Support Coordinator
We’re looking for an innovative, compassionate and professional person with strong communication and organisational skills to join the Stroke Recovery Service in Herefordshire and Worcestershire.
Position: 000012 Stroke Support Coordinator
Location: Home-based covering Herefordshire and Worcestershire. However, this role will require extensive travel across the delivery area. Travel will also be required for team meetings and other work-related meetings
Hours: Part-time, 28 hours per week
Salary: Circa £22,600 per annum (FTE circa £28,300 per annum)
Contract: This is a fixed-term contract until 31st March 2027. Services are contracted and there is currently funding for this contract until 31st March 2027

The Role
In this role, you’ll support stroke survivors and their families as they adjust to life after stroke. You’ll help identify their needs, provide personalised information, advice and support, and work with them to achieve the outcomes that matter most to them.
Reporting to the Stroke Service Delivery Coach, the Stroke Support Coordinator will:
· Support stroke survivors and carers as they move from hospital discharge back into the community.
· Use a person-centred approach to help stroke survivors and carers set goals and access the right support.
· Provide clear, tailored information, advice and practical support.
· Encourage stroke survivors to make informed lifestyle changes that may reduce the risk of further strokes.
· Work closely with NHS colleagues and community partners to improve the lives of people affected by stroke.
About You
You will:
· Have a background in a caring and/or charity profession supporting people with disabilities.
· Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
· Have experience of working with people who may have additional communication support needs.
· Be proficient in using Microsoft applications, including Word and Excel and experience using IT systems to record and maintain beneficiary data.
This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.

Would you like to make a positive difference to the lives of survivors of torture? Do you have experience of coordinating clinical services delivery to marginalised groups in a community setting?
We have an exciting opportunity for a Clinical/Counselling Psychologist to work with an innovative and friendly multidisciplinary team at our London Centre, which is a purpose-built centre specifically designed for the treatment of torture survivors, with extensive indoor space and gardens.
About the Role
Our team of experienced clinicians offer a range of specialist therapy interventions, to survivors of torture and organised violence, on an individual and group therapy basis. The interventions equip survivors with the resolve to deal with the physical and psychological impacts of their experiences. Throughout the rehabilitative journey, we closely monitor the impact of the interventions as it relates to each survivor using specifically designed Outcome Tools, which in turn assist in ensuring that the survivors receive the best treatment possible.
About You
To be successful in this role you must be qualified in Clinical or Counselling Psychology, (you will be trained to doctorate level or an equivalent vocational level 7). If you are nearing the completion of your training in either discipline, we would still love to hear from you. You must have knowledge of at least one evidence-based approach to trauma therapy including trauma focused CBT, NET and EMDR, sufficient to be able to deploy this model clinically and determine appropriate intervention pathways. It is also essential that you are registered to practice as a psychologist with HCPC. Due to the psychological trauma experienced by our clients you must have up to date knowledge of relevant mental health and safeguarding legislation. Your role will include offering supervision to trainees.

We're looking for a compassionate, proactive and organised Refugee Resettlement Support Worker to join our Resettlement Team and support individuals and families arriving through Government resettlement schemes. The role is predominantly focused on families resettled from Afghanistan, although support may be provided to people arriving through other schemes as well.

You'll work directly alongside refugee families as they settle into life in Bournemouth, Christchurch, Poole and wider Dorset, helping them navigate unfamiliar systems, access services and become active members of their new communities.

The Southmead Project is seeking an experienced and motivated Fundraiser to support the charity’s long-term financial sustainability.
The postholder will identify, research and secure funding from trusts, foundations, charitable bodies, statutory funders and other appropriate sources. They will lead on developing high-quality, persuasive funding applications and reports that clearly communicate the impact of the Southmead Project’s work and the needs of the people it supports.

You will manage a caseload of female survivors within the National Referral Mechanism, advocating on their behalf to ensure access to ECAT rights and supporting them to navigate complex systems. Your role includes conducting regular safeguarding risk assessments, maintaining detailed case records, and responding promptly to incidents or urgent issues to protect residents' safety and wellbeing. You will carry out daily, weekly, and monthly health and safety checks to ensure the accommodation remains secure, while also covering other safe houses as needed and supporting new staff members. Your responsibilities extend to liaising confidently with public and private services, writing official correspondence to agencies such as the Home Office, and keeping up to date with relevant legislation, policies, and child welfare considerations. Additionally, you will participate in team meetings, supervisions, clinical support sessions, and mandatory training, all while working to help survivors maintain positive and safe relationships with their children.

Role Overview
The Chief Operating Officer (COO) is a key member of HCPT’s Senior Leadership Team, responsible for the effective delivery and high performance of all operational and business support aspects of the charity’s work. Reach Charity is a small volunteer-led national charity supporting children born with, or who acquire upper limb differences (ULD) in childhood, for the whole of their lives! Being born with/acquiring an ULD in childhood is rare. Our role in the Reach team is to support our volunteers in every way we can to keep Reach families connected and supported with and by their community as and when they need it for life. Core purpose of the role:
The Operations & Development Officer will provide high-quality operational, administrative and executive support to the Operations Lead and the wider Reach team.
The role will help ensure the smooth running of Reach’s central operations, including governance support, policy coordination, safeguarding administration, donor stewardship, event delivery, communications and day-to-day organisational support.
The postholder will play an important role in maintaining operational continuity, supporting compliance, strengthening community engagement and helping Reach deliver effectively for adults, children and young people with upper limb difference and their families.
Key responsibilities
1. Governance, compliance and safeguarding
Policy and procedure coordination
Coordinate and maintain a rolling programme of policy and procedure review, liaising with the Operations Lead, trustees and relevant staff or volunteers as required.
Support the updating, formatting, circulation and storage of policies and procedures, ensuring records are accurate and accessible.
Safeguarding support
Subject to appropriate training, support Reach’s safeguarding arrangements and participate in the safeguarding on-call rota.
Provide cover during colleagues planned or unplanned absence, acting in accordance with Reach’s safeguarding policy and escalation procedures.
This may include occasional weekend on-call duties when the Operations Lead is unavailable.
DBS, safeguarding training and appointment requirements
Appointment will be subject to satisfactory references and the appropriate level of DBS check for the role.
Safeguarding training must be completed before the postholder undertakes safeguarding duties.
Data protection and confidentiality
Handle personal, sensitive and confidential information in accordance with UK GDPR, Reach policies and confidentiality requirements.

2. Donor relations and fundraising administration
Donor stewardship
Provide supportive cover for donor stewardship during colleague absence or holiday, including drafting thank-you communications, updating records and helping maintain positive supporter relationships.
Ensure donor communications are timely, accurate and aligned with Reach’s tone, values and messaging.
Fundraising administration
Support the Operations Lead and wider team with fundraising administration, including funding applications, milestone tracking, data collection and information required for funder reports.
Maintain accurate records of funding activity, deadlines and reporting requirements.
3. Event support and promotion
Event delivery
Support the planning, coordination, delivery and evaluation of Reach events.
This may include support from initial concept through to on-the-day coordination and post-event follow-up.

Event communications and promotion
Promote events and campaigns through digital channels, newsletters, local networks and other appropriate platforms to maximise visibility, attendance, engagement and donations.
