The Minorities Community Gambling Link Worker will deliver an awareness-raising and personalised, holistic support programme for minority community groups in Lambeth experiencing gambling and gambling addiction-related harm, or those impacted by the gambling of a close family member. You will:

The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.
South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.
The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.
You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.

This is an exciting opportunity to be part of and help shape a new DHSC funded project - Early Support Hubs - providing innovative ways to support Young People’s (11 – 25 years) Mental Health and Wellbeing across the borough of Hackney.
This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.
We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.
Main Responsibilities:
· Undertake 1:1, group, detached and outreach work with young people
· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.
· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.
· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.
· To maintain confidential, up to date and accurate records of all sessions and plans.
· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:
o mental health and wellbeing of children and young people.
o Supporting vulnerable groups who would not otherwise receive support (levelling-up).
o Improving children and young people’s engagement with EET.

· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting
· Experience of working with young people, delivering therapeutic groups and/or 1 to 1 interventions in a youth work or similar setting.
· To have an active interest in working with those affected by mental health issues and recognise the impact this has on their life.
· Experience of building positive, impactful relationships with young people responding to their needs

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.
The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

About this role…
Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
You will:
- Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
- Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this
- Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation
- Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation
- Accurately record all work completed within given timeframes
To succeed as the Lead Worker, you will have:
- A strong commitment to providing a high standard of service
- Experience working with people who have rough slept or vulnerable adults and supporting them to access services
- Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently
- Good communication skills and confidence in public speaking when representing Thames Reach externally
- Experience in forming strong working relationships with partner agencies, being able to challenge them when required
- Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone

About this role…
Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. The services are short stay, and staff provide wrap around support and liaison with several external partners, including physical health, mental health, substance use and housing. Staffed 24 hours to ensure this quick turnaround, they are a key service to ensure people are off the streets and in stable accommodation.

We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing.
You will support patients within Primary Care in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community.
