Youth Outreach and Engagement Worker

Streets of Growth is seeking to employ an experienced, motivated, and personable individual to work as part of the frontline team to deliver targeted street work to engage and build relationships of trust with high-risk vulnerable young people (15-25 years), in hotspot areas known for Anti-Social Behaviour (ASB), exploitation, serious youth violence and young people that are furthest away from engaging in services.

You should expect to be appointed at the starting point of the salary scale and a higher starting salary band is dependent on you having extensive experience and skills in street work and detached outreach work as well as relevant frontline experience of working with at-risk young people.

These working hours will vary and will be worked according to business needs and notified to you in advance. You will be required to work late afternoons, evenings, and weekends as part of your normal working week.

The role:

Delivering targeted street work and outreach across the borough of Tower Hamlets. This involves engaging and building relationships of trust with young people at street level and in identified hotspot areas of risk where there is low to high anti-social behaviour, criminality and violence. Working with and engaging young people in strategic interventions across Lifestyle, Enterprise/Employability, Education and Environment that constructively challenge negative patterns of behaviour, create an experience for positive thinking, confidence building, entrepreneurial work skills, and tapping into the young person’s ambition, potential and talent.

You will be working alongside Intervention Coaches to progress street-engaged young people into Streets of Growth’s staged-based Appropriate Intervention Bridging Programme delivered from our flagship Centre based in Aldgate East and delivering appropriate targeted intervention work at the street/estate level during critical incident situations in the community.

Who we are looking for:

Along with a relevant qualification up to at least Level 2, you will have robust frontline experience working with young people, including experience of detached outreach work with young people. You will also have good knowledge of safeguarding risk assessment and contextual safeguarding. A key component of the job role is the ability to engage and build transitional relationships with young people, most particularly hard-to-reach individuals.

The Community Health and Wellbeing Worker will speak to households about health and wellbeing, alongside offering support to connect people to services and activities in their local community. Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project. 

The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

About the role:

As a Resettlement Worker and Domestic Abuse Specialist, you will work in person and remotely with clients who have support needs, in particular, those who are survivors of domestic abuse. Main duties:

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. About the Role:

Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. The role requires individuals to work on a rota pattern including weekend and bank holiday working, as the service is open 365 days a year. Hours worked: 21:30 - 07:30.

A bit about the role:

As a Outreach Support Worker, you will provide a service where planned support and crisis intervention is delivered to vulnerable people with low, medium and high support needs living in independent accommodation with the aim of them maintaining their home.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.

ABOUT THE ROLE

A new opportunity has arisen to join a new contract in Lewisham. You will support over 2 properties which supports residents and participants with acute and/or complex mental health needs who are supported/treated in the community in their recovery as appose to hospital inpatient care. You will focus on rehabilitating and reintegrating our residents and participants and help our residents and participants achieve their personal goals and gain a level of recovery that enables them to return home or move into supported housing.

Shift Pattern: Rota based Monday to Sunday between 08:00 and 19:30 

ABOUT THE SERVICE

The Integrated Community Rehabilitation Services will enable people to receive a community-based offer and avoid admission into a rehabilitation unit. 

The service is across 2 properties in Lewisham (one 8 self contained rooms, and another with 6 self contained rooms) located in close proximity to each another and both will be staffed 24/7 by a minimum of 2 Mental Health Recovery Workers (MHRW) at all times, at each site. This will include Shift Leads to support the Management Team.

The Senior Multiple Needs Independent Domestic Violence Advisor (IDVA) will work within a dynamic, crisis intervention, advocacy and support service based at the CGL Ealing Women’s Wellness Zone.  The IDVA will provide support in the local community and at the Ealing Women’s Wellness Zone.  The successful candidate will be co-located up to 5 days a week at the Women’s Wellness Zone.

The IDVA will be closely working alongside CGL colleagues/team to deliver support to women with a range of complex support needs including substance misuse, mental health, domestic abuse, sex working, trafficking, and offending behaviour. The IDVA will work in collaboration with their team members to holistically support women in the borough of Ealing to achieve sustained recovery, improved health and wellbeing, helping women to manage/mitigate risks, and empowering them to reach their fullest potential in society as well as ensuring the voice of survivors informs every stage of the process.

The Senior Multiple Needs IDVA will assess the needs and risks of survivors, carry out short and longer-term risk management, safety planning and support. As well as identify and refer to services appropriate to their needs. They will also proactively advocate and advice women of their rights and options for seeking help and support from other agencies, encouraging them to engage with other agencies, and help co-ordinate the provision of multi-agency support where necessary.

About You:

 

To be successful as the Senior Multiple Needs IDVA you will need to have:

Excellent understanding of domestic abuse and its effects on women and children, of best practice in supporting survivors of domestic abuse, and of the MARAC. As well as understanding of drug and alcohol issues and experience of working within a related field.

Bringing your experience of risk and needs assessment, safety and support planning, and crisis planning and successfully managing high numbers of referrals and experience of working in a co-located setting with another agency and providing consultations/briefings to professionals to support in upskilling them around Domestic Abuse. The candidate will have experience of partnership working and of maintaining excellent working relationships with a range of stakeholders.

This is an exciting opportunity for you to work as part of an innovative service, designed to offer multi-disciplinary wrap-around support to women with multiple needs, with the aim of facilitating long-term recovery.
