About the role
We are recruiting for two posts, one of which will be focussed on our Legacy Project in Blackpool.

You will plan and deliver casework to individuals and communities to resolve their housing situation and engage with community groups, local organisations and individuals to understand local housing issues and raise awareness of people’s rights.
The role also works in local community settings and with community groups to deliver advice and rights awareness workshops and make sure that people with lived experience of homelessness have opportunities to share their story, give their opinions and participate in the design and delivery of Shelter services. In the case of the longer term role, you will be working at least two days a week in community settings in Blackpool. Offering day to day support to volunteers, providing learning, shadowing and mentoring and being a consistent role model for our values are also key aspects of the role.
About you
You are able to listen to, engage and work with individuals and communities, including people with lived experience of homelessness. Role Context
General practice teams inPrimary Care Networks across Englandnow haveaccesstodedicatedLink Workers to provide the following functions:
1. To support patients with long term conditions to access non-clinical support in their communities with a view to reducing loneliness and increasing wellbeing activities
2. To support the clinical staff within the PCN to understand what services are available in their communities and how to access them.
The successful Link Worker candidates will work withinCitizens Advice on Prescription Liverpool service to provide allocated GP teams and a variety of statutory and non-statutory partner agencies with access to a person-focused service thatwill be deliveredin health centres, community health settings andinpeoples’ homes.
The Link Worker serviceis an essential part of theestablishedCitizens Advice Liverpool service andincreasestreatment options for health practitioners, and its highly motivated teamwork closely with health professionals andcommunity organisations to support patients to make lifestyle changes. The successful candidates will meet patients in a variety of settings and support them to develop a pathway to improved health and well-being. To provide this support you will need to develop a sound knowledge of local health, social and voluntary services.
This scheme will enable GPs to refer patients to social activities and other types of support to improve health and wellbeing. The new staff team will engage the patient and ‘co-design a non-clinicalsocial prescription serviceto improve their health and well-being using services provided by the voluntary and community sector’
TheLink Worker is responsible for enabling and supporting a patient to assess their needs, co-producing solutions for them making use of appropriate local resources.

Wintercomfort works with people who are homeless, at risk of homelessness or with a history of homelessness. We provide services to aid every stage of recovery - from immediate basic welfare needs to long term help in identifying and dealing with the problems which are undermining their stability, and enabling them to engage with education, employment and specialist health services. National statistics rank the numbers of rough sleepers in Cambridge within the highest 20 UK local authorities.
Wintercomfort is the only day-time service in the city, providing year-round advice and support for homeless or vulnerably housed people. Over the past three decades Wintercomfort has continued to grow and adapt to meet the needs of the homeless community.
Our specialist Housing Team provides expert advice and support to help people successfully navigate the search for housing and crucially, support them to sustain life away from the street.
About you and the Role
The Tenancy Sustainment Project Worker will provide targeted, practical and relational support to individuals who have recently secured accommodation or who are at risk of losing their tenancy. This is an outreach-focused role, working proactively with people in community settings, partner agencies, drop-ins and other accessible locations to maintain engagement and offer support where it is most appropriate.
The role may occasionally involve contact in or around accommodation settings, but this would usually be planned and supported alongside other services where appropriate. The role will focus on preventing repeat homelessness by helping people stabilise their housing, understand and manage the responsibilities of a tenancy, and build the confidence and skills needed for independent living. Working in trusted community settings and in partnership with statutory and voluntary sector services, the postholder will act as a critical bridge between vulnerable individuals and the support systems available to them.

We are looking for a New Christian Communities Development Officer to help grow new Christian communities across the Diocese. This is an exciting opportunity with an enormous amount of variety and autonomy as you will work alongside parishes, clergy, lay leaders and diocesan colleagues to encourage fresh approaches to mission, connecting with new worshipping communities, church planting, revitalisation and leading or supporting project boards to help shape the future of mission across the Diocese.
You’ll spend time listening to local communities, building trusted relationships and connecting people, ideas and resources to help new initiatives flourish. You’ll also play a key role in coordinating strategic projects, sharing learning and ensuring new communities are supported in ways that strengthen parish life and contribute to our diocesan vision.
