Are you passionate about supporting vulnerable adults during critical moments in their care journey? As a Hospital Care Navigator, you’ll be a vital part of ensuring patients leave the hospital safely and confidently, with the right support in place.

In this rewarding role, you’ll be the single point of contact for adult patients preparing for discharge from the hospital. You’ll work closely with the Adult Social Care team, health professionals, hospital practitioners, and community services to:

• Identify needs and barriers to safe discharge
• Achieve safe and timely patient discharge
• Coordinate outpatient appointments and follow-up services
• Provide clear, compassionate advice and support to patients and their families
• Ensure patients understand their discharge plan every step of the way.

You’ll meet with patients, their families, friends, or carers to assess ongoing needs, ensuring a safe plan is in place for their return home. This could include ensuring that medication is arranged from the hospital pharmacy, someone is at home to greet them, food and heating are available, and the condition of the home is safe to return to. You may be required to liaise with Adult Social Care and other home providers to coordinate home support. You’ll follow up on calls or visits to ensure the patient is managing well at home. By providing the proper support, patients can be discharged sooner, and the risk of readmission is significantly reduced. 

We’re seeking someone who:

• Has experience working with vulnerable adults in a hospital, GP or other healthcare setting
• Communicates with empathy and confidence across different teams at different levels, and with patients
• Is highly organised, with excellent time management skills
• Has awareness and respect for cultural differences
• (Bonus) Experience working in a hospital or clinical setting.

The role is based at our Southwark office, and you will be required to travel to hospitals and across the City of London, as well as to Tower Hamlets, Camden, Hackney, and Southwark.

Location:Hybrid working with some travel to Hearing Dogs offices in either Buckinghamshire or East Yorkshire.

Hearing Dogs for Deaf People, whose mission is build confidence, companionship and connection for people with hearing loss, is seeking a High Value Events Team Manager to lead the development and delivery of inclusive, inspiring and impactful high-value event experiences that deepen relationships and generate transformational philanthropic support for the charity.

Hearing Dogs for Deaf People has been creating life-changing partnerships between hearing dogs and deaf recipients since 1982. As well as acting as an ear to their partners and alerting them to sounds, the charity’s clever and expertly trained dogs help deaf people to live life with confidence and independence, whilst providing love and emotional support.

Following on from a strategic review, the charity is now building a new Income Generation Directorate, to enable them to transform many more lives across the UK. This role will be critical to help Hearing Dogs reach their goals to significantly grow and diversify income.

With a strong focus on values-led, relationship-based fundraising, this role will provide strategic leadership to the charity’s new High Value Events function, bringing a consistent, high-quality approach to engagement, cultivation, fundraising and stewardship events. The ideal candidate will be a strong leader with a background in planning and delivering high-value fundraising or engagement events generating £100k+. The CRM Impact Manager is a specialised role that will focus on developing our use of the Beacon CRM and ensuring it’s used to our best advantage throughout the charity.

London National Park City is looking for an organised and adaptable Administration Manager to join our small staff team. As our Administration Manager, you'll be the operational backbone of our organisation – managing our CRM and internal systems, maintaining policies and procedures, and ensuring our staff and 150+ volunteer Rangers have seamless access to the information and resources they need. London became the world's first National Park City in 2019, and we are a long-term grassroots movement connecting people and nature across the capital. • Provide regular updates to our staff team and Ranger community on your projects and activities, and overall progress on our programme goals.

• Contribution to half yearly progress reports and final project reports for external stakeholders and funders.

You will report to the Executive Director and work closely with your colleagues in our staff team and with our wider group of volunteers.

Candidate Profile

We’re looking for an organised, thoughtful and considerate colleague who will help us better support our Ranger community, and extend the reach and effectiveness of our staff team.

You will have the following experience:

• Experience in an equivalent administration or operations role, or demonstrable experience of transferable skills.

• Experience working with an online CRM system, such as Monday or Hubspot, as well as proficiency with Google Drive or other office applications.

• Demonstrable experience of supporting a small distributed staff team, and working with volunteers will be a distinct advantage.

• An ability to communicate and coordinate with a wide range of people at all levels of a variety of organisations.

• Excellent oral and written communication in a clear, concise, accessible and effective manner.

• Fluency in English, both spoken and written, is essential.

• Ability to effectively plan and prioritise workload within deadlines, and to set and meet high standards of delivery.

Salary and benefits

This is a permanent role and the salary range is £30,000 to £34,000 dependent on experience. This may be a part or full-time role and we are open to applications for flexible working arrangements.

About Worldwide Radiology (WWR)

Worldwide Radiology is a registered charity working internationally creating and delivering solutions to enhance access to high quality and appropriate diagnostic imaging tests in low- and middle-income countries. 

We’re a small but busy team, mainly based in the UK, working remotely across several countries and time zones. Our work involves close collaboration with international hospitals, universities, and global health partners, with a team of volunteers delivering most of our projects.  We’re looking for a Programme Manager to develop and drive forward our project portfolio. 

Overview

The Programme Manager role is crucial to the success and development of the charity. You will be responsible for the management and delivery of 6+ Worldwide Radiology projects, operating across Malawi, Ghana and The Gambia.

The Programme Manager will be responsible for quickly grasping the needs of our ongoing programmes, steering them toward desired impact, ensuring compliance with donor requirements, monitoring our impact and managing risks effectively to continue effective delivery across the organisation.

The successful candidate will be responsible for working with key stakeholders, including our volunteers, partners and donors to build and nurture strong working relationships. 

They will contribute to seeking out further grant opportunities within the sector, working closely with the rest of the programmes team and the fundraising team.

The key responsibilities of this post are as follows:

1. Oversee the smooth running of our projects; 

2. Ensure consistent and appropriate monitoring and evaluation;

3. Provide strong, holistic solutions, focused on operational leadership; and 

4. Support the overall growth and development of the organisation.

Role-Specific Responsibilities

1. Provide overall day-to-day management for WWR projects and WWR volunteers, reporting directly to the WWR Director.

2. Oversee the development and operational execution of detailed work-plans and budgets for WWR programme activities. 

3. Track all project milestones, risks and reporting schedules, taking timely action to ensure they are addressed.

4. Co-ordinate and deliver project MEAL activities in accordance with our MEAL framework. 

5. Produce (gather, collate and edit) high quality, timely project progress, annual and donor-specific reports.

6. Oversee the management of partnership agreements, data management requirements and contracts in accordance with donor specific and legal requirements.

7. Support the identification and securing of new grant opportunities in collaboration with the programmes and fundraising teams.

8. Communicate administrative and technical information to WWR project partners in a timely, sensitive and effective manner.

9. Assist team members, volunteers, partners and members of the public with queries, demonstrating tact and understanding when dealing with complex and serious issues, delivering a consistently high and appropriate level of service and support to individuals contacting WWR.

10. Assist with recruitment, onboarding, induction and management of both UK and international WWR volunteers.
     

Qualifications and Experience

Essential

• Educated to degree level or equivalent work experience.

• Demonstrable experience of adhering to principles, methods, techniques and tools for the effective management and administration of programmes and project lifecycles.

• Experience in grant or contract management in a humanitarian or development environment.

Purpose

Horatio’s Garden is a nationwide charity creating and nurturing beautifully designed gardens in NHS’s spinal injury centres.

The Patient Experience Manager will be instrumental in supporting patients and families use of the garden. 

Along with the Head Gardener, you will be responsible for the smooth running of a thriving, vibrant garden ensuring the patient and their family are appropriately supported throughout their time in the spinal unit. 

This role requires you to manage a team of volunteers and work closely with the Head Gardener who maintains the horticultural standard of the garden and delivers therapeutic horticulture sessions.  You will be managed by the National Patient Experience Lead.

You will be responsible for developing and maintaining relationships with clinical and non-clinical NHS staff, corporate sponsors, funders, the Fundraising team, the Communications team and the National Patient Experience Manager. 

The gardens have been beautifully designed and maintained, and the role of the garden needs to reflect an appropriate environment for patients and their families at all stages of their stay.  From the initial welcome, to ensuring the contact and relationship is consistent with activities to suit their needs.

You will be responsible for developing and delivering social activities, supported by the charity’s National Patient Experience Manager and in collaboration with the wider Spinal Injuries team in Cardiff. These activities will mainly need to be held at the weekend or in the afternoons which are critical times in the garden when most patients are available and often have friends and family to visit. Currently the Events programme includes seasonal celebrations and social and craft activities. External facilitators also deliver workshops and live music performances.

This is a fantastic opportunity for a confident, proactive individual who is efficient, well organised and has outstanding communication and relationship building skills. You will need to be adaptable and friendly and confident working with a wide range of people including people with spinal injuries and their families, NHS staff, volunteers and many different teams within the charity.

Main Duties and Responsibilities

People with spinal injuries

· Develop relationships with patients and families to ensure the best experience for them in the garden at every stage of their stay

· Maintain a consistent social and arts activity programme appropriate for the current patient group, liaising with the National Patient Experience Manager and managing the volunteers accordingly

· Organise a programme of seasonal events, reflecting the needs of patients

· Work with the Impact Manager to monitor patient experience and the impact of the garden

·       Ensure the garden is managed to allow the correct balance to be in place to benefit both patients and external parties (major donors, trusts, corporate groups and other visitors)

NHS staff

· Develop and maintain productive and collaborative relationships with NHS clinical teams and voluntary services

Volunteers

· Recruitment and training of new volunteers

· Manage the team of volunteers to ensure the smooth running of the garden with patient experience at its core

Fundraising and Communications

· Work with fundraising managers, Head Gardener and volunteers to develop opportunities to generate income in the garden through friends, corporate friends and third-party fundraising

· Support corporate sponsor events and fundraising opportunities

· Represent the garden and charity at key events

· Promote the local social media accounts and provide regular updates for content

· Interviews with press/media as required

Administrative

· Ensure all administrative tasks associated with the role are monitored, completed and maintained

· Manage the garden budget along with the Head Gardener

· Collate and bank donations and manage petty cash

· Assist with ensuring the garden room is kept clean and tidy at all times

Following the launch of this leading universities bold and ambitious strategy they are preparing to launch a comprehensive fundraising campaign. The Regular Giving and Legacy Giving team will play a pivotal role in bringing about a step-change in regular, mid-value and legacy donations to the University.
 
You will oversee and implement the overall strategy and supporter journey for alumni and donors primarily through direct mail, telephone, email and social media.
 
After an internal promotion, an exciting role has arisen with opportunities for the postholder to oversee an award-winning acquisition campaign, a brand-new mid-value programme and a successful legacy marketing programme.  You will be instrumental in building collaboration with the Alumni Engagement team to promote philanthropy and take alumni on a journey to their first gift.
 
This position is based in West London (Central Line) offering a hybrid working pattern of mostly 2 days a week which are Tuesdays and Thursdays, with some requirements to come in 3 days per week when needed.

Working across the Regular Giving & Legacy Giving team, you will oversee supporter journeys for donors, working on cases for support, appeal development and data segmentation. You will have the opportunity to pilot new ideas as well as delivering a mature and sector-leading programme.
 
Working across Advancement, you will build relationships with key internal stakeholders especially the Alumni Engagement team, Major Gift, and Event teams to capitalise on opportunities to embed regular giving and legacy messaging in all University activities.

Connect to Work is part of the National Get Britain Working strategy which is a new government funded programme. Within County Durham the initial programme is based upon the IPS model which looks to support people who would like to gain employment by delivering tailored support to move into the right job quickly and support to maintain employment.

The available role is a part time post (0.5 FTE – 17.5 hours per week) with days and times to be worked open to discussion and agreement with the successful candidate. This post is for a fixed term until 31st March 2028, with the possibility of an additional 2 year extension once funding is confirmed.
