Housing is one of the biggest factors affecting people's health and wellbeing. We are looking for two Community Wellbeing Coordinators to join our Community Connect Social Prescribing Service, with a specific focus on housing advocacy and support.This is an exciting opportunity to work at the intersection of health, housing and community support, helping residents navigate complex systems, understand their options and access the support they need. 

You will support residents whose wellbeing is affected by issues such as: 

• Housing insecurity 
• Risk of homelessness 
• Overcrowding 
• Disrepair 
• Temporary accommodation 
• Digital exclusion 
• Difficulties understanding housing processes and evidence requirements 

The role is about practical support, advocacy and navigation. You will work alongside residents to understand their circumstances, explore options, gather evidence, complete applications where appropriate and connect them with relevant services. 

We're looking for someone who: 

• Has experience supporting people in health, social care, housing, advice, voluntary sector or community settings. 
• Can build trusting relationships with residents facing difficult circumstances. 
• Understands how housing and other wider determinants impact health and wellbeing. 
• Is confident working with multiple agencies and coordinating support. 
• Can manage expectations sensitively whilst maintaining hope and motivation. 
• Has strong organisational and case management skills. 
• Understands safeguarding and person-centred practice
