You may need to collaborate with other professionals such as GP's, memory clinic staff, and other health and social care partners. Support will be provided in person, over the telephone, via virtual options, or in writing. Our goal is to reach out to people affected by dementia from all communities and support their needs within their own community.

- Have a sound knowledge of IT systems such as Microsoft Office and Salesforce.
- Manage your time effectively by connecting with your colleagues/team on a weekly basis, remotely, working out in the community and at home.
- Be able to travel independently across the Borough of Wandsworth.
- Have the ability to produce localised reports.
- Due to the nature of the service, experience in safeguarding would be beneficial.
