132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.

The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.

The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

What you'll do:

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.
The initial and co-produced assessment will incorporate models of co-production, observation, and person-centred planning. This is with a view that it is solution focused as to provide a framework for progression towards independent living.
Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.
Create support/ action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.
Liaise with partner agencies regarding making referrals where an unmet need has been identified via the multi-agency assessment.
Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.
Undertake key-working responsibilities for a caseload assigned by the Team Leader/Manager
Support customers to undertake all domestic tasks as past of the assessment of daily living skills, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
Develop and maintain links with all key agencies and service providers in the local community.
Empower customers to ensure they receive the service and benefits they are entitled to, including assessment and support of budgeting and debt management to garner their tenancy readiness.
Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards.
Adhere to Look Ahead's Policies and Procedures
Engage in learning and development activity to increase knowledge and skills.
Provide day-to-day instruction of domestic staff/Personal Support Assistants where appropriate.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.

Specialist Support Workers within Look Ahead are required to have either an extra dimension of experience or a higher qualification level as determined relevant for the particular post. Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.

The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across london on a spot purchase basis.

Specialist Support workers will manage a caseload of customers to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

Consists of working 5 days a week across all days, 8 hours shifts starting at either 8am or 2pm.

Assessment & Reconnection Worker - Rough Sleeping Prevention Service 

London,  Islington, Lambeth, Kensington and Chelsea

Are you interested in supporting people at immediate risk of rough sleeping and making a real difference to the lives of people who are homeless?

Our client's exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.

Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.

The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be expected work across all three sites.

• 7 day rolling rota including early, mid and late shifts.
• (K&C): medium – high support accommodation.
•  (Lambeth): medium support accommodation.
•  (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.

Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.

The service will work in partnership with the local authority to secure onward move-on options e.g. access the council PRS scheme, advocating for access to supported housing.

As an Assessment and Reconnection Worker, you will: 

• Be part of a busy team environment working closely with those new to, or at risk of rough sleeping. 

• Provide a personalised case management service to clients, turning assessment recommendations into practical action plans which fully involves the client.

• Build and maintain effective relationships to support clients throughout their recovery. 

About you

These roles will suit people with experience of working at pace in a public facing environment with the ability to adapt to new situations; as well as:

• An understanding of the issues faced by homeless or vulnerable people and the difficulties they experience in accessing services.

• Good communication skills and the ability to work with others whilst managing your own time.

Job Purpose

This role sits within our new Men’s Minds project, based in Hounslow. The aim of the service is to improve men’s mental health outcomes by developing a mental health network supported and directed by the voices of its members.

This will be achieved through three service strands: 1) Peer support network, 2) Psychoeducation workshops, addressing common needs of men’s mental health 3) Outreach support to raise awareness of men’s mental health and promote this service.

The Men’s Minds Project Coordinator will work closely with the members in shaping the service and ensuring voices of men in Hounslow are heard and acted upon. This service will be staffed with 1 part-time (25hrs per week) Project Coordinator and a pool of peer led volunteers. The Project Coordinator will be responsible for overseeing the volunteers.

The Role

The role of the Men’s Minds Project Coordinator is to arrange and coordinate a range of psychoeducation, and peer-led workshops which will be delivered weekly. With the addition of attending various community and stakeholder events to deliver the outreach strand of the service.

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. The Senior Multiple Needs IDVA will assess the needs and risks of survivors, carry out short and longer-term risk management, safety planning and support. As well as identify and refer to services appropriate to their needs. They will also proactively advocate and advice women of their rights and options for seeking help and support from other agencies, encouraging them to engage with other agencies, and help co-ordinate the provision of multi-agency support where necessary.

About You:

To be successful as the Senior Multiple Needs IDVA you will need to have:

Excellent understanding of domestic abuse and its effects on women and children, of best practice in supporting survivors of domestic abuse, and of the MARAC. As well as understanding of drug and alcohol issues and experience of working within a related field.

Bringing your experience of risk and needs assessment, safety and support planning, and crisis planning and successfully managing high numbers of referrals and experience of working in a co-located setting with another agency and providing consultations/briefings to professionals to support in upskilling them around Domestic Abuse. The candidate will have experience of partnership working and of maintaining excellent working relationships with a range of stakeholders.

We are excited to be taking the lead in the London Whole Housing Service Partnership (LWHSP) which will address and enhance the pathways of housing support for women experiencing domestic abuse, from removing them from immediate harm, to enabling recovery and resettlement across all levels of need. The LWHSP are seeking to build on our delivery across 11 East and West London boroughs of the Whole Housing Approach (WHA).

The Housing IDVA will work within a dynamic, fast paced, crisis intervention, advocacy and support service to ensure the voice of survivors informs every stage of the process. You will specialise in working with clients for whom housing, and risk of tenancy breakdown is a factor. You will work within the team to make proactive contact and provide high quality advocacy and support based upon a client led needs and risk assessment to women, focusing on working with those aged 18 and over who access the domestic abuse service. You will hold a caseload of survivors and will also be required to work as part of Advance’s duty team on a rota basis which will involve completing intake assessments and providing crisis intervention support.

About You:

As a Housing IDVA, you will need to have a degree of flexibility for colocation work across London boroughs; providing support to women who have experienced domestic violence, focusing on individuals with housing or tenancy issues. Working in our fast-paced crisis intervention, advocacy and support service, you’ll ensure the voice of survivors informs every stage of the process. You will have your own caseload of survivors and will focus on working with those aged 18 and over who access our domestic abuse service. 

You will bring experience in providing independent domestic violence advocacy and support for women who approach housing advice and assessment for help. Ideally experience in working with housing service providers, and advising women of their rights and options for seeking help and support from other agencies. You will have the ability to develop individual safety and support plans in-line with the clients’ needs and the risks they face.

We are currently seeking dynamic Practitioners to join our Children & Young People’s service (CYPS), working directly with young people affected by domestic abuse.

Domestic Violence Intervention Project (DVIP), a division of Richmond Fellowship since 2018, has been a leading provider of Respect accredited community perpetrator programmes working to stop domestic violence, increase safety for women and children, and reduce harm to families for over 29 years.

This role requires a good understanding of adolescent development, domestic abuse, child abuse and trauma, and the impacts of these on young people’s development, as well as a good understanding of the nature of domestic violence and its effects on women and children, and on the parent-child relationship. You will have experience of direct work with young people and their parents/carers, working flexibly but robustly and supportively, responding to challenging or harmful behaviour to facilitate change. You will have experience of providing structured one-to-one and/or group work counselling or behavioural change interventions, and of working with both children and adults from diverse backgrounds. You will have an excellent understanding of safeguarding procedures and be confident working both independently and as part of a team, working closely and collaboratively alongside partner agencies such as Children’s Services, Schools and Youth Offending Services.

You will have the opportunity to play a key role in developing DVIP’s young people’s services, working in partnerships with a range of statutory and voluntary agencies to deliver effective community responses to domestic violence, in line with RESPECT practice standards. Day to day, you will be working flexibly across a number of locations, including providing outreach in schools and other relevant or referring services, managing referrals, assessing risk and suitability for therapeutic intervention, and engaging families.

In this rewarding role you will be working directly with young people and their parents/carers, delivering structured interventions to increase safety, as well as providing specialist support to other professionals working with families experiencing abuse, through provision of expert consultations and training.

You will be able to work independently and flexibly, including some early evenings if needed, within any London Borough, where DVIP is contracted, for part of the working week. Hillside Clubhouse is recruiting for a mental health employment specialist to join our dynamic Camden team supporting people with mental health conditions to gain and retain employment.  The role is working within the nationally recognised IPS (Individual Placement and Support) model within an NHS secondary care mental health team based in the heart of Camden.

Hillside Clubhouse is a co-produced mental health employment support charity working across Camden and Islington. We support people with mental health conditions into training, volunteering, and employment opportunities. People can access our services via our Clubhouse, where they can share their skills and expertise with others and support the running of the charity.  Alternatively, they can receive support out in the community from one of our employment specialists based within primary and secondary care.

The Role: 

The role is within our IPS Service and will be based within a Camden NHS secondary care team. As an IPS Employment Specialist, you’ll play an important role in supporting people with lived mental health experience on their recovery journey by supporting them back into paid employment. This is an opportunity to offer ongoing support, to motivate and provide people with hope, while developing the tools and skills required to recover, ultimately transforming their lives.

A variety of skills are needed in this post including but not limited to -  a strong ability to develop empathy and rapport with clients; a determined approach to seeking out job opportunities; a person centric-approach to working with people; strong communications skills to motivate clients; strong administrative capabilities and an attention to detail engage with prospective employers via business development and face-to-face meetings, and to be able to challenge some perceptions around mental health.

• To engage directly and collaboratively with the clinical team while maintaining the existing multi-disciplinary approach to assisting clients seeking work. 
• To report and meet regularly with clinical team service managers and the IPS Operations manager, and to participate in both casework and clinical supervision from IPS Operations and clinical team services managers. 
• To be an active member of both the clinical and Hillside teams 
• To be responsible for own management of administrative duties tied to both Hillside Clubhouse and clinical team and adhering to existing Hillside and NHS protocols. 
• Source job opportunities specific to client needs from a range of sources including those advertised via current and conventional means, as well as through contact with local and Pan London employers. 
• To build up a network of employers to secure available and future vacancies. 
• To maintain existing and seek out new relationships with a range of external agencies of use to the post. 
• To provide ongoing support to both client and employers (where appropriate) when starting work and until such time as the client makes the decision to end the support provided. 
• To work with clients to collaboratively manage job searching activities and engagement with the service. 
• To manage and meet all KPIs, statistics and documentation as required, including but not limited to: referrals, first meetings, job outcomes, job sustainments of 6, 13 and 26 weeks, and work retention. 
• To provide guidance on the impact and/or the potential change to benefits when clients start work. 
• To work and manage time effectively in both community/external settings, and at clinical team.

• Secondary Duties: 
• To work out of hours as and when required, working in line with Hillside Clubhouse risk management procedures. 
• Undertake training when required and participate in ongoing learning and development. 
• To maintain professional boundaries with all internal and external stake holders including clients, and staff members of both Hillside and the clinical team. 
• To engage and participate in external IPS support services, training, and events. 
• To undertake additional tasks and duties as and when required, and to work outside of normal office hours when necessary.
