Role Overview:

Main Duties/Responsibilities:  

Person-centred care and support  

• Deliver effective social prescribing appointments using a holistic and person-centred approach that helps people focus on what matters to them. 
• Empower people to take control of their own wellbeing by building their confidence, motivating them to set goals and creating personalised action plans. 
• Signpost and refer people to local voluntary, community and statutory services that help meet their needs. 
• Support people to overcome their personal barriers to access VCSE organisations and follow up to ensure they are satisfied, able to engage, and receiving good support. 
• Treat everyone with dignity, and respect different cultures, religions, ways of life and other personal characteristics at all times. 

Communication and Collaboration  

• Build excellent relationships with primary care/adult social care teams, providing regular updates, sharing knowledge of local VCSE organisations and working together to develop the service. 
• Provide support and develop partnerships with local VCSE organisations, community groups, and statutory services to make safe and effective onward referrals. 
• Work with the Enable Health and Wellbeing Team and local partners to identify unmet needs within the community and gaps in service provision. 
• Promote the social prescribing micro-commissioning fund and support VCSE organisations to apply. 
• Act as a champion of social prescribing, increasing awareness and promoting its benefits to a wide range of audiences. 
• Work with a team of volunteer buddies to provide additional support to clients. 

Service Quality and Development  

• Collect and record outcome, monitoring and impact data using required systems, contributing to the evaluation of the service. 
• Manage own case load to ensure clients receive appropriate levels of support and expectations are met. 
• Gather client feedback to evaluate the quality of the service and to inform service changes and developments. 
• Identify how the service could be developed and improved and, drawing upon knowledge of service delivery, make suggestions about how social prescribing could be embedded to a greater extent across Wandsworth. 
• Keep up to date with key changes in the health and social care sector in order to shape and develop the service, including those relating to good practice and policy. 
• Lead by example by demonstrating excellent customer service, delivery skills, organisation and professionalism. 
• Complete Quality Assurance Assessments for VCSE organisations accepting social prescribing referrals to ensure they are safe, effective, and have the support they need to be part of social prescribing. 
• Work with your line manager to identify your own support needs and undertake continual personal and professional development.

Skills and Experience:  

• At least 2 year experience in a role that involves delivering face-to-face support in a relevant health or social care environment.
• Experience adopting a person-centred approach in order to meet the needs of service users and of empowering them to make informed choices.  
• Experience of working with people of all ages with a range of social needs.  
• Experience of working with the VCSE sector (in a paid or voluntary capacity), including with volunteers and small community groups.  
• Motivational interviewing, behaviour change, or health coaching qualification (desirable) or willingness to undertake training (essential).  
• Motivational interviewing, behaviour change or health coaching experience (desirable).  

Knowledge 

• Knowledge of the wider determinants of health, including social, economic and environmental factors. 
• Understanding of personalisation and the skills required to support self-care.   
•  An understanding of the principles of confidentiality and how these apply when handling service-user information.

About The Job:

This is a vital role in delivering our fundraising strategy and goals. The role holder will be responsible for raising income from individuals and legacies and increasing the volume and value of our supporter relationships, engaging new audiences and driving sustainable income growth. We will also trust the role holder to deliver fantastic supporter journeys and supporter experiences and contribute to the overall implementation of the Fundraising Team’s objectives.

Main Areas Of Responsibility:

Income Generation

· Lead on raising income to an agreed target, through acquisition, development and retention of individuals and legacy pledgers, as agreed with the Head of Fundraising and Communications.

Strategy

· Work closely with the Head of Fundraising and Communications to build and deliver a highly effective, sustainable, audience-centred public fundraising and supporter experience plan.

· Engage in market and supporter insight to inform decision-making and development of new activity/products.

· Develop a good understanding of our supporter base, their motivations and behaviours. Use our CRM database (Raisers’ Edge NXT) and data analysis and insights to inform the development of new fundraising campaigns and products.

· Take the lead role in the development of effective supporter journeys, working with the Community Fundraising Team to convert community fundraisers and events participants into regular donors.

· Lead the design and delivery of individual giving campaigns (including the Christmas campaign), using direct mail, digital marketing and advertisements to drive supporter acquisition and retention.

· Develop and design our legacy programme, working with Farewill and local law firms to encourage supporters to include Homeless Oxfordshire in their will.

· Work with the Head of Fundraising and Communications to develop and deliver a mid-level giving programme, driving the major donor pipeline and effectively stewarding donors giving at the £300-£1,000 level.

Operational Delivery

· Be responsible for the setting and managing of budgets, targets and KPIs. Report on progress, trends and take action when required. Analyse results and evaluate activity to inform future plans.

· Act as the lead-user for Raisers’ Edge, supervise the Data Administrator to ensure accurate data entry and reconciliation, and support the team with reporting and data analysis.

· Work collaboratively to ensure that all income generation activity is integrated and opportunities for cross-team working are maximised.

· Develop processes that enhance the team’s capacity, efficiency, and ensure effective fundraising.

· Manage external partners and agencies involved in fundraising delivery, ensuring due diligence, high quality and effectiveness.

· Ensure the highest standard of supporter experience in order to make the most of every interaction.

· Ensure legal and regulatory compliance across all areas of the public fundraising portfolio, including the Fundraising Code of Practice, Data Protection and Gift Aid. Model best practice and coach team member as required.

Leadership

· Line manage the Data Administrator, supporting them on reconciliation, data entry and gift processing.

· Provide management information for SMT and the Board of Trustees as requested and attending meetings when required.

· Build positive working relationships with all internal and external stakeholders.

· Represent the charity at external meetings as and when required.

· Engage with the wider charity sector, learning from peers and sharing best practice.

General Duties:

· Compliance with Homeless Oxfordshire’s policies and procedures.

Main Purpose Of Job:

We are seeking a dedicated and compassionate individual to join our team in a dual-role capacity, supporting two vital services run in partnership with Homeless Oxfordshire and Connection Support. The role covers both our Step Down provision—designed for individuals discharged from hospital under the Discharge to Assess model—and our service at Mawle Court, which provides temporary accommodation and support for people with complex needs, including those preparing for more independent living or in need of emergency provision. Working as part of a small, dynamic, and enthusiastic team, you will be responsible for delivering a safe and supportive environment across both services. You will lead on the case management of clients with complex needs, working collaboratively with partner agencies to develop and implement holistic, individualised support and move-on plans. • To proactively and dynamically assess, monitor, manage and respond to risk for individuals and in the service.

Purpose of the Job: 

The Corporate Partnerships Manager (New Business) drives income growth by securing high-value, multi-year corporate partnerships. With new business separated from account management, and as The Felix Project moves through its merger with FareShare to form a larger national food redistribution network, the role requires a strategic, results-driven specialist who can build a robust pipeline, develop compelling propositions and convert interest into significant private-sector income. Working within the wider Income Generation Team, the post-holder collaborates across all fundraising streams to ensure opportunities are aligned and maximised. 

The role leads prospect research, pitch development, early-stage cultivation, income forecasting and CRM accuracy, and ensures a smooth transition of secured partners into account management. The role also works with senior leaders, including the CEO and Trustees, to unlock major opportunities linked to the merger and the organisation’s expanding national profile.
 

Additionally, the post-holder will build and maintain strong relationships across the organisation, engaging with key internal stakeholders such as the CEO and Trustees to identify and cultivate funding opportunities. Role Purpose 
 
Grandmentors is our award-winning, inter-generational mentoring project running in almost 20 locations across the UK.We recruit, train and support Volunteer Mentors (normally people aged 50+) who are matched 1:1 with young people (aged 16-26) facing a range of life challenges and barriers.
