Hope into Action is a national Christian charity that enables local churches to house people experiencing homelessness.

We are launching a new Hope into Action project in Milton Keynes, and are looking for a Church & Tenant Empowerment Worker to support tenants and church volunteers as the project begins.

This role focuses on relationship, encouragement, and practical support as people rebuild stability and independence.

About the role

At launch the project will involve one house with up to three tenants, supported by church volunteers and the Hope into Action network.

As Church & Tenant Empowerment Worker you will:

• Walk alongside tenants as they settle into their new home
• Support them in developing independence and life skills
• Help them access relevant services and appointments
• Encourage positive relationships within the house
• Support church volunteers who build friendships with tenants
• Maintain simple casework records and communication with the team

You will be supported by:

• a Location Lead
• Hope into Action UK systems and advisors
• trained church volunteers
• partner agencies

This means the role focuses primarily on people and relationships, rather than large caseloads or complex housing management.

About you

We are looking for someone who:

• Is motivated by their Christian faith
• Has compassion for people facing difficult life circumstances
• Builds trust easily and listens well
• Is organised and able to maintain clear boundaries
• Enjoys working collaboratively with volunteers and churches

Experience working with vulnerable adults, homelessness services, or community work would be helpful, but the most important qualities are empathy, resilience, and relational skills.

A role that can grow

As the Milton Keynes project grows and additional houses are opened, hours and responsibilities are expected to increase in line with the number of tenants supported.

PCN Cancer Care Coordinator

Clifton & The Meadows PCN 8, Nottingham (Hybrid)
30–37 hours per week | Permanent

Salary: £26,620 pro rata (pay award pending)

About the Role

We are excited to recruit a PCN Cancer Care Coordinator to support people living with cancer across the Clifton and The Meadows Primary Care Network in Nottingham.

This role is central to delivering a holistic, person‑centred cancer care coordination service, supporting patients beyond clinical treatment and ensuring their wider emotional, practical and social needs are met. Working closely with GP practices, Primary Care Network colleagues, and the voluntary and community sector, you will help improve experiences and outcomes for people affected by cancer.

You will work in direct partnership with Self Help UK’s Macmillan Beyond Diagnosis Service, carrying out Cancer Care Reviews and Holistic Needs Assessments (eHNA), developing care and support plans, and helping people to access the right support at the right time.

What You’ll Be Doing

• Supporting people newly diagnosed with cancer through Cancer Care Reviews and eHNAs
• Creating individualised care and support plans based on each person’s needs
• Acting as a single point of access for non‑clinical cancer support
• Referring and signposting patients to appropriate voluntary and community services
• Working closely with GP practices to ensure reviews are recorded on NHS systems
• Supporting work to increase cancer screening uptake, especially within targeted communities
• Building strong relationships across health, social care and the voluntary sector
• Ensuring safeguarding, confidentiality and data protection standards are upheld

You’ll work flexibly across GP practices, community settings, patients’ homes and the Self Help UK office in Nottingham, with some home working available.

About You

You will be someone who is compassionate, organised and confident working with people facing complex and emotive situations. You will bring:

• Experience in a people‑facing role within health, community or voluntary settings
• Experience supporting people with long‑term health conditions or vulnerabilities
• Strong communication, organisation and relationship‑building skills
• Confidence working both independently and as part of a multi‑disciplinary team
• Emotional resilience and a genuinely person‑centred approach

Experience of working with people affected by cancer, delivering holistic needs assessments, social prescribing or working across NHS–voluntary sector partnerships would be an advantage, but is not essential.

A full UK driving licence and access to a car is required for this role.

• Lead the day-to-day landlord function across supported housing services, keeping standards consistent and addressing issues before they escalate.

Safeguarding and Risk Management

• Take responsibility for ensuring housing management activity actively identifies and responds to safeguarding risks, including where concerns emerge through non-engagement, property condition, or repeated enforcement action.

Property, Estates and Void Management

• Oversee a planned programme of estate inspections, tenancy audits, and housing-led health and safety checks, ensuring actions are clearly recorded, owned, and followed up where standards fall short.

Compliance and Resident Safety

• Take day-to-day responsibility for housing-led compliance activity, working closely with Property and Compliance teams to make sure statutory requirements are met in practice, not just on paper.

Leadership and Team Management

• Line manage Housing Officers, setting clear expectations around performance, accountability, and professional conduct, and addressing issues promptly where standards slip.

Cross-Departmental Coordination

• Act as the main operational link between Housing, Property, Compliance, Care and Support, and Income teams, ensuring issues are addressed collectively rather than passed between services.

Customer Involvement and Tenant Experience

• Promote meaningful resident involvement in housing services, ensuring opportunities for engagement are accessible and appropriate to the needs of supported housing residents.

Performance, Quality and Assurance

• Use performance data, case audits, complaints, and service insight to identify emerging risks, recurring issues, and gaps in housing management practice.

What you'll do:

Building supportive, trusting relationships with customers who are currently in refuge to make the transition to permanent accommodation smoother.
Supporting community customers who have accessed the Sanctuary Scheme to remain in their properties and manage the risks associated with that.
Supporting key customers to set personalised goals in the form of a move on Support Plan
Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking.'
Ensuring customers have full access to any move on welfare support by completing move on paperwork prior to customers move on date.
Liaising and building partnerships with local charities to support customers with move on items
Accompany customers to property viewings.
Helping to set up utilities for move on customers
Working proactively with the refuge and IDVA teams and referring any clients/customers to the IDVAs where required.
Empowering customers to ensure they receive the service and benefits to which they are entitled
Proactive development of links with local statutory and voluntary organizations to provide a range of engagement opportunities for customers in the community
Develop links with private sector landlords, estate agents and other housing providers in order to source additional units and move on accommodation
Deliver domestic abuse training and awareness raising to housing teams and local landlords forums

We are seeking to recruit to a brand-new post and are looking for an experienced and strategically minded person to strengthen Salford CVS’s strategic VCSE leadership role within Salford.

Main purposes of the post

To provide strategic leadership for Salford CVS’s place-based work, ensuring the VCSE sector is influential, represented and embedded within key strategic partnerships, boards and system-wide initiatives across Salford. The postholder will work closely with the Senior Strategic Lead for People and Place to shape strategic agendas, strengthen cross-sector relationships and ensure VCSE voices help influence the city’s long-term plans relating to Pride in Place, regeneration, housing, social value, climate action, culture, and wider place-based priorities.
