About the Role

We are looking for a Triage and Referral Caseworker who will be an initial point of contact for people seeking support from Headway London. They will identify individuals’ needs, explore appropriate support options and help people access the services they need.

A key part of the role will be supporting people to access a Care Act needs assessment, working closely with local authorities to ensure their needs are understood and appropriately considered. The postholder will also help individuals access Headway London’s longer-term services, where appropriate, including supporting referrals and funding arrangements, or signposting and referring to external organisations.

This position will involve building strong relationships with local authority teams and developing a good understanding of Headway London’s services and referral processes. This will help ensure people are connected with the right support as quickly and effectively as possible.

Principal Duties and Responsibilities

Triage and Assessment 

• Guide new referrals through the care needs assessment process, ensuring that each person’s individual circumstances, needs and priorities are fully understood. 
• Undertaking initial assessments and triage appointments to identify presenting needs, support requirements and any immediate risks.

Referral Management

• Support the casework service, ensuring assessments and further referrals and signposts are completed in a timely manner.
• Maintain regular contact with individuals awaiting support.

Referrals and Signposting

• Support individuals to access appropriate services, including Adult Social Care Needs Assessments, welfare support, housing, health and community-based provision.
• Provide signposting and referrals to relevant internal services.

Fee-Based Services Development

• Identify individuals whose needs may be appropriately supported through fee based services Discuss available options with individuals and families in a sensitive, person-centred way.
• Facilitate referrals where appropriate and contribute to developing sustainable routes into both funded and fee-based provision.

Partnership Working

• Develop positive working relationships with local authorities, health services, hospitals and community organisations tosupport improved access for people living with brain injury.
• Promote awareness of available services among external professionals and contribute to relevant partnership meetings where appropriate.

 Monitoring and Reporting

• Maintain accurate and timely records of activity. 
• Maintain monitoring information of referrals, outcomes and the progress of waiting list reduction.

Internal: Members and their families and carers, Headway London Teams and services and Volunteers.

External: Adult Social Care teams, Hospitals and rehabilitation services, Community health services, Housing providers, Welfare benefits services, Community and voluntary organisations, Commissioners and funding partners.
