About the Role

• Located at a 24-hour supported housing service in Clapham, we provide high quality self-contained accommodation for 15 women who have experienced homelessness and multiple disadvantages.
• The aim of the service is to provide a therapeutic environment in which women feel safe and can flourish, enabling them to move on into independent, permanent accommodation. The accommodation also includes overnight safe spaces.
• You will be responsible for providing support to all the residents as needed during the evening and night, including running co-designed activities, and responding appropriately to requests and incidents.
• The successful applicant must be able to model person-led, psychological, gender, and culturally informed support, be skilled at building trust and be a great team player. You must be confident working as the sole support worker on site and have good judgement in managing incidents.
• This is a waking night support role, undertaking 12 hour shifts alongside concierge, 8pm to 8am every Friday, Saturday and Sunday night. Please be aware that the building has several floors and no lifts.
• The post is open to female applicants only as the role is deemed to be a genuine occupational requirement under Schedule 9, Paragraph 1 of the Equality Act 2010.

Working in partnership with the London Borough of Newham and East London Foundation Trust, IBIS House supports people who are medically fit for discharge from mental health inpatient stays but have other barriers to discharge. These may include housing, living skills, accessing benefits or the community, or other interventions and will be on a person-centred basis. The main aim of the service is to provide short-term support to customers to link with longer-term services to reduce escalations in need, and reduce inpatient readmission and reduce pressure on inpatient beds.

What you'll do:

* Assessing referrals received by the service, within the 4-hour target, and providing rapid response to these.
* Inducting new customers, and helping orient them to the community and local area
* Completing/supporting housing referrals as required, supporting customers with budgeting tools and providing support to customers in accessing any benefits or addressing any issues such as benefits.
* Building a relationship with the local borough and any key agencies. For example: no recourse to public funds teams, homelessness teams, community mental health teams etc.
* Support planning, including ward management liaison, medication profiles and medication reviews as required, identifying the goals and steps to support customers to manage their own support and risk and ensuring these are correctly documented within Look Ahead's paperwork.

Voluntary Action Harrow is looking for a Health Inequalities & Community Engagement Officer to work within our Stronger Communities team. Stronger Communities aims to remove barriers to accessing healthcare, increase community engagement and co-production and reduce health inequalities in the borough of Harrow. We are particularly interested in increasing engagement from communities in deprived areas and encouraging applications from those who might be under-represented in our work.

Based in Harrow, we are a not-for-profit co-operative owned by our workers. We have in-depth knowledge of the local area and of who’s doing what. Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. IBIS House MH Step Down & Crisis

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.

East London Apartments provides 24 hour accommodation based support for up to 31 individuals with complex histories, multiple traumas, complex personality disorder(s) and a varied, disrupted housing history with experience of homelessness as well as individuals who have been diagnosed with severe and enduring mental health conditions who are under the Care Programme Approach (CPA).

When working with individuals with complex needs, who may display challenging, chaotic behaviours, regularly testing boundaries and have experienced multiple traumas as a result of abuse, neglect, years of street homelessness, or long stays in mental health hospitals, it is integral that the post holder is motivated to support individuals to progress. They should continually hold aspirations for individuals despite multiple setbacks, and display emotional resilience, able to process challenging incidents and remain motivated to deliver excellent support.

Night Support Workers enable people with particular needs to manage their tenancies and live independent lives of their choosing in the community. Night Support Workers are responsible for ensuring continuity of a support service at night and for keeping customers and the building safe.

What you'll do:

* Building supportive, trusting relationships with customers and creating a positive atmosphere
* Working proactively with other members of the team to handle the service caseload and administrative responsibilities
* Conducting key work sessions as required, that are innovative and engaging in order to achieve Support Plan goals
* Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
* Providing support with daily living activities, including practical assistance to support customers to independent living e.g. budgeting, cooking, establishing a night time routine
* Supporting customers to maintain a healthy environment and where appropriate assisting them to clean their accommodation and maintain good personal hygiene

Job Purpose

This role is within our crisis alternative service Safe Space - an out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. The service operates 365 days a year from 12:00pm (midday) - 11:30pm across all of our boroughs. The role will consist of two shift patterns to ensure the service is covered: 11:30am-6:30pm and 4:30-11:30pm. This will be on a rota basis across support workers.

Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone on a one-to-one or group basis to individuals experiencing mental health crisis. This will consist of 1:1 support; de-escalating and providing interventions, signposting for specialised or longer-term support, and delivering collaborative self-management plans with clients to improve mental wellbeing. The aim of this service is to de-escalate and prevent further crises.

The objectives of the service include:

• To improve the mental wellbeing of people experiencing mental health crisis in HFEH.
• To provide support to clients accessing the service- for instance: signposting, de-escalation, planned wellbeing activities etc.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To treat service users with respect, dignity and personalised support
• To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation
• To increase self-management skills of those accessing the service
• To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis.
• To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual.

The Role

The role of the support workers will be to support the rest of the team in delivering interventions on a one-to-one and group basis. The role will also involve triaging and assessing all clients accessing the service, signposting and delivering collaborative self-management plans with clients to improve mental wellbeing.

This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.

The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.  

You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.

Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. Experience of working in mental health or dementia services would be an advantage.

As a Resettlement Worker you will be supporting homeless women to gain the skills needed in order to live independently; our clients are expected to be with us for no more than 2 years and the main route into independent living will be PRS (Private Rented Sector). You will be carrying out a variety of workshops to support the clients to gain the skills needed and use our internal and external partners to help our clients get back into work or education. You will also be supporting the women to understand financial responsibilities, paying rent, bills, council tax and applying for benefits.

You will need to be someone who can work with other people unconditionally, who has an understanding of the challenges of homelessness and can work without judgement. You will need to be a strong team player, with excellent communication skills and able to work well with others. You will be part of a fantastic team, being one of five Resettlement Workers holding a caseload of women, with the focus on resettlement and preparing for independence.

This role is part of a much larger Project, so here you have the opportunity to work alongside and support your colleagues working with our women focusing on reflection, recovery, training, employment at all stages of their journey out of homelessness.

Salary

£33,158 per annum

Hours

Average 40 hours a week on a five-week rota. Normal working pattern will be working across early and late shifts & working 2 weekends out of 5. Working hours and rotas are subject to change by the organisation as required, based on operational/service need, though we try not to do this frequently. Some flexibility in working hours is expected.

Pension

The employee will be enrolled into a pension scheme providing the post holder meets the criteria for eligibility. Minimum pension contributions will be paid by the employer for you if you are eligible.

Annual Leave

28 days per year, inclusive of bank holidays. Rising by 1 day per year of service to a maximum of 33 days, inclusive of bank holidays. As the Project is 24/7, this role will require some weekend and bank holiday working, as per service need.

Occupational Requirement

Due to the responsibilities of the role, there is an occupational requirement under the Equality Act 2010 that the post holder is female.

The Marylebone Project is based over two sites, 100m apart, up to 7 floors, and the job regularly involves walking throughout and between both sites. The postholder must be able to access all areas of the Project, and may be required to do so quickly in the event of an emergency. There is also a requirement to travel across London for work related purposes. The ability to undertake these physical aspects of the role and to carry smaller items are required as essential.
