About the role
 
Location: community-based in Bridgend with some office and home-based working.

We have an exciting opportunity to join a multi-disciplinary team as a Dementia Adviser. You will have the rewarding experience of providing information, guidance and support to people affected by dementia in Bridgend. You will help them in identifying their needs, maintain independence, improve their sense of well-being, and putting them in more control of their lives. This post forms part of the Team working around individuals and their families who are experiencing difficulties with changes to cognitive, emotional, or physical health needs. 

This new post will work as part of the Integrated Community Network teams in Bridgend. The Network Teams consist of staff across several professions. This includes social work, district nursing, community psychiatric nursing, occupational therapy, physiotherapy, speech and language therapy, dietetics and pharmacy. It aims to provide a seamless and holistic approach to an individual’s care or support needs during times of change or challenge.

As a Dementia Adviser, you will:

- Work closely with health, adult social care and third sector colleagues in assessing need and providing information and guidance to support people affected by dementia.
- Help meet the health and wellbeing needs of an individual by looking at “what matters” to them by providing timely and accurate information, advice, and assistance so that they may benefit from what is available to them.
- Work within the Integrated Community Network Teams in supporting the provision of support to people with dementia and their carers and wider support networks. 
- Deliver specific components of the service users care plan as delegated by the Integrated Network Team professional and will be required to implement care plans within the scope of their competence.
- Build robust working relationships and develop networking opportunities with a range of local partners and community organisations. 
- Create pathways with partner organisations who work in the field of dementia, ensuring seamless, holistic consistent support and empowering individuals to make informed choices.  
- Provide dementia support face-to-face, by telephone and online.

About you

We are looking to welcome an approachable, enthusiastic, and knowledgeable Community-based Dementia Adviser who will focus on supporting people affected by Dementia across Bridgend. 

You will have:

- A passion for supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives
- A solid and compassionate understanding of the needs of people affected by dementia
- The ability to work inclusively with people affected by dementia, along with an understanding of the need for client confidentially
- Great communication skills, adapting your approach to ensure understanding at all levels
- Effective networking and influencing skills and the ability to manage several tasks at once and keep yourself motivated to perform under pressure
- Good IT skills and experience working with databases and virtual meeting software (e.g., MS Teams and Zoom)
- The ability to travel across Bridgend independently when required
