enterprise, where they support themselves and one another. Living in a stable environment with the opportunity of work and individually tailored support helps our companions to regain lost self-esteem and the confidence needed to get back on their feet.
The majority of our charity's income is generated in our social enterprise which includes our four charity shops based Online, in Plumstead, Lewisham and Poplar. - Lead and implement a culture which provides confidentiality, promotes professional boundaries, protects data, and safeguards Companions, Volunteers and staff.

- To assist and encourage Companions to move into independent accommodation where appropriate
- Ensure that effective Safeguarding systems are in place and maintained, acting as the organisation’s Designated Safeguarding Person, with delegated responsibility for policy development, implementation, and reporting.

About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.

About you:
Ability to advocate for young people in different professional and community spaces
Values working collaboratively with young people to ensure the service best meets their needs.
An understanding of peoples' contexts and how this might be impacting them their lives and mental health
Cultural awareness and respect for the many different ways a person might make sense of their experiences and distress.

Approachable and open behaviour
Recognises the value of both 1:1 working with young people and the importance of working working as part of a group or team
Confident in the completion of regular paperwork and is well organised

What you'll bring:
Essential:
Up to NVQ Level 2/3 or equivalent or experience supporting young people or people experiencing mental health challenges.
Desirable:
Experience working in complex mental health environments
