Job description

The Community Mental Health and Wellbeing Hub will sit alongside and compliment those provided by primary and secondary care ensuring that by working together anybody who needs a service sees the right person at the right time, through a single point of entry. Together, we’re making sure we’re walking alongside people who need support every step of the way. This delivers against the NHS’s vision for mental health care and support through more local, community mental health services and how community services should offer whole-person, whole-population health approaches.

If you are eager to make a positive impact and tackle meaningful challenges, we offer a cooperative and supportive working environment. You will have the chance to contribute to the development of care and support systems across the borough.

The role of a Mental Health Advisor has been specifically designed to provide wellbeing support to Bexley residents accessing the Community Mental Health and Wellbeing Hub. You can encourage, motivate and connect with people who access the service, working alongside the Multi-Disciplinary Team (MDT), to provide recovery orientated, solution focused, holistic care.

Key Responsibilities

  • To undertake focused one to one assessment with Bexley Residents in a range of settings
  • To complete appropriate and detailed documentation to support pathways,
  • To support the development and maintenance of professional links with external agencies, enabling effective signposting,
  • To support individuals to improve their own physical and mental wellbeing
  • To provide flexible support based on individual need and preferences
  • To enable individuals to make informed decisions and choices for their support and care pathways
  • To support the delivery and development of groups and brief interventions
