Last year we saw a 133% growth in the numbers of young people facing homelessness coming through our doors. Now, nearly half of them are - or recently have been - sleeping rough and with the numbers of young rough sleepers in the capital growing at an alarming rate, New Horizon Youth Centre has had to take decisive action to unlock support and housing for this group. The Head of Services - Rough Sleeping is a brand-new role for the organisation and will lead on developing a new internal strategy for young people facing rough sleeping, as well as holding operational oversight of our established youth-specific street outreach function and emergency accommodation hub, both of which are unique in London. You will need to be strategic in approach and an excellent relationship builder. You will act as our lead colleague on relationships with the Greater London Authority and London Councils’ rough sleeping teams, working with them to ensure our learning and insights inform policy and ultimately open up access to emergency support for those under 25. Alongside this you will need a keen eye for operational detail, have experience of delivering homelessness services, and be passionate about piloting new solutions to prevent homelessness among young people. More than anything you will need to champion what young people can achieve, be committed to collaborating with others and determined to find a way.

Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.

The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across london on a spot purchase basis.

Specialist Support workers will manage a caseload of customers to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

Consists of working 5 days a week across all days, 8 hours shifts starting at either 8am or 2pm.

Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. The services are short stay, and staff provide wrap around support and liaison with several external partners, including physical health, mental health, substance use and housing. Staffed 24 hours to ensure this quick turnaround, they are a key service to ensure people are off the streets and in stable accommodation.

The vacancy

Could you be a Caseworker for vulnerable young people? Could you give them the support and guidance they need to get back on their feet and live independently?

The young people we work with have become homeless for a number of reasons – isolation or mental ill health, abusive relationships, substance misuse, involvement in gangs/county lines - their issues are complex so you will need to be able to build strong, trusting relationships so that our customers feel safe and supported. 

We are opening two new supported housing schemes in Lewisham.  Both schemes are part of our well-established Young Persons and Complex Needs service in London and will provide temporary housing for young people aged 16-17 who are experiencing homelessness.

A bit about the role:

As a Caseworker, you will be working with a team of highly motivated, compassionate, and dedicated colleagues supporting a caseload of young customers with complex needs. You’ll need to meet each customer weekly to help them with their goals and discuss their progress. And, when someone’s ready to move on, we'll rely on you to help them access the housing register or look for suitable rented accommodation.

What we’re looking for:

You may have worked with vulnerable young people in a paid or voluntary capacity, maybe as a Youth Worker or in an education setting providing pastoral care. You’re a great relationship builder, who can create trust so that our young people feel comfortable sharing their issues with you, you listen but in a non-judgemental way and you are able to spot signs for concern and act on them.

You’ll also need to be able to work on your own, sometimes dealing with challenging situations and diffusing them to avoid escalation. Is this you?

We are seeking an Advanced Recovery Practitioner to support in the Women and Families team, within Mind in the City, Hackney and Waltham Forest, working in partnership with Turning Point and Antidote.  The successful candidate must have demonstrable experience in working within a substance misuse treatment and recovery setting, must have specialist training and supervision in one or more complexities alongside substance misuse. This may be mental health, long term conditions, criminal justice, offending behaviour, homelessness, motivational interviewing, safeguarding, family work.  Have an understanding of risk and risk management, as it applies to substance misuse and complexity.  Have a working knowledge of Safeguarding of Vulnerable Adults framework, the ability to follow procedure and development of a culture of safe practice.  Ability to work under pressure and have significant experience of working in health and social care.

Mind in Croydon is a charity concerned with the welfare of people with mental health problems living in the London Borough of Croydon and the surrounding areas. Mind in Croydon delivers Mental Health Transformation Program services alongside Croydon BME Forum, these include Croydon Health and Wellbeing Space and Mental Health Personal Independence Coordinators. Other services include Recovery Space, Advocacy, Active Minds, Carers Support, Peer Support In-Reach, Social Networking, Counselling, and an Information Service.

This exciting role will manage the provision of a non-clinical Recovery Space (crisis café) funded by South London Partnership Commissioning. This project operates in the day and evenings, people are referred by our Trusted Assessors in various Teams such as Croydon University Hospital, Psychiatric Liaison, Assessment and Liaison, Croydon College, Home Treatment Team and others.

The aim of the service is to divert people, who would be better supported elsewhere, away from Accident and Emergency (A&E) and towards services that can support with their immediate crisis within the Borough of Croydon. The Recovery Space Service Manager leads this service which is non-clinical but a warm and welcoming setting to support and hold individuals during the day and evening. The Recovery Space service referrals will not be accepted after 10pm for attendance at night.

The role will work closely at senior operational level with external partners, clinical, commissioning, and voluntary sector organisations to plan, develop and promote the highest quality service. They will manage a team of nine staff, three are Crisis Workers, operating a day service, one outreach crisis worker also operating during the day and five Recovery Space Workers in the evening.  The role will need to produce reports, develop, and implement policies and procedures in line with best practice and to ensure the service is making a difference to people’s lives.  The role will work with other service managers including those managers within the Transformation services to ensure a One Team approach to provide an effective and efficient service for clients.

The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.
• To participate in internal/external meetings as required, attend training events, conferences and other functions as necessary.
• To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
• To ensure that all The Passage policies and procedures are being adhered to, particularly those relating to Code of Practice and Confidentiality.
• To contribute to the effective implementation of The Passage’s Diversity and Equality Policy as it affects both The Passage and its work with vulnerable adults.
• To at all times undertake your role in a professional manner maintaining a high quality standard of work, and to always work in accordance with the aims, values and ethos of The Passage.

Desired experience:

• Significant experience of delivering support services to socially excluded clients in a street based/day centre or similar setting.
• Significant experience of working in a mental health and/or substance misuse setting, working collaboratively to achieve positive outcomes for clients.
• Experience of providing a fully sensitive and responsive service to clients in a psychologically informed manner.
• Experience of working cooperatively with external partner agencies, e.g. Police, Social Services, NHS, as well as housing providers and voluntary agencies.
• Experience of assessing client needs and supporting clients to address their needs through assertive outreach and case management.
• Experience of working in a client-centered recovery focused approach and the necessary professional social skills to initiate and maintain constructive and appropriate relations with clients and agencies.

• Understanding the support needs of the street populations with complex health, and housing needs.
• Knowledge of services and legislative environment regarding housing, health, work and immigration as it relates to people experiencing homelessness.

• Knowledge of Psychologically Informed Frameworks.
• An understanding of the causes of homelessness and, in particular, the needs of people who are multiply-disadvantaged in their homelessness.
• Knowledge of housing pathways available to dual diagnosis clients and how to access them.
• A working knowledge of how to support people with either substance or mental health needs.

Organisation Overview

CDS UK (the Clinic for Dissociative Studies) is a psychotherapy service, established 25 years ago to provide assessment and long-term specialist outpatient treatment to people with dissociative disorders. Its patients have often suffered the most extreme forms of abuse over a long period, and represent one of the most vulnerable and complex client groups. CDS UK is a Charitable Incorporated Organisation (CIO) and an Independent Provider to the NHS, and is subcontracted by NHS commissioning organisations nationally on a discretionary perpatient basis, where local mental health services are unable to provide appropriate treatment. CDS UK has a small core team of employees, mainly based around London, and a larger group of associate therapists, support workers and supervisors working throughout England and in Scotland. The organisation has seen steady growth in referrals over the past few years.

Job Summary

One of the key elements of the role will be to manage a caseload of referrals from receipt of referral through to submission of funding application and referral outcome. The postholder will be one of two people within CDS UK’s core team with responsibility for this. 

There will also be an allocations management function, involving matching new (and/or existing) patients with suitable clinical staff, usually from within CDS UK’s group of associates, but sometimes via the recruitment of new practitioners.

The role will include a range of other tasks key to the successful functioning and development of CDS UK. Amongst these are maintaining a log of, and costing where appropriate, allpotential service developments (from requests for training to submissions to funders), developing reporting from basic information systems, checking and sending of patient progress reports and monitoring of patient funding. They also include developing our outgoing communications – such as via our website – and helping to explore opportunities for developing our charitable arm.

As part of our small Business and Administrative Team, the role will encompass some ad hoc administrative tasks, and may require delegation of some tasks and/or some line management duties as required.

The post-holder will work on their own initiative and independently deal with issues of workload and problem-solving, becoming a specialist in their areas of work within an agreed period, supported with appropriate training if required. This will include responsibility for analysing, interpreting and comparing a range of complex material, proactively reporting on key metrics, and helping to develop systems and procedures to serve the aims of the organisation.
