You will deliver personalised support to patients, independently managing new referrals from health professional and a caseload. We have an exciting new opportunity for a Social Prescriber to join our team in Merton.
The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing.
You will support patients within Primary Care GP practices in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.

The Role
A new opportunity has arisen to be part of the North Yorkshire Horizons substance use treatment service in Northallerton. North Yorkshire Horizons is a service made up of several key partners, led by Humankind, enabling the development of a peer led, recovery community. Providing a safe environment in which people can develop the life skills, and confidence to achieve and maintain recovery and abstinence from their substance use, successfully re-integrate into society, and become active, contributing citizens. You will be working within a fantastic team, where a real sense of team work is present.
Working as part of a multi-disciplinary, multi-agency team this role requires someone with great communication skills who can create and sustain effective working relationships with external agencies to achieve quality outcomes for substance misusing offenders. You will have experience of providing drugs and alcohol related advice and an in depth knowledge of the issues facing individuals with a history of offending and wider support needs.
In addition to delivering a range of PSI interventions, the post holder will also facilitate court imposed Drug Rehabilitation Requirements (DRR’s) and Alcohol Treatment Requirements (ATR’s), including the group work element and oversee the prescribed treatment for a caseload of service users within the Criminal Justice system. You may also be working with a cohort of people who use our service who may be on licence, subject to court orders or recently released from prison.
The Criminal Justice Drugs and Alcohol Worker will motivate service users to understand and manage their recovery and offending needs, and to support engagement with treatment provision. This role will work in partnership with other Humankind staff, NPS responsible officers, IOM, police, and community support services.
Skills and Qualifications
- To thrive in this role, you will need experience and knowledge of the criminal justice system, ideally with experience in substance use.
- A driving licence and access to a car is essential within this role due to the rural nature of our hubs.

We are looking for someone who will work with professionalism, empathy and kindness, have knowledge of working with children and families who have experienced trauma due to abuse and have a trauma informed, child focused approach. Experience of working with complex family situations where there are safeguarding concerns with the ability to assess risk is required.

In October 2022 Salford City Council commissioned Salford CVS to undertake research into the strengths of the Voluntary, Community and Social Enterprise (VCSE) sector as part of their Joint Needs and Strengths Assessment.
Every three years since 2008, Salford CVS leads on State of the VCSE Sector research in Salford and also across Greater Manchester. Working with other local infrastructure organisations, we aim to provide a comprehensive overview of the VCSE sector. Our last research report was published in 2021 and we have started planning for the research to be conducted this year.
In August 2023 we further developed our VCSE sector research and insight work with the commencement of our Salford 99 longitudinal study. This research project is designed to understand the issues affecting the VCSE sector in Salford and aims to track 99 VCSE organisations every six months for a three-year period, so that we gain ‘live’ insight in between our three-yearly State of the VCSE sector large research project. Alongside this work we conduct a Pulse Check survey with a variety of VCSE organisations to get a snapshot of what is changing for them each quarter.
This exciting VCS Research and Insight role involves a combination of VCSE engagement and insight work across Salford.

About the role:
The Foundation/Domestic Abuse Prevention Practitioner (FDAP) will play a key role in working directly with individuals using harmful/abusive behaviour within their intimate relationships. The role will include suitability and risk assessments, delivery of interventions, facilitation of group programmes, risk identification, information sharing and preparation of reports for courts and other agencies.
The FDAP Practitioner will be co-located into Priority Crime Teams situated in Newport Police Station to assist with the identification, screening, and engagement of individuals using harmful behaviour in their relationships, working closely with police to provide a timely and streamlined process for earlier identification, referral, and engagement/disruption of high harm perpetrators of domestic abuse.
For this role is it essential you have excellent understanding of domestic abuse, including the impact on victims and their children. Experience of working with individuals experiencing domestic abuse/ and or causing harm. Experience of group work delivery is desirable; however, we will consider applications from candidates who demonstrate an aptitude for the role.
You will need to be able to build and maintain effective working relationships with statutory and voluntary agencies to deliver the best possible outcomes within a multi-agency setting. As well as being tenacious in support of your clients and adept at problem solving.
IT literacy is a must and candidates must evidence both the ability to use bespoke packages and excellence in their recording practice.

Through the partnership of Macmillan Cancer Support, Self Help UK and the Nottinghamshire Integrated Care System, the Macmillan Beyond Dagnosis service delivers support for people affected by cancer, as well as facilitated access to voluntary and community sector cancer specific support and wellbeing interventions across Nottingham and Nottinghamshire. Caring Dads exists to change current practice to better include fathers in efforts to enhance the safety and well-being of their children. The practitioner will work directly with fathers with an aim to improve relationships with their children, families, and co-parents/carers. This will include promoting the service to a range of teams across Hampshire and Southampton, to generate programme referrals. You will work closely with Hampton Trust domestic abuse perpetrator service to identify client suitability for the Caring Dads programme.
As the Caring Dads Practitioner, you will be required to deliver the Caring Dads and Accredited Domestic Abuse Prevention Training (ADAPT) Programme. This includes one to one support and group work. Groups are delivered on weekday evenings; you will be required to deliver one group work sessions per week.
You will undertake treatment and risk assessments of individuals causing harm in their relationships; this includes active case/risk management of families living with domestic abuse. Although this position is based in Ashurst, you will be co-located into social work teams across Hampshire three days per week, with an aim to providing support and advice in identifying suitable families for the Caring Dads and ADAPT programme.

This post is an exciting opportunity to work with Nottingham City’s PCN 8 (Clifton and Meadows).. Using the Macmillan Electronic Holistic Needs Assessment to support Cancer Care Reviews will ensure patients have access to the right support for all their non-clinical needs. The post will also work on the preventative agenda, supporting uptake of cancer screening across the PCNs and supporting the practices within the PCNs to deliver their improvement plan around early diagnosis of cancer. You will work in partnership with GP surgeries to give non-medical support that maximises patients’ wellbeing through their Cancer journey. The primary role is to do Cancer Care Reviews, with access to each surgery’s IT systems to track patient lists, document, and communicate with practice staff. This role will involve liaising with services from within the NHS structure as well as external services, so you will need to be skilled in navigating intricate processes and organisational structures from both the inside and the outside. You will also help with improving screening uptake and community engagement, building relationships with local services and groups to further enhance the support.
