Support Coordinator

We’re looking for an innovative, passionate and professional individual with excellent communication and organisational skills to join the Stroke Recovery Service based in the Durham area.

This is an exciting opportunity to work with stroke survivors and their families to support them following stroke.

Position: S11233 Stroke Support Coordinator

Location: Home-based, Durham. However, frequent travel will be required as part of this role (may include team meetings or other work-related meetings)

Salary: £19,596 per annum (FTE £27,400 per annum)

Hours: Part-time, 25 hours per week

Contract: Permanent. However, services are contracted and there is currently funding for this contract until 31 March 2026.

The Role

The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

Reporting to the Stroke Service Delivery Coach, the Stroke Support Coordinator will:

• Support new stroke survivors and their carers from hospital discharge into the community.
• Deliver 6 month post stroke reviews.
• Provide personalised information, advice and support.
• Support clients to make informed lifestyle changes which will help them to prevent further strokes.

About You

The post holder will have experience/background in:

• A caring profession ideally with experience of supporting people with disabilities.
• Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
• Ability to use basic Microsoft systems.

This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.

We are looking for a highly organised and proactive Administrator to support the smooth running of our Programmes and Operations. This role is essential in ensuring that our logistics, partnerships, and communications run efficiently, allowing us to reach and support more young people. You will work closely with suppliers, referrers, and our internal team to coordinate key aspects of our programmes.

RESPONSIBILITIES

Programme & Operations Support

• Support the coordination of OSH’s residentials, events, and programmes, ensuring logistics run smoothly.

• Arrange bookings (e.g. facilitators, venues, catering, transport) to ensure contracts and payments are managed efficiently.

• Assist in staff recruitment and volunteer management with recruitment, interview scheduling, onboarding, inductions, organising meetings and receiving feedback.

• Manage resources in our lock up (based in Hornsey)

Administrative & Organisational Support

• Maintain accurate records, databases, and documentation to ensure compliance and efficiency.

• Handle general inquiries via email and phone, acting as a key point of contact for external stakeholders.

• Support the team with minute-taking in meetings.

Partnerships & Referrals

• Communicate with referrers (such as youth groups, schools, social workers, and community organisations) to ensure young people are referred and engaged in our programmes.

• Support with phoning participants and referrers to support them to complete sign up forms where necessary

• Track and monitor referral processes, ensuring smooth communication and follow-ups with referrers to maximise participation at our events

• Maintain good relationships with external partners such as venues, facilitators, transport companies and other organisations we work with. 

Finance & Compliance Support

• Assist with financial administration, such as processing invoices, logging donations, and tracking expenses.

• Ensure OSH meets data protection and safeguarding requirements, keeping records up to date.

Other duties

• Getting involved in OSH events in the evenings and at weekends, including residentials. These are overnight trips from 2 to 5 nights

IDEAL EXPERIENCE

• Strong organisational and administrative skills, with the ability to manage multiple tasks.

• Experience in the charity, youth, or refugee/migrant sector.

Length and Salary

This is a full-time role, on a one-year fixed term contract, to cover parental leave. The annual salary is £42,193 per annum, less any required deductions for income tax and national insurance. 

This role is based in Reprieve’s London office. Reprieve operates a hybrid working model and we require staff to work two days per week from the London office and the rest of the week from home. Applicants must have the current right to work in the UK, which will be checked prior to interview.

At Toc H Newport, we offer co-produced activities, outreach services and practical support to some of the most disadvantaged individuals and families in the area, reaching over 600 individuals in 2024.Our vision is of a neighbourhood where everyone has the strong community life, emotional wellbeing and socioeconomic opportunities needed to flourish.

Our centre for service delivery is the Toc H Community Centre in Eveswell, Newport, where our offer includes our popular Stay and Play sessions for low-income families, and children’s after-school club Active Juniors. Our activities and services are incredibly popular, and we hold longstanding partnerships with local agencies and stakeholders. 

It is an exciting period of growth for Toc H Newport, with a number of new funding streams making it possible for us to reach many more families in need across Newport in 2025 and beyond. This includes a £300,000 grant from the Ministry of Housing, Communities and Local Government’s Community Ownership Fund, allowing us to soon complete a full structural and interior renovation of our facilities.  Following a Community Asset Transfer, our space will soon triple in size, meaning we can offer more activities and services to more local people. About the Youth Endowment Fund

We’re here to prevent children from becoming involved in violence. We do this by finding out what works and building a movement to change things.

In recent years, violent crime involving children has increased. This is a tragedy. Every child is an important member of our community and society has a duty to protect them.

The Youth Endowment Fund (YEF) is a charity with a £200m endowment that exists to prevent children from becoming involved in violence. We will achieve this by finding out what works and building a movement to put this knowledge into practice. A big part of the movement that we need to build is in the world of health. We need to inspire and connect with health leaders across Integrated Care Services (ICBs), Local Health Boards (LHBs), Child and Adolescent Mental Health Services (CAMHS) and other relevant parts of the system. We need to spread what works and make our country safer for some of our most vulnerable children. We are looking for someone to lead on making this happen.

Key Responsibilities

We are making progress building the evidence of what works within and around health services to reduce violence. But the big risk is that nothing changes. That’s where you come in. Your role is to identify the best way to make change happen within relevant health services. Your main responsibilities will be ensuring that:

We have great relationships with the people who can make change happen.

This will include:

• Developing great relationships with senior policy makers, sector leaders and experts, including representing YEF in external meetings and speaking at events.
• Build a Strategic Advisory Board of leading experts across the health sector and keep members onside and excited about our work.
• Manage excellent Strategic Advisory Group meetings. You can read more about our Education Strategic Advisory Group here.

We deliver the health system recommendations. 

This will include:

• Helping to identify the right recommendations at a system level (such as changes in policy, regulation, inspection, funding, or guidance) that make it more likely highly vulnerable children get access to the right support at the right time.
• Creating and delivering a plan to deliver the health system reforms, working closely with leaders to make the change happen.
• Tracking progress carefully, being thoughtful and creative about when and how to change the plan. 

We work out the most effective ways to connect people with the evidence, then making those things happen.

This will include:

• Helping health leaders change how they plan or provide services to better protect children from violence, based on our Practice Guidance.
• You can read our first guidance for school, college, and alternative provision leaders here.
• Creating a plan to get people to follow our guidance, using what we know about how they think and behave.
• Continuously testing and improving our approach to get better results.

As a senior member of staff in the organisation you also:

About You

You are this sort of person:

• You bring deep experience of the health system. You’ve worked at a senior level in or with health services – potentially commissioning support for young people at risk of or involved in violence. You understand how ICSs, LHBs, CAMHS and other health leaders think, and know how to navigate and influence within the system.
• You communicate complex ideas clearly. Whether speaking or writing, you break down complicated concepts in ways that make sense to different audiences – without oversimplifying. You bring clarity where others bring jargon.
• You get things done. You’re organised, delivery-focused, and produce high-quality work, even under pressure. You work independently and to a high standard.
• You build trust and connect with people. From government ministers to youth workers, CEOs to 15-year-olds – you know how to listen, build rapport, and make people feel heard. You’ve led meetings, made strong introductions, and bring people with you.
• You think big and adapt fast. You’re a strategic thinker who can see the big picture without losing sight of the detail. You’re logical, creative, and open to challenge – always testing and refining your ideas.
• You understand young people. You get what life can be like for vulnerable young people and you understand the systems and organisations around them. Ideally, you’ve seen this first-hand, whether professionally or personally.
• You’re committed to equity, diversity, and inclusion. Not just in theory – but in how you work, who you listen to, and what you prioritise.

You must have this sort of experience.

• Delivering concrete change in practice or systems that improved children’s lives.
• Leadership experience in the health system. You’ve worked at a senior level in or with health services – potentially in commissioning – and you understand how to navigate and influence within these complex systems.

First-hand knowledge of the system that supports highly vulnerable children, particularly those at risk of or involved in violence. This includes children with conditions such as conduct disorder, psychosis, substance use disorder, ADHD, developmental language disorder, and traumatic brain injury. You understand the barriers these children face and what it takes to get them the right support.

While it’s not a criterion, we are especially interested to hear from applicants who have lived experience of violence affecting young people.

Hybrid Working

The office is based in Central London. Those living in and around London are expected to be in the office a minimum of 2 days per week. Interview Process

This will be a two-stage panel interview process. Interviews will take place in the week commencing the 16th June 2025. Second stage interviews are currently scheduled for the week commencing 23rd June.

The post holder will work within an integrated Psychological Therapies Stepped model delivered in partnership with the Lead Provider in local NHS Trust (CNWL) who will provide the Step 2 and Step 3 delivery within the integrated Psychological Therapies Service.

The post holder will provide line management and clinical supervision to a range of clinicians and teams within the Psychological Therapies Services.
The successful candidate will hold a small clinical caseload within the Psychological Therapies service as required.
They will also provide weekly clinical supervision and regular line management to Psychological Therapies Step 3 Counsellors.
Provide regular clinical group supervision to Honorary Counsellors in RISE.
Provide regular clinical fortnightly group supervision to Suicide Postvention Liaison Officers
Provide regularly fortnightly clinical supervision for the Suicide Bereavement service.

You are:

The postholder will be a qualified, skilled clinician and supervisor, highly motivated with the ability to organise and prioritise their own workload, preferably with experience of working and supervising within a Psychological Therapies programme.
