About the Role
Rough sleeping pathways only work when the system around people moves with urgency, consistency and accountability. This role is about making sure people sleeping rough, or at risk of returning to the streets, are not left waiting, drifting between services, or losing accommodation because barriers have not been gripped quickly enough.
As Outreach Housing Pathway Coordinator, you will work within Camden’s integrated Rough Sleeping and Outreach Hub to help people access and sustain appropriate accommodation. You will hold oversight of where people are in their housing journey, identify what is blocking progress, and work with outreach teams, Navigators, hostels, pathway providers, community safety partners and statutory services to keep plans moving.
You will focus on the points where pathways can break down: delays, exclusions, unsuitable offers, unresolved risks, street activity linked to accommodation, or support needs that have not yet been fully understood or responded to. Through assertive engagement, practical case coordination and strong partnership working, you will help people stabilise their accommodation and reduce the risk of returning to rough sleeping.
Working within Camden’s No Wrong Door model, you will bring grip, judgement and persistence to complex situations, ensuring people experiencing multiple disadvantage receive a response that is joined-up, trauma-informed and focused on lasting routes away from the street. SHP will support you with the training, reflective practice and development opportunities needed to continue building your expertise in this work.

We're looking for a compassionate, proactive and organised Refugee Resettlement Support Worker to join our Resettlement Team and support individuals and families arriving through Government resettlement schemes. An experienced Women’s Housing Project Worker dedicated to delivering outreach-led, trauma-informed support for women who are homeless, at risk of homelessness, experiencing domestic abuse or facing multiple disadvantage.
The role will focus on building safe and trusting relationships in community settings, with additional support delivered within the centre where appropriate, helping women access services, address housing-related barriers and work towards safe accommodation, tenancy sustainment, wellbeing and independence. The post provides practical housing-related support, signposting, liaison with partner agencies and sustained casework.

We are seeking a flexible, reliable, and enthusiastic Sessional Worker to join our Young Carers team. The primary focus of this role is to provide consistent support at our weekly Young Carers After School Club, which takes place every Thursday during term time from 4:30pm – 6:00pm. The club is delivered across two age groups (5–11 and 12–16 years), and regular attendance and commitment to supporting both groups each week is an essential requirement of the role.
You will work alongside the Young Carers team to deliver engaging, age-appropriate activities that promote wellbeing, reduce isolation, and provide children and young people with a fun, safe, and supportive break from their caring responsibilities. You will encourage participation, build positive relationships, support behaviour where required, and help ensure every young person feels welcomed, included, and valued.
You will also play an active role in planning and preparing for the After School Club as well as assisting with the set-up before each club and the clear-up afterwards to ensure the space is safe, organised and welcoming.
In addition to the weekly After School Club, you will support the delivery of our school holiday programme, including trips, workshops, activity days, and special events, subject to your availability. This will include contacting families, promoting upcoming activities, recruiting and booking young carers onto both the After School Club and holiday activities, supporting attendance, and helping to ensure activities are well attended by those who would benefit most.
From time to time, you may also support outreach and engagement activities, including attending community events, working with local schools and partner organisations, and helping to identify hidden Young Carers so they can access the support and opportunities available through our service.
This is a sessional role, therefore hours are not guaranteed but will be offered based on project need.

In response to feedback from parents and carers, the PACS project was set up in 2023 to assist Parents and Carers identifying their child as ‘at risk’. Since this the service has gone from strength to strength with Parents and Carers engaging from all 10 GM boroughs. We have securing funding to continue this project until 2027 with potential for further extensions.
Oasis is recruiting a PAC Support Worker to be part of the PAC Project.
As a PAC Support Worker you will;
· Support parent and carers that identify their child is “at risk” offering them advice and guidance, practical support and linking them in with services/groups that will provide them with a sustainable support network.
· Help parents and carers to develop projects to raise awareness of the issues they are facing, offering peer support and influence policy and system change.
· Be trained in Non-Violent Resistance (NVR) and other parenting approaches to support parents and carers to respond to their children’s needs, de-escalate situations and maintain good relationships within their home.
· Delivering our ‘Encounter’ Parenting Training (based on the principles of NVR) online and in-person to groups of parents and host online and in-person support groups for parents and carers.

Real change rarely happens in a straight line. At our Ashley Road project, you’ll work alongside people who have experienced homelessness, trauma and multiple disadvantage, offering the consistency, practical support and belief that can help them begin moving forward.
As a Project Worker, you’ll hold a caseload and build purposeful, trusting relationships with clients. You’ll complete assessments, develop person-centred support and safety plans, and help people identify goals that feel meaningful and achievable to them. This could include supporting someone to improve their health, address substance use, manage their accommodation, strengthen their daily living skills or reconnect with services and opportunities in their community.
You’ll be there when situations become challenging, responding calmly to risk, safeguarding concerns and periods of crisis while continuing to recognise each person’s strengths and potential. Working closely with colleagues and external partners, you’ll advocate for clients, coordinate joined-up support and help remove the barriers that have previously prevented them from accessing the help they need.
This is a hands-on and varied role where no two days will look the same. Homelessness Outreach Support Worker
We are seeking someone who can build trust, support people experiencing complex needs and help them move away from rough sleeping.

About the role
This is a hands-on outreach role providing intensive, end-to-end support to people who are rough sleeping and experiencing complex, interlinked challenges. These may include housing, substance use, offending behaviour and physical or mental health needs.
Working with individuals on the street and in the community, you will provide consistent, personalised support to help them access accommodation, healthcare, treatment, benefits and other essential services.

Sounds great, what will I be doing?
Providing admin and other support to colleagues, particularly in relation to compiling documents relating to residents medication, curfews and sign-in registers. Completing welfare checks on residents in the Approved Premises. Ensuring timely and accurate recording of information on electronic databases, and completing tasks to a high standard. Following protocols, encouraging compliance with curfews/drug & alcohol testing, and liaising with partners including on-call manager, probation and police. Ensuring that colleagues, multi-agency partners and management are informed and updated on matters as is necessary for risk management and safeguarding duties. Actively monitoring and maintaining the health, safety, security, cleanliness and hygiene of the Approved Premises and the well-being of its occupants utilising information sharing, CCTV, direct observation, inspection, personal contact and other methods, as appropriate. Ensuring that all related administration is done in good time; that detailed, concise and accurate records are created contemporaneously, that information is shared appropriately, and that confidentiality is properly maintained. Too many women are lost in the gap between being offered accommodation and feeling safe enough to accept it, trust it and stay. As our new Assertive Outreach and Transitions Worker, you’ll stand alongside women at this critical point, making sure a move away from rough sleeping feels possible, supported and led by their choices.
Working across the streets, community settings and our Women’s Recovery Service, you’ll begin building relationships with women before they move in. You’ll carry out flexible, trauma-informed assessments, help remove barriers to accommodation and provide steady, practical support throughout the transition into the service.
You’ll also hold a small caseload of women who need a more flexible or assertive approach to remain connected, use their bedspace and sustain their accommodation. Whether you’re advocating for access to health or domestic abuse support, responding to safeguarding concerns or bringing partners together around a woman, you’ll balance safety with dignity, choice and voice.
This is a brand new role with real scope to shape how we support women across the Camden Homeless Pathway. Your persistence, creativity and belief in women’s capacity for change will help prevent avoidable evictions, reduce repeat rough sleeping and give women a stronger foundation from which to recover and move forward. In addition to the around 40 young people we see every day out our centre, New Horizon provides the only youth-specific street outreach service to young people facing homelessness in London. We use our extensive housing expertise to place young people who are currently or at risk of rough sleeping into accommodation that is appropriate and safe, whilst encouraging them to access the day centre to benefit from the wide range of services we offer. This role will involve regular street outreach shifts (early mornings and late evenings) across London, aswell as supporting young people at the day centre. The role works as part of our wider Rough Sleeping team, developing a best practice approach to supporting young rough sleepers and those who are hidden homeless.

Key objectives:

Main tasks and responsibilities
Young people services
Providing comprehensive support through 1-1 and small group work with young people sleeping rough. This will include:
Working within the Rough Sleeping team to ensure young people receive a high-quality service from the point of registration to move-on.
Carrying out at least two street-based outreach shifts each week (early morning and late evening required).
Carrying out housing assessments with young people and referring them into appropriate accommodation.
Managing a transitional case load of young people and helping them move forward and access appropriate services.
Working in partnership with the Outreach Team and other external teams, such as commissioned street outreach services, mental health, and substance misuse services.
Providing expert coaching and guidance to young people and monitoring and evaluating their progress.
Continued use of feedback and showing a commitment to a service that is sensitive and responsive to young people’s needs, including:
· Completing registrations, outcomes assessments, risk assessments and action plans with young people.
· Ensuring that all relevant work is recorded on the New Horizon database system and Chain database.
· Ensuring risk management and assessment procedures are followed.
Delivering advocacy and referrals to appropriate services, including making internal referrals
Contributing to the management and development of the team and day centre, including attending daily handovers, ensuring that Health and Safety issues are addressed at all times, attending Reflective Practice and team meetings.
Taking a hands-on approach to working with young people
Participating in the training and supervision of new staff, volunteers, students and locum workers and actively promoting a learning and development environment.
Networking and Liasion
To proactively liaise, communicate and negotiate with internal specialists and external agencies. This will include:
Seeking out and responding to opportunities with external partners who might offer resources, funding, or individual services relevant to young people’s needs e.g, Local Authorities, Social Services, Community Psychiatric Services, DWP, drugs and alcohol support, and other voluntary agencies.
Promoting and representing New Horizon Youth Centre at relevant forums and meetings.
Information management
To produce and maintain accurate and useful information in a range of formats in order to promote effective service delivery and evaluation. This will include:
Producing written reports in a variety of formats to meet the requirements of New Horizon, external service providers and funders, e.g., project reviews and assessment reports.
Inputting and extracting information from client monitoring systems other accounting and database systems. Utilising other relevant software for the production of correspondence and reports.
Being self-servicing in day-to-day administration and following team and NHYC’s administrative procedures.
Reviewing personal and professional development needs
To be proactive in reviewing and evaluating own performance and identifying and acting upon areas for improvement. This will include:
Continuously reviewing own working practices in line with feedback and current best practice.
Taking a proactive approach to supervision and attending clinical supervision, weekly team meetings as well as line management supervision every 4-6 weeks.
Reviewing and evaluating own performance to identify strengths and areas for development.
Undertaking development and training opportunities and being responsible for obtaining maximum benefit through review, reflection, and practice.
Other
To contribute positively and constructively to the development of the teams, service and NHYC. This will include:
Covering for other members of the team where necessary.
Following NHYC’s policies, procedures and performance expectations in all functions of the post.
