JOB TITLE: Community Mental Health Key Worker

INTRODUCTION

An exciting opportunity to work with the Islington Core community mental health multidisciplinary/multiagency team, which integrates services across Health, Social Care and the Voluntary & Community Sector.

The North London NHS Foundation Trust provides mental health and social care services, across five Boroughs. The Keyworker role will be based within the Islington Core Teams. The Core Team’s vision is to provide integrated and person-centred care and to work together in partnership with primary care, the Voluntary & Community Sector (VCS), social care, communities, service users, their carers and families to support and improve mental and physical health for the whole population through a combination of prevention, supporting people to stay well and responsiveness to changes in need.

The service provides five keyworkers to work across the localities in Islington. Three keyworkers are employed by Age UK Islington and two keyworkers by MIND Islington. 

Age UK Islington and Islington MIND are both independent charities, Age UK Islington provides a range of one-to-one personalised support and Islington MIND offer a range of structured and open access mental health interventions including our peer led bespoke projects.

ABOUT THE ROLE:

We are looking for a key worker who can:

·     Work in a structured way using support plans with clear goal focussed objectives.

·     Improve service user experience and outcomes.

·     Ensure people with mental health issues have improved access to mental health support.

·     Provide care and support that is holistic and person-centred, truly orientated towards the promotion and maximisation of individuals’ health, wellbeing and independence.

·     Reduce health inequalities often faced by people with mental health issues.

·     Integrate the community service approach across secondary mental health services, the VCS, primary care, social care, community assets as well as other physical healthcare provision.

KEY SKILLS:

You will be able to build effective relationships with people affected by mental health issues, be flexible in the method of support, ensure that a personalised approach is maintained. Be able to work as part of a multi-disciplinary team, develop partnerships with key organisations and play a key role to community mental health services.

ABOUT YOU:

You will have previous experience of working in similar role or related setting and in working in a person-centred way.

You will have responsibility for managing a caseload, triaging referrals, case management, connecting people to specialist services or community services to promote independent healthy living.

The role involves working with people who will be experiencing complex issues so you will need good listening and communication skills.

You will have excellent IT and organisational skills, be able to adapt to changing demands and priorities, be solution focussed, decisive whilst remaining service focussed.  For more information see the full job description and person specification.
