About the role:

We began a contract with the Ministry of Justice in June 2021, delivering a pan London Housing Advice & Interventions Service, in partnership with St Mungo's. With Single Homeless Project staff co-located in probation offices, you will be working in close partnership with Probation Services, to support people on probation to either sustain their current accommodation or to secure new housing as needed. 

We are seeking people who could deliver housing advice, guidance and support to clients. Working with a caseload in your local designated boroughs, you will produce comprehensive and high-quality assessments on their support needs; particularly their housing, employment and financial needs and follow the most suitable pathway for them. 

We are also looking for people who are interested in working in this sector and with this client group.  We encourage applications from those, who are also at the beginning of their career path - graduates and those with transferrable skills & experience.

About you:

• Some understanding of working in supported housing or floating support services, with a demonstrable knowledge of Housing and other related Legislation or a relevant qualification

• Experience of managing complex and difficult situations in relation to vulnerable client group(s). 

• Ability to effectively liaise with a range of service providers or agencies in order to establish or improve services for clients. 

• An understanding of the principles underlying a quality service, and customer focused, with proven ability to empower service users. 

• Ability to be self-motivating, work under pressure, and manage time effectively, prioritising different areas of work according to need

About the Role

Chance UK has been delivering early-intervention services across London since 1995. Our longest running programme is our mentoring service where Mentors pick the child up from home and run a 1:1 session in the community, once a week for 2 hours. They use the solution-focused approach to work on building the child’s self-esteem and confidence, set goals and explore new places.

In addition, we are growing a range of community based groups running after school as well as some workshops delivered in schools which will support children with developing social and emotional skills and resilience through activities or skills development.

The balance between 1:1 work and group delivery will change according to services commissioned so the post holder will need to be flexible accordingly.

We are ideally looking for someone for this role who is in easy proximity to South London as we have a number of children South of the river that need support.

You will be working with children and families to provide trauma informed support with the aim of reducing trauma symptoms and rebuild relationships.

KEY RESPONSIBILITY

To work with children (aged 5 – 16) and their families to provide trauma informed support with the aim of reducing trauma symptoms, rebuilding relationships fractured by domestic abuse and helping the non-abusive parent to understand the impact of domestic abuse on their children so they are better able to support them.

DIRECT WORK WITH CHILDREN AND THEIR FAMILIES

• To understand and assess the needs of children and families and devise a child-focused support plan
• To utilise evidence-based approaches to strengthen attachments and rebuilt relationships with the non-abusive parent that have been fractured as a result of domestic abuse
• To utilise trauma informed ways of working to reduce trauma symptoms and distress in children
• To work with families in order increase stability and reduce risk 5
• To assist the non-abusive parent / carer in their understanding of the impact of domestic abuse so that they can respond appropriately to the emotional needs of the children
• To attend and initiate mutli-agency meetings where required

Aside from the 50+ young people we see every day at our day centre, New Horizon’s Outreach service also works with young people on the streets and in the community. The team works pan London to deliver a unique, youth-specific outreach service to young people currently or at risk of rough sleeping. We use our extensive housing expertise to place those young people into accommodation that is appropriate and safe whilst encouraging them to access the day centre to benefit from the wide range of services we offer. The Outreach Worker will work as part of Rough Sleeping Team to deliver at least two street outreach shifts per week and transitional casework to young people facing rough sleeping. You will need to be willing to work early mornings and late evenings, to travel across London to identify young people who need our support, and to work flexibly to respond to evolving needs. The Role

You might have organised in your workplace or local community to take action on issues that you care about. The Marylebone Project is one of the UK’s largest providers of support services to women experiencing homelessness. The women we support have multiple disadvantages and require dedicated services like us to provide them with the right help to get back on their feet. Our purpose is to empower those we work with to take the steps they have identified are needed to achieve their goals. We do this by being person-centred with support and creating a Psychologically informed environment that encourages recovery. Ideally, you will have experience of working within a Psychologically Informed Environment and would have had experience supporting people with complex needs.

Salary

£33,158 per annum

Hours

Average 40 hours a week on a five-week rota. Normal working pattern will be working across early and late shifts & working two weekends. Working hours and rotas are subject to change by the organisation as required, based on operational/service need, though we try not to do this frequently. Some flexibility in working hours is expected.

Pension

The employee will be enrolled into a pension scheme providing the post holder meets the criteria for eligibility. Minimum pension contributions will be paid by the employer for you if you are eligible.

Annual Leave

28 days, including bank holidays, rising by one day a year to a maximum of 33 days after each full year’s service. As the project works on a 24/7 basis, you will be required to work some bank holidays, including Christmas and New Yea

Support Worker

We're looking for a kind, compassionate and resilient Support Worker to join our Hope House service in Slough.

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.
This is a great opportunity for any support worker to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT) We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.

What you'll do:

- Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
- Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
- Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
- Develop and maintain links with all key agencies and service providers in the local community
- Empower customers to ensure they receive the service and benefits they are entitled to
- Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards
- Encourage and enable tenants to pay their rent and to ensure that rent accounts are managed effectively in conjunction with the Housing Management team
- Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.

The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.

The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

What you'll do:

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.
The initial and co-produced assessment will incorporate models of co-production, observation, and person-centred planning. This is with a view that it is solution focused as to provide a framework for progression towards independent living.
Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.
Create support/ action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.
Liaise with partner agencies regarding making referrals where an unmet need has been identified via the multi-agency assessment.
Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.

Provide day-to-day instruction of domestic staff/Personal Support Assistants where appropriate.
