Job description

As a Link Worker, you will support and empower adults across Surrey to improve their mental health and wellbeing. You will provide person-centred one-to-one support, assessments, structured interventions and onward referrals, helping people identify their goals, make positive changes and connect with community activities and services.

 Location: Woking, supporting individuals across Guildford and Waverley, Surrey Heath and Farnham, and North West Surrey

Key responsibilities:

·        Support clients to identify their needs and goals and make positive changes to their mental wellbeing.

·        Complete assessments, risk assessments, support plans, reviews and onward referrals where appropriate.

·        Use the Recovery Star with clients, review progress regularly and record outcomes.

·        Deliver motivational and structured psychosocial interventions that promote health and wellbeing.

·        Maintain prompt, accurate and professional electronic and paper records in line with Catalyst policies.

·        Connect clients with partner agencies, community organisations, groups and mainstream activities that meet their needs and reduce social isolation.

·        Report incidents promptly and follow Catalyst policies and procedures.

·        Work collaboratively with Group Workers and facilitate or support group activities where required.

·        Act as an ambassador for clients and Catalyst.

About you

Essential:

·        At least one year’s community-based experience supporting adults with mental health, wellbeing or behaviour change.

·        Knowledge and understanding of mental health issues, safeguarding, confidentiality, consent, information sharing and GDPR.

·        Strong communication and written skills, with a commitment to accurate and confidential record keeping.

·        Ability to engage effectively with clients, colleagues and other professionals while maintaining clear professional boundaries.

·        Knowledge and skills in engagement, motivational and behaviour-change approaches.

·        Understanding of community and lone working, with the ability to maintain personal safety.

·        Ability to manage challenging behaviour and recognise when to seek advice and support.

·        Ability to use supervision effectively and work in line with service policies, procedures, standards and partnership arrangements.

·        Proficiency in Microsoft Word, Excel and Outlook, and the ability to enter information onto service and commissioner databases.

·        Ability to travel between different locations regularly and work flexibly across operational hours, including evenings and weekends where required.

Desirable:

·       A diploma or degree in a relevant subject, such as counselling, psychology, social work, probation, mental health or Level 3 Health and Social Care.
