Integrated Care Facilitator – Band 4

Plymouth, Devon

£25,601 - £28,095 (Pro rata)

12 Fixed Term Contract

Part time (22.5 hours per week, weekdays only)

Closing Date: 2nd May 2025

1st Interview Date: 14 May 2025

2nd Interview Date: TBA

St Lukes Hospice is looking for an Integrated Care Facilitator, who has experience working within health and social care, who has a passion to use this knowledge to enhance the lives of our Palliative care patients and their significant others.

About the role

As their Integrated Care Facilitator, you will use your specialist knowledge of the health and social care system to support your clinical colleagues within the Hospice, primary care and wider healthcare providers. You will be working directly with patients and their significant others in the communities where they live, helping to develop a deep understanding of the many local initiatives which can support them in a more creative way.

You will be pivotal in helping us develop and evaluate this exciting role, and your views and experience will be critical in helping us be the very best at what we do. We recognise the critical impact and challenges current health and social care provision has on our patients and their significant others, and we are looking for someone who can maximise the support and choice for our patients, from referral through to end of life.

We are looking for someone who uses their own imitative and is able to work as part of a team, who can provide support across our 3 community hubs.

About you

It is your values that are most important to us. Experience of working within the clinical setting and/or specialist palliative care would be an advantage but not essential and training will be given to the right individuals. Knowledge of care provision across Plymouth and South Hams and West Devon would be a particular advantage.

St Luke’s is a friendly, dynamic, and progressive organisation, where innovation and a ‘can-do’ ethos are championed. We want our team to represent the diverse community we serve and have a strong track record in supporting flexible working and developing people. Our One Team approach values everyone’s contribution and we look forward to welcoming you to the team.

As a staff member of St Luke’s you will receive an attractive employment package, which includes 7 weeks annual leave rising with service (inc. bank hols) and membership to a cash back health plan.

We would encourage informal discussions regarding this post with our Clinical Lead or Deputy Clinical Leads for End of Life Urgent Care Service.

St Luke’s are committed to equality of opportunity, to being fair and inclusive, and to being a place where everyone can bring their whole selves to work. We therefore particularly encourage applications from candidates who are likely to be underrepresented in St Luke’s workforce. These include people from Black, Asian and minority ethnic backgrounds, disabled people and LGBTQ+ people.

Please note St Luke's Hospice Plymouth does not possess a license to sponsor individuals to work in the United Kingdom.

We reserve the right to close this vacancy early if we receive sufficient applications for the role. Therefore, if you are interested, please submit your application as early as possible.
