This is an excellent opportunity to truly make a role your own and get involved in the project from the start, which will be supporting 10 adults with mild/moderate to severe learning disabilities, as well as additional mental and physical health needs.
You will play a pivotal ambassadorial role, spearheading Centre’s 404 service expansion into the London Brough of Brent and bringing our expertise, vision and values to families in need.
This varied and dynamic role will lead on the set up, coordination and provision of excellent person-centred care and support to service users, enabling them to live independently in their home and be supported in all aspects of day-to-day living. As Project Manager, you will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet individual needs. You will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of support workers.
The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Support is delivered in partnership with a number of stakeholders and partners in the community such as mental health services; substance misuses agencies, housing teams, police and social services.
About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. The role requires individuals to work on a rota pattern including some weekend and bank holiday working, as the service is open 365 days a year. Hours worked:21:30 - 07:30

What you'll do:
Ensure the welfare of customers providing emotional and practical support to customers who are in distress, in crisis or require medical assistance such as emergency first aid or the intervention of medical professionals.
Remain calm and professional when dealing with emergencies and incidents following the reporting procedures and completing the required paperwork.
Ability to work empathetically and compassionately with customers who present with complex needs such as mental health, substance misuse, safeguarding and physical health support needs.
Ensure the safety and welfare of customers by implementing and monitoring the visitors' procedure, preventing unauthorised access to the service.
Conduct health and safety checks and patrols around the building and exterior reporting repairs and any Health and Safety issues.
To undertake patrols and security checks of the building and exterior, monitoring CCTV, addressing any antisocial behaviour and completing the relevant reports.

About the role
As a Coach in the Structured Coaching Team, you will be the lead worker for people who engage well with the service and are able to work with you to develop and follow a personalised plan to end their homelessness. You will work collaboratively across departments to support Crisis' mission to end homelessness by delivering services that are person-centred, sensitive and responsive to the diverse needs of Crisis Members. You will understand and promote the importance of evidencing our impact in preventing and ending homelessness. Also evidencing the barriers faced by members to directly inform our Policy and Campaigns strategies, using the Crisis Member Achievement and Progression System (MAPS) and case management standards.

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.

Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.

Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.

About the Role
You will provide advice and support to women who have experienced domestic abuse in a refuge setting, across two sites. You will carry out risk and needs assessments, develop support plans and carry out key work with women survivors. You should have a minimum of two years’ experience of working with this or a similar client group. An in depth understanding of the needs of women and children from a diverse background is essential.
You will be expected to work across the two refuges as part of your role.
About You
The ideal candidate will have in-depth knowledge of domestic abuse and its impact on women and their children, as well as experience of providing emotional and practical support to victims of domestic and sexual violence.
You will have experience of managing risk and following case management procedures to meet the needs of the client group, and the ability to work with women to help them to understand their needs, risk assess, develop safety plan, and support them in linking with other agencies.
You will have a working knowledge of the available legal remedies, housing, and welfare benefits, as well as experience of providing workshops.
Outside of this, you will have excellent administrative and time management skills and the ability to effectively manage a caseload.

About the Service
Ella House is an Accomodation Service that provides support to women who have experience Rough Sleeping, Trauma and Multiple Disadvantage. The aim of the project is to provide support to women for around 18 months to build skills to be able to live independently.
The service is a 24 hour service that runs on a shift pattern over the week and has a concierge service covering nights. Staff will provide holistic support to service users looking at all their needs and putting a robust plan in place.

The Activities and Wellbeing worker will provide both 121 and group sessions. Speaking with women to understand their needs and wants and developing appropriate session plans. These can be wellbeing activities, skills based or creative depending on what the women require. There will be a small budget to accommodate activities.

Salary £25,175 - £28,387 depending on experience.
Holiday 20 days per annum with the addition of time off over the Christmas period when FST closes for two weeks.

FST is a charity and alternative education provider. We have been delivering practical and emotional support, as well as education for vulnerable young people for nearly 20 years. Working closely with schools, PRU’s, local authorities and other youth professionals day to day. We care for, empower, and give hope to our young people to realise their own potential. Aiming to empower our young people to return to full time education, in addition to developing the soft skills needed to make positive and healthy choices in their lives.

Our longstanding service to the youth of Wandsworth has allowed us to create amazing relationships with other youth organisations in the area. This has led to the formation of Youth Battersea, a partnership of 5 Battersea based youth organisations who work collaboratively on joint projects and share ideas, resources and skills.
Almost uniquely in our sector, we have an open-door policy to make sure that our young people feel supported long after they have finished their programme with us.
We run projects which encompass personal, social development and life skills. Our flagship mentoring scheme includes one-to-one and small group sessions. The focus is on the outcome, but also on the journey. In addition we collaborate with Caius House to deliver their after-school programme.

Main aim of the role:
Playing a vital role within our expanding youth support team at Future Skills, you will work to support and empower our young people to re-engage and return to education and employment. You will be creating structured lesson plans to deliver high quality soft skills sessions. You will be proactive and get involved with the pupil’s daily activities, promoting teamwork and setting a good example. You will also build positive relationships with the young people, fellow professionals, parents and carers as this is key to this role.
In addition to that, you will work with the team here at Caius House to help deliver an after-school programme for young people, usually one evening per week. To this effect, you will work with both the FST team and the team on-site at Caius House.

The post holder will be responsible for:

Working with colleagues to develop a soft skills curriculum for our group and one to one sessions.

Actively participating in the sessions we deliver including off-site trips and sporting activities.

Undertaking one-to-one and small group mentoring with vulnerable and at-risk young people aged 8-16 years old.

Working with the FST team delivering personal and social development workshops with excluded young people, encouraging them to engage and develop and challenging negative behaviours.

Building relationships and regularly communicating with parents/carers and professionals.

Building positive relationships with children and young people with social, emotional and mental health needs, inspiring and motivating them, acting as a positive role model at all times.

Bringing passion, ideas, creativity, energy and strategies to all projects.

Demonstrating understanding of the issues faced by children and young people.

Keeping accurate records of attendance and session evaluations using our online database system.

Having an active role in our regular team meetings.

Complying with the organisational requirements for health and safety, child protection, safeguarding and other policies and procedures.
The post holder should possess:

Previous experience of working with vulnerable, at risk and disengaged children and young people

A passion for building relationships with children and young people and helping them grow and develop.

Experience of building partnerships with other organisations and communicating with professionals as well as parents and carers

Knowledge of formal and informal education systems.

Ability to be flexible and adaptable, reliable and innovative.
