We are seeking to recruit a highly skilled Key Worker for our Key Worker Service in East Berkshire. The role will be diverse and service user centred and would suit someone with significant experience in supporting young people and young adults with Autism and Learning Disabilities and their families.
The day to day work will involve a flexible approach to coordination and direct support for the young person and their family, as well as providing responsive communication at times of increased need. This is an exciting role which allows for the Key Worker to support families to navigate the system whilst advocating for them and ensuring that their voice is heard.
As the service is new there will be opportunities for collaboration and co-production and our vision is to provide young person and family centered support, whilst influencing system change to achieve better outcomes for more children, young people and adults under the age of 25.
The service will be provided across the entire East Berkshire footprint covering: Slough, Royal Borough of Windsor and Maidenhead, Bracknell Forest.Although the team will be “virtual”, there will be frequent travel within and across the East Berkshire area, therefore being a car driver with access to a car is essential.
The Key Working Function has been developed as a response to the NHS England & NHS Improvement Long Term Plan (LTP) commitment that by 2023/24, children and young people with a learning disability, autism, or both, with the most complex needs will have a designated Key Worker, applying the recommendations made by Dame Christine Lenehan in ‘These are our Children'.

About the role
As an Engagement and Assessment Worker, you will be the first point of contact for someone who comes to Crisis for help to prevent or end their homelessness. You will:
- welcome them to the service on site and at partner agencies,
- listen carefully and non-judgementally to their story,
- complete sensitively thorough Crisis and Mainstay assessments,
- Agree realistic options and the support needed to assist them to prevent their homelessness or a pathway out of homelessness
- Help them formulate a holistic plan and help them take the initial steps, where necessary.
- Hold a caseload of members who require short term intervention to end their homelessness.
About you
To be successful in this role you will have:
- Experience of working successfully with disadvantaged/or socially excluded groups and individuals, utilising a psychologically informed approach.
- Ability to deliver excellent customer service, ensuring accessible, welcoming and safe ‘front of house’ environment in a busy service.
- Experience of assessing presenting needs, and an understanding of strengths-based assessments. Empathy and understanding of the barriers to accessing services.
- Ability to manage successful relationships with external agencies and relevant service providers and successfully advocate on members behalf.
- Excellent verbal and written communication skills used to ensure accurate record keeping on a variety of Case Management Systems/Databases.

YMCA West Sussex Transitional housing provides safe, secure accommodation for young people aged 18-25, who have been, or are at risk of homelessness. Our staff team comprises of Supported Housing Support Workers, Mobile Night Workers and two Managers. We offer a support package alongside the accommodation to actively encourage residents into education, training, employment or voluntary work.
In delivering the role, you will work with the Deputy Supported Housing Manager and the wider housing team to provide effective day to day management of our transitional accommodation service. You will have your own caseload of residents, working creatively and effectively alongside young people to develop their assets and talents and to acquire the skills they need to live independent and fulfilling lives. You will contribute to the smooth and safe running of the service by providing consistency and reliability. Windsor Road service offers 11 beds to people experiencing mental ill health. We are a CQC registered residential care home. We offer individual one to one interventions and sessions such as social anxiety, anxiety, finance and budgeting, positive self-image, building self-esteem, support with diet and nutrition, healthy eating, sleep hygiene, DBT skills, building resilience, life skills, solution-based therapy, stress management etc. We offer a range of activities in our service planner including a daily coffee morning, a ‘here and now’ group, community gardening, cooking, arts and crafts, social inclusion and family and friends’ days. We support our residents with daily living skills where this is needed, such as support with shopping and cleaning tasks, this could include prompting or supporting alongside them. We also work closely with other agencies and our residents wider care teams, so good communication skills are required. And of course, all of this is recorded on our information management systems, so good record keeping and documentation is also essential. Our service consists of 8 24/7 Recovery Workers, a Service Manager and Registered Manager.
We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in. Responsible for helping and inspiring the people who use our services to achieve independence in the community, we’ll rely on you to work with them to come up with a person-centred support plan that will see them achieve their goals and aspirations, as part of your key worker role. Whether it’s developing their domestic and finance management skills or accessing work or volunteering or educational opportunities, one thing’s for sure ‐ you will have every chance to shine.
Care home, mental health support or social care experience is preferred. You will be required to administer medication and complete a medication qualification and the care certificate. Candidates with an understanding of mental health issues and/or lived experience would be welcomed. More important is your caring and compassionate nature and empathy and enthusiasm for helping others. Our staff team comprises of Support Workers, Night staff and two Managers. Each Support Worker holds a caseload of residents and meets with them weekly to build a support plan and help them achieve their goals.
In delivering the role, you will work on a rota shift pattern, ensuring young people at the service have non-judgemental, objective, and supportive staff during the day. You will work with the Managers and the wider housing team to provide effective day to day management of our accommodation project. This includes delivering one to one sessions and group work sessions to assist residents to develop personally and socially in order that they grow in confidence and move towards independence. You will help to contribute to a great working environment, with a calm, yet assertive manner, being able to handle potentially difficult situations. You will also support young people to safeguard themselves against abuse and risk.
Based on site, this role is on a seven-day rolling rota, including weekends and bank holidays. There is a small amount of lone working, but you will get to know the team and project, along with extensive training prior to starting on a rota.

Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. About the Service
Our refuges provide emergency accommodation to women and children fleeing domestic abuse and sexual violence, last year nearly 1000 women and children found a safe place to stay in one of our refuges. Refuge Case workers provide emotional and practical support this can include housing support, legal options, reporting to the police, benefits, financial matters and accessing therapeutic support services.
About the Role
You will be a keyworker for a caseload of (on average 6-10 women) living in or moving on from the refuge. You will be with them from the start of their journey to the end, welcoming and helping them settle when they arrive to when they are ready to move on to their next home. This includes carrying out risk and needs assessments and developing support plans. Your work with service users will ensure their practical needs as well as emotional needs are taken care of. You also provide a vital role in keeping the service a safe and secure environment for the women and children who now call the refuge home. It is a face paced, target driven role.
About You
Key to this role is experience of working/volunteering with anyone who has experienced any form of social disadvantage. A knowledge of domestic abuse and its impact on women and their children is important as well as an ability to build supportive and empowering relationships. You must be open and willing to learn new skills which will build on your working practice with service users to co-develop and deliver support planning, risk assessment & linking in with other agencies where needed.
Knowledge and experience of risk assessing, case management and legal remedies, housing and welfare benefits is ideal however not required as we are confident that we can train around these.
