Age UK Camden Dementia Wellbeing Service is looking to recruit a Dementia Wellbeing Worker to join their team working 35 hours per week on a permanent basis.
The Dementia Wellbeing Service is part of our specialist dementia services and works closely with the Dementia Befriending Service.
The service supports Camden residents over the age of 55 in the early or moderate stages of dementia, to live independently and safely at home for as long as possible, stay socially engaged and carry on doing what they love.
The successful candidate will:

Offer a holistic and person-centered assessment for people living with mild cognitive impairment or Dementia to ensure meaningful interventions.

Manage a case load of clients, providing practical and emotional support to the most isolated and vulnerable in our community.

Provide information, advice and support to carers (family/friends), health/social care professionals and other voluntary organisations and to disseminate knowledge and best practice so that individuals living with cognitive impairment in relation to dementia or undiagnosed dementia have the best care possible.

Maintain accurate and appropriate records via Charity Log database and in accordance with UKGDPR.
Salary: £33,525.36 to £34,743.24 pa incl. London Weighting.
Hours: 35 per week
