We are looking for a warm, thoughtful and organised Dementia Navigator to join our supportive team in Bristol working 21 hours per week.
 
This is a community-based role, so you'll regularly travel across Bristol area to meet people in their homes, community venues and partner locations. As such, the ability to travel independently across the area is essential.
 
As a Dementia Navigator, you'll take a person-centred approach to help people understand their diagnosis, navigate health and social care services, access local support and feel more in control of their lives. You'll build trusted relationships with individuals, families and professionals, ensuring people receive the right support at the right time.
You will be part of a pro-active, caring and supportive local team who work in a fully integrated partnership with NHS colleagues, supporting people from point of diagnosis through to end of life.

About you

We're looking for someone who is passionate about supporting people and making a real difference.

- An understanding of the needs of people living with dementia and their carers.
- Experience of working with health, social care, voluntary or community organisations.
- Experience of assessing people's needs and providing person-centred support.

Site locations

The SERT region, covers the south east of England, extending from Hampshire in the west to Kent in the east, with the northern border being south of the Thames, extending down to the English Channel.

Our office is based in Leatherhead which you will be required to attend approximately four days a month.

The amount of travel required depends on the role, and can be extensive. Most travel for an individual will be to work on project sites, which can be remote, or attending external meetings within the SERT region. The geographical range and frequency of travel is role dependent.

To support travel costs, SERT pays mileage at the current HMRC amount for travel within the SERT region. We do not pay mileage, time or any other costs for travel outside the SERT region, nor for any commutes to the office. Toll charges cannot be claimed.

We are looking for a warm, thoughtful and organised Dementia Adviser to join our supportive team in Selby and Tadcaster working 25 hours per week, on a fixed term contract until 31st March 2027.

This is a community-based role involving regular travel across the area including local home visits, meetings, and direct support, with the flexibility to work from home based on business and community needs.

You will work closely with people living with dementia and those who care about them, providing ongoing support. You will listen carefully, understand what matters most to each person and offer guidance that helps them feel steadier and more confident as their situation changes.

Our Hospital Dementia Adviser provides vital support to people living with dementia during their hospital stay. You’ll work directly with individuals who have a dementia diagnosis and their families, helping them understand their situation, navigate hospital processes, and plan a safe and confident discharge home with the right support in place.

This is a hospital-based role that involves regular walking between wards and departments. You will work closely with the wider Local Services Team to ensure that people are smoothly connected to ongoing support in the community once they leave the hospital.

Although there is no direct personal or clinical care involved, this role is all about offering high-quality advice, guidance, and emotional support. You will collaborate with hospital staff, volunteers, and external partners to ensure that people affected by dementia receive the right information and feel supported throughout their journey.

We operate a triage system for new referrals and you will play your part in making sure people are responded to quickly and thoughtfully. You will manage your own caseload, keeping clear and accurate records, while always seeing the person behind the paperwork.
You will be part of a caring and dynamic local team who share ideas, reflect together and support one another. There will be time to meet as a team and opportunities to keep growing your knowledge of dementia, with your manager and colleagues alongside you.

About you 

We are looking for someone compassionate, organised, and confident working in a busy hospital environment.

- Have an understanding of dementia and the needs of people living with dementia (experience with other health conditions or support needs will also be considered).
- Be comfortable moving around a hospital sites at Hull Royal Information and Castle Hill Hospitals and engaging with people in a fast-paced setting.
- Be comfortable providing information, guidance, advice or emotional support - this could be from health, social care, community, voluntary, customer service or other people focused roles.
- You will be able to listen and communicate, with the ability to build trust and rapport with people from a wide range of backgrounds.
- Be able to manage your workload effectively, with appropriate support and tools, and to balance competing priorities.
- Confidence using IT systems to maintain accurate and confidential records.
- A compassionate, non-judgemental and person-centred approach, with respect for diversity and individual lived experiences.

- Facilitate and deliver regular peer support groups creating a safe, inclusive environment where people affected by dementia can share experiences, build connections, and access tailored information and guidance.

Education Against Discrimination brings Jewish & Muslim educators into the classroom to facilitate informal conversations with young people.​ By creating a safe space and through free interactive workshops, students are empowered to learn about and act against racism and discrimination with a specific focus on antisemitism and anti-Muslim hate. Young people are provided with the tools on how to report hate crime, whilst developing their sense of social responsibility to their local communities and British society as a whole.

The programme has reached over 150,000 young people across the country since its inception in 2017 and has received recognition from the Mayor of London, Sir Sadiq Khan, who funds the programme through the Shared Endeavour Fund. Workshops are delivered by expert facilitators who create a safe space for young people to debunk myths and misconceptions about the Muslim and Jewish communities.

As part of this role, you’ll help shape and deliver prospect strategy, insight and KPIs aligned to our fundraising priorities and long-term organisational growth plans.

As our Global Senior Prospect Research Manager, you’ll be responsible for (but not limited to):

• Prospect Research -Leading and strengthening the global prospect research function,ensuring it effectively supports ethical fundraising growth across key markets.
• High Potential Prospects-Identifying,researchingand prioritising high-potential prospects from Salesforce and other sources, working closely with colleagues in CRM & Insights.

About you 

To succeed in this role, you’ll need to bring strong prospect research experience, from an international fundraising team or comparable environment, alongside a good understanding of global philanthropy trends, donor behaviour and ethical fundraising practice. 

You’ll have to be confident in turning research into clear, useful insight for fundraisers, with the ability to build strong working relationships across teams and communicate findings in a way that supports action. You’ll also have excellent attention to detail and the ability to self-manage a varied workload in a busy, values-led environment.

Skills and experience you’ll need to bring as our Global Senior Prospect Research Manager:

• Prospect research experience within an international fundraising team or comparable environment.
• Experience building and managing a prospect pipeline that supports income growth.
• Experience using a wide variety of resources to produce high-quality intelligence for fundraisers.
• Experience of due diligence and applying ethical fundraising policies.
• Strong understanding of prospect research systems and processes, with experience using CRM systems, ideally Salesforce.

We are looking for a warm, thoughtful and organised Dementia Adviser to join our supportive team in hull working 35 hours per week, on a fixed term contract until 31st March 2027, working 8am - 4pm. 

You will work closely with people living with dementia and those who care about them, providing ongoing support. You will listen carefully, understand what matters most to each person and offer guidance that helps them feel steadier and more confident as their situation changes.

Our Hospital Dementia Adviser provides vital support to people living with dementia during their hospital stay. You’ll work directly with individuals who have a dementia diagnosis and their families, helping them understand their situation, navigate hospital processes, and plan a safe and confident discharge home with the right support in place.

This is a hospital-based role that involves regular walking between wards and departments. You will work closely with the wider Local Services Team to ensure that people are smoothly connected to ongoing support in the community once they leave the hospital.

Although there is no direct personal or clinical care involved, this role is all about offering high-quality advice, guidance, and emotional support. You will collaborate with hospital staff, volunteers, and external partners to ensure that people affected by dementia receive the right information and feel supported throughout their journey.

We operate a triage system for new referrals and you will play your part in making sure people are responded to quickly and thoughtfully. You will manage your own caseload, keeping clear and accurate records, while always seeing the person behind the paperwork.
You will be part of a caring and dynamic local team who share ideas, reflect together and support one another. There will be time to meet as a team and opportunities to keep growing your knowledge of dementia, with your manager and colleagues alongside you.

About you 

We are looking for someone compassionate, organised, and confident working in a busy hospital environment.

- Have an understanding of dementia and the needs of people living with dementia (experience with other health conditions or support needs will also be considered).
- Be comfortable moving around a hospital sites at Hull Royal Information and Castle Hill Hospitals and engaging with people in a fast-paced setting.
- Be comfortable providing information, guidance, advice or emotional support - this could be from health, social care, community, voluntary, customer service or other people focused roles.
- You will be able to listen and communicate, with the ability to build trust and rapport with people from a wide range of backgrounds.
- Be able to manage your workload effectively, with appropriate support and tools, and to balance competing priorities.
- Confidence using IT systems to maintain accurate and confidential records.
- A compassionate, non-judgemental and person-centred approach, with respect for diversity and individual lived experiences.

- Participate in the triage rota, ensuring new referrals are handled promptly, sensitively and equitably.
- Manage a defined caseload, maintaining accurate, respectful and confidential records.
- Signpost and connect individuals to appropriate local services and community networks, reducing isolation and increasing access to support.
- Work collaboratively within a supportive team environment, contributing to reflective practice and continuous improvement.
- Facilitate and deliver regular peer support groups creating a safe, inclusive environment where people affected by dementia can share experiences, build connections, and access tailored information and guidance.

About the role

Do you have a passion for delivering impactful environmental projects and restoring rivers for people and nature?

We are looking for an experienced Project Manager to join our Restoring Rivers & Catchments team. In this role, you will oversee a portfolio of large-scale environmental projects, typically valued between £300,000 and £1 million, working with key partners including water companies, the Environment Agency, local authorities and consultants.

From river restoration and habitat creation to fish passage improvements, floodplain reconnection and water quality schemes, you will play a vital role in ensuring projects are delivered successfully and achieve meaningful environmental outcomes.

Site locations

The SERT region, covers the south east of England, extending from Hampshire in the west to Kent in the east, with the northern border being south of the Thames, extending down to the English Channel.

Our office is based in Leatherhead which you will be required to attend up to 4 times a month.

The amount of travel required depends on the role, and can be extensive. Most travel for an individual will be to work on project sites, which can be remote, or attending external meetings within the SERT region. The geographical range and frequency of travel is role dependent.

To support travel costs, SERT pays mileage at the current HMRC amount for travel within the SERT region. We do not pay mileage, time or any other costs for travel outside the SERT region, nor for any commutes to the office. Toll charges cannot be claimed.
