About the role
Salary: £23,933 - £25,873 + London allowance of £3,600 and home working allowance £312 per annum (pro rata for 17.5 hours)
Hours: 17.5 per week
Location: 2 days per week at the Southwark and Lambeth Memory Service or Maudsley Hospital and one day from home.
We have an exciting opportunity to join our talented team as a Dementia Adviser. You will have the rewarding experience of providing information, guidance and support to people affected by dementia in identifying their needs, to help maintain independence, improving sense of well-being, and putting them in more control of their lives.
You will;
- Assist people with dementia and their carers in identifying their needs, providing information and guidance to help maintain their independence, improving their sense of well-being, and putting them in more control of their lives.
- Build relationships with a range of local contacts, networking with health and care professionals, and providing dementia support.
- Develop networks and pathways with partner organisations who work in the field of dementia, ensuring seamless and consistent support and empowering individuals affected by dementia to make informed choices.

About you
We are looking for a highly motivated individual, with an understanding of dementia, and the needs of those affected with these conditions. We are looking for someone who is approachable and knowledgeable to join our knowledgeable and passionate team. Our Dementia Adviser’s support people in their homes we interact face to face, over the telephone and virtually, to give people the tools and knowledge to make informed decisions about their future. As a community-based worker, you will be required to attend a range of meetings, appointments and venues as well as working from home.
You will;
- Be empathetic and non-judgmental in your approach with a commitment to equal opportunity.
- It is desirable that you have knowledge and experience of Adult and Children Safeguarding, Information Governance policies and procedures and statutory laws such as the Mental Capacity Act
- Have good communication skills to meet the diverse needs of our community and represent their needs to statutory and other voluntary agencies.
- Have a sound knowledge of IT systems to record data, write reports and communicate by email.
- Understand client confidentiality and how this is applied when representing client needs.
- Preferably had some experience of working with a wide variety of relevant agencies.
- Have the ability and means able to travel independently around Lambeth.

The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.
South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.
The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.
You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.

Are you passionate about improving support for families affected by substance misuse?

Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.
The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

This will form part of the new transformation of mental health services in the Royal Borough of Greenwich under the NHS Long Term Plan. The service will bring together the expertise of local primary, secondary and voluntary sector mental health care providers with the aim of providing flexible, holistic, and integrated services for adults with mental health problems.
Unpaid carers in our communities are supporting relatives, friends and neighbours, often with limited or no support, to live the best lives they can. The support of people with dementia, learning or physical disabilities, mental health difficulties or substance misuse problems by unpaid carers has been estimated to save the government close to the equivalent expenditure for the whole NHS.
We are looking for a Mental Health Carers Advisor who possesses an understanding of the challenges of unpaid caring and whose experience of working within mental health services includes support, prevention, recovery, and self-management. Applicants should possess effective skills in communication and have experience of completing assessments, managing a diverse caseload and working in a busy work environment. An ability to work independently but collaboratively, to be proactive and to show initiative is essential for this role.
The role will involve:

Make a difference
This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.
The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.
You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.
Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. Experience of working in mental health or dementia services would be an advantage.
You should be an excellent communicator and able to motivate and empower others. This is a full-time role.
