This role is based at our centre a short walk from Clapham Common tube station, in London. We support homeless people from the local area and further afield. Our services are open access, so the people who come to the Ace of Clubs are diverse and from multiple walks of life. KEY DUTIES
- To participate in the provision of advice and advocacy for service users addressing their needs in the following areas; Housing; Welfare; Health; Education; Employment, (this list is not exhaustive);
- Refer to and liaise with partners and agencies within the statutory and voluntary sector to secure positive outcomes for Centre Users, e.g. referring and signposting guests to accommodation providers, health services and other relevant internal and external support services, as necessary.
- Work in a manner which honours our organisational values and is responsive to the trauma and experiences of homelessness and vulnerable adults.
- Maintain essential records accurately on Charity Log, (ensuring high quality case recordings).
- Ensuring the Centre Manager is kept informed of all safeguarding and risk concerns.

As a Sport Coordinator, you'll be responsible for planning and delivering varied activity sessions for both adults and young people across different services, both internally and externally. You will work closely with the Sport Manager to effectively coordinate support from freelancers and volunteers. Your role will extend to broadening the accessibility of sports and exercise within SHP's services and among its residents, for both adults and the young people we support. The Rough Sleeping Team in Oxford provide a rapid response service to support people at immediate risk of rough sleeping. We are currently looking for inspirational people to join the Somewhere Safe to Stay Service (SSTS) team as Assessment and Reconnection Workers where no day is the same. You will work as part of a busy, dedicated team while developing the necessary skills to:
- Provide direct support to people new to, or at risk of rough sleeping; working closely with individuals on assessment action plans which fully involves them in their recovery. To do this you will build positive working relationships with a range of services and individuals across Oxford.
- Work on the weekly shift rota including shifts from Mon – Fri 9am – 5pm, and some weekends and bank holidays.

Overview of Role:
As a Support Worker, you will be working with young people aged 12-25 years old who have experience of self-harm. Within that age group, you will predominantly be working with young people under 18. The post would suit an experienced support worker, preferably with knowledge of the issues experienced by young people who are trans / non-binary.
You will play a key role in providing them with support and advocacy both on a one to-one basis and within a group setting; working with them to develop their emotional skills which will allow them to cope more effectively with the difficulties in their lives. The Amber Project exists to support any young person (aged 14-25) in Cardiff and the surrounding areas who has experience of self-harm. With our Constellation element working with young people aged 12-25. Since The Amber project was established in 2002 individual support and creative
workshops have been key elements of our work with young people.
Salary: £13,406 per annum (£26,812.00 per annum FTE).
Hours: 20 hours per week. Working pattern to be agreed with the line manager but will include late afternoons and early evenings to facilitate meeting with young people outside of school hours and attending workshops. Occasional weekends.

Your mission: to support and empower young people to develop the lifelong tools they need to lead fulfilling lives outside the confines of traditional services. You will carry a caseload of clients who you will formulate a comprehensive support and safety plan with, which forms part of the ongoing casework, promoting independence and preparing young people for moving into independent accommodation, and employment and education.
About the Role:
As the Young Person's Worker, you will be working within a strengths and recovery framework embedded within a trauma informed approach to ensure client needs and aspirations are at the centre of the support planning process. You will be the lead worker overseeing the support and development of the young person’s needs and aspirations. You will guide and signpost the young person to the relevant statutory and non statutory services and will be key in motivating and coaching the young person to develop independent living skills.

The Role
We are currently looking for an enthusiastic, highly motivated, and experienced support worker to join our Young People Support Team based in Hackney. You will provide client directed, person centred support to help young people achieve personal goals and aspirations for independent living, training, employment, and education. You will have an individual caseload but will work alongside a team of Support Workers, with similar client groups, working to the same principles and goals.

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.
This role requires a valid Right to Work for the UK as Look Ahead are not a sponsoring organisation.
What you'll do:
Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.
The initial and co-produced assessment will incorporate models of co-production, observation, and person-centred planning. This is with a view that it is solution focused as to provide a framework for progression towards independent living.
Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.
Create support/ action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.
Liaise with partner agencies regarding making referrals where an unmet need has been identified via the multi-agency assessment.
Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.
Undertake key-working responsibilities for a caseload assigned by the Team Leader/Manager
Support customers to undertake all domestic tasks as past of the assessment of daily living skills, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
Develop and maintain links with all key agencies and service providers in the local community.
Empower customers to ensure they receive the service and benefits they are entitled to, including assessment and support of budgeting and debt management to garner their tenancy readiness.
Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards.
Adhere to Look Ahead's Policies and Procedures
Engage in learning and development activity to increase knowledge and skills.
Provide day-to-day instruction of domestic staff/Personal Support Assistants where appropriate.
Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned.
