Job Purpose

This role sits within our crisis alternative service, Safe Space, which is a core and out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. Our aim of the service is to keep individuals well in the community and prevent readmission into hospital. The service operates 365 days a year from 14:00-22:00pm across all of our boroughs.

Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone or virtual on a one-to-one basis to individuals experiencing mental health crisis or preventing crisis.

The objectives of the service include:

• To improve the mental wellbeing of people experiencing mental health and social crisis in HFEH.
• To proactively work with keeping people well in the community to reduce re-admitters into hospitals by working with WL NHS teams (e.g. discharge, LPS, CATT, MINT and SPA)
• To provide a true alternative to A&E via a non-clinical drop-in service to support clients
• To provide support to clients accessing the service- for instance: signposting, de-escalation and crisis recovery planning.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To treat service users with respect, dignity and personalised support
• To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation

About the Role

You will provide advice and support to women who have experienced domestic abuse in a refuge setting, across two sites. You will carry out risk and needs assessments, develop support plans and carry out key work with women survivors. You should have a minimum of two years’ experience of working with this or a similar client group. An in depth understanding of the needs of women and children from a diverse background is essential.

As part of both roles, one of the job posts will be located in one refuge, with the other job post located across three refuges.

About You

The ideal candidate will have in-depth knowledge of domestic abuse and its impact on women and their children, as well as experience of providing emotional and practical support to victims of domestic and sexual violence.

You will have experience of managing risk and following case management procedures to meet the needs of the client group, and the ability to work with women to help them to understand their needs, risk assess, develop safety plan and support them in linking with other agencies.

We are looking for a Specialist Support Worker, to work alongside our Senior Specialist Support Worker to provide focussed support to our beneficiaries across London.

Main Purpose of the Role:

To lead the Support Team to provide emotional and practical support to families and individuals living with Duchenne muscular dystrophy. • To work with the Head of Operations and Events to contribute to Agenda topics and speakers from the community; assign roles for the conference to the Support Team.

• Establish a database for external stakeholders also supporting those living with Duchenne on a national basis, fed in to from the Support Team.

• To develop a triaging and caseload process for the Support Team, and how best to regularly monitor this.

• Be involved in the recruitment, selection, and induction of volunteers appropriate to your area of work.

• To maintain the contact database, keeping it up to date and accurate.

• Coordinate cover for the Support Team when there is sickness or leave.

This peer support project is delivered in partnership with South West London and St George’s Mental Health NHS Trust to support Carers of people with mental health conditions.  This project will work to support Carers through one-to-one support, outreach, training and peer support groups.

Using your lived experience of caring for someone with a mental health condition, and a strengths-based approach, you will support mental health Carers in their caring role.  You will provide person centred one-to-one- support, information, signposting, and advocacy, develop, and facilitate peer support groups and deliver training and outreach sessions at different locations across the borough.

You will have proven experience of providing direct emotional and practical support to women as well as up-to-date knowledge of legislation relating to survivors of gender-based violence. You will have excellent casework skills, good written and verbal communication skills, clear professional boundaries and be a proactive team player.

Specialist Support Workers provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. Support is also provided to develop a model of re-enablement towards self-advocacy, self-management and empowerment, incorporating the principles and practice of recovery, co-production and peer support.

I am a happy young man who enjoys socialising with those around me. I mostly communicate through non-verbal language, touch, and vocalisations. I have a good relationship with my family and get excited when they visit me.

I enjoy spending time in nature and like going for walks in the local parks. I also enjoy car rides and would prefer that people who support me have a current UK driving licence (I have my own Motability car).

I have specific sensory needs, those around me must be mindful of those.

The Creighton Centre is a well-established local charity based in Hammersmith & Fulham, dedicated to supporting older, disabled, and vulnerable adults. We are currently seeking a compassionate, proactive, and engaging Project Worker to join our Learning Disability Outreach (LDO) team.

You will work directly with adults who have learning disabilities, supporting them to lead more independent, connected, and fulfilling lives. The role involves a mix of 1:1 support, group work, advocacy, casework, and mentoring, as well as helping to deliver a timetable of social activities and workshops.

You will also play a key part in volunteer coordination, helping to ensure a warm and supportive environment for everyone involved.

Empathy, excellent communication skills, and a genuine interest in people are essential.

What We're Looking For

We’re looking for someone who has:

Experience working with people with learning disabilities or in a similar support or community-based setting

A flexible, team-oriented attitude

Confidence leading group activities and supporting individuals

An interest in community development and inclusive practice

A passion for gardening or the outdoors would be a welcome bonus, as the role may involve supporting participants in a small community garden.

AllChild (formerly West London Zone) is a non-profit organisation that proactively works with children and their families to flourish socially, emotionally and academically through our tailored Impact Programme.

Working with schools, we identify and support children and young people before the need for crisis intervention, connecting them to the help they need through our trusted adult, the Link Worker.

Our Associate Link Worker/Link Worker is based in the school with the children and young people they are working with on our programme. They work directly with the children, their families, their teachers and our partners to design and facilitate a two-year Impact Programme of support and champion the children and young people along the way.

Since our launch in 2016, our Impact Programme has helped thousands of children and young people at the tipping point of need. 75% are no longer at risk in their emotional and mental wellbeing; two-thirds improved their grades, and 90% of schools renew after the first programme, noting ‘transformational or significant positive impact’ for the children and wider school environment.

This is National Lottery funded role in a growing charity representing children and young people affected by the neuropsychiatric conditions PANS and PANDAS. 

The purpose of this role is to work closely with our established Youth Board to ensure that the voices of young people affected by PANS and PANDAS are heard, valued, and reflected in our work.

The successful applicant will support the Youth Board in identifying, developing, and delivering one or more youth-led projects that align with their priorities and lived experiences.
