About the role:

The Housing First Worker is responsible for coordinating and providing a personalised, trauma informed support service to people with complex needs facing multiple exclusions in line with Housing First principles to create long term sustainable tenancies by increasing independence and maximising connectivity with the local community.

Main Responsibilities Of The Job:

• Deliver wrap-around intensive, trauma informed support to people with multiple and complex issues within their own independent tenancies.
• Maintain accurate up to date records of progress against individual support plans, safety plans and outcome information. • Liaise with other agencies to facilitate access to support.
• Regularly review the person-centred support plan to update, record and monitor progress of caseload.
• Build and maintain networks with staff from other agencies to ensure the Housing First pilot is widely known and promoted.
• Adhere to good practice safeguarding procedures with a positive risk-taking approach to ensure the wellbeing of vulnerable people.
• Liaise with landlords to assist the person living in accommodation of their choice, including practical help with furniture, benefits advice and accompanied visits.
• Perform robust assessments of referrals for the service to ensure the service is provided to those that meet the eligibility criteria.
• Promote, and encourage peer involvement within the service.
• Complete dynamic risk assessments and maintain own safety by following lone working procedures.
• Comply with Homeless Oxfordshire’s policies and procedures.
• Contribute to a positive working environment in which equality and diversity are valued and staff are enabled to do their best.
• Contribute to the work of the broader team.

Join Single Homeless Project (SHP) as a Senior Outreach Worker and take a leading role in our response to rough sleeping across Redbridge. This is frontline work with purpose - combining assertive outreach, strong local partnerships and trauma-informed practice to help people move away from the streets for good. As a Senior Outreach Worker, you’ll lead outreach shifts across the borough, offering direction, reassurance and momentum to a team of Outreach Workers while holding a focused caseload of people facing complex challenges. Working in close partnership with Navigators, you’ll help ensure each person’s journey off the streets is joined-up, consistent and grounded in trust. The position is a dual role. You’ll join as one of three dedicated Youth Workers and will lead on a portion of Free to Be Kids’ year-round follow-on support for vulnerable young people who we have already engaged via our Thrive Outside residentials. Crucially, a core aspect of the role will be leading an average of 8-10 of our residential projects across each year - working on the ground with the children, leading the volunteers, and helping make the projects special, safe, and transformative for vulnerable young people, many of whom have complex needs. You’ll be joining a small staff team of 9, supplemented by a vibrant and committed group of volunteers.

Attendance at approximately 8-10 five-six night residential projects per year outside London is a core component of this role. A full driving licence is not required but may be an advantage.

On offer to the right candidate:

Salary: Circa £29,000-£32,000, dependent on experience.

Hours: Fulltime: 37.5 hours per week. Essential attendance on at c.8-10 residential projects a year.

Leave: 25 days per year, plus bank holidays and up to 13 additional pre-set discretionary reward days in September and during the Christmas period.

Other benefits: 4% employer pension contribution paid on full base salary, rising by 1% per year of service, up to a maximum of 7%, hybrid working, office with great transport links, flexible working (outside core hours), employee reward scheme, a tight-knit effective team with great morale.

Based: Hybrid – our small office in Waterloo, London. Hybrid working is well established with the majority of the team splitting time around 50:50 between the office and home.

Who we’re looking for:

Our next Youth Work hero. You'll be fun, active, energised, resilient, and experienced in work with young people with complex needs, and looking to join an organisation you can believe in and who in return will believe and invest in you.

You’ll have multi years’ experience working alongside young people who might struggle in other settings and will be used to dealing with a range of behaviours and complex support needs, ideally including mental health support needs,with a good understanding of safeguarding. A basic level of comfortableness in the outdoors, resilience to keep going with a smile when it's wet outside, and the silliness to be the first to jump into the lake will see you in good stead for this role.

You’ll have the flexibility to work away from home on up to 10 week-long residential projects per year. Ideally, you’ll also have experience of leading projects for children – whether residential or non-residential. Experience of working on or leading residentials projects themselves is not required but may be an advantage.

We are a small, vibrant, fun and friendly staff team of 9, growing to 11 when these 2 Youth Worker vacancies are filled, supported by a committed volunteer base of around 200.

Support Assessment & Delivery

• Implement and deliver dual-diagnosis support through 1.1 and group sessions to tenants experiencing issues with poor mental health and substance misuse.
• Undertake joint holistic assessments and risk assessments with tenants participating in the service to determine service and support needs and identity where these can be met.
• Provide outcome-based support measured against goals, personal aspirations of the tenant, and their achievements.
• Working with tenants, encourage them to identify their own goals and work towards independence and resilience. The key aims are to reduce the need for acute clinical interventions and hospitalisations.
• Put in place with the tenant, agreed SMART targets with a range of evidence-based interventions that addresses areas of need, such as harm minimisation, prevention of relapse for those in addiction recovery, and improved self-awareness and motivation.
• Through collaborative working with other teams in the organisation, enhance the personalised and choice-led support plan of tenants to integrate with their tenancy support needs and reducing the risk of tenancy loss due to dual-diagnosis issues.
• Help reduce episodes of crisis though support initiatives such as peer-led support groups.
• Be proactive in observing and responding to changing characteristics of tenants receiving support, recognising deterioration in mental wellbeing and putting in place interventions that reduce the likelihood of more acute support needs.
• Be a point of contact for tenants who are on the verge of experiencing a crisis episode, providing appropriate support and interventions to help avoid the need to access acute services.
• Foster positive working relationships with external stakeholders, breaking down barriers and improving access to appropriate support services such as specialised mental healthcare and addiction recovery programmes.
• Develop effective community links that expands support resources that to be offered to tenants.
• Develop all support plans and coaching sessions to embrace the key elements and principles of Trauma Informed Practice and Psychologically Informed Environments.
• Keep up to date with strategies and methodologies used in the support sector to ensure dual-diagnosis support in the organisation remains at a high level for all tenants accessing it.
• Promote the achievements of tenants receiving support through KPI reporting and case studies.
• Recognise goals and aspirations achieved by individuals and using the evidence obtained, formulate and develop future support plans and strategies that enhance the service.
• Work with team leaders and managers to develop our lived experience support in a structured and safe way.
• Deliver practical harm reduction, relapse prevention, and coping strategy workshops.

Risk Management

• Ensure accurate and up to date risk assessments are undertaken, putting in place effective risk management plans to mitigate risk of harm, neglect, and any other areas of risk to the tenant, staff, and wider community.
•  Ensure all records and support plans are managed in compliance with organisational policies governing confidentiality, privacy, data sharing consent, and keeping to the requirements and principles of the UK GDPR.

General Responsibilities

• Support colleagues through the sharing of support strategies and techniques that will benefit tenants outside of dual-diagnosis sessions.
• Foster positive and effective working relationships with colleagues in the Housing and Support teams.

You will need to form links with partner organisations and agencies and your role will be fundamental in supporting someone on their journey through Housing First, supporting five[TG1]  clients at any one time.

About you

You will need experience of support work, a proven ability to listen to, engage and work with individuals and communicate effectively with a variety of stakeholders, especially substance misuse services, health and probation and people with lived experience of homelessness. You will have an awareness of working in a trauma informed, strengths based approach to help people with multiple disadvantage, as well as having experience of writing and implementing person centred support plans and writing detailed case-notes.  Also, you have flexible time management skills and are comfortable collaborating with people from other teams and organisations.

Job Purpose

• The hospital to home service works collaboratively with hospitals to support people
when they are discharged from hospital to return home. As a Hospital to Home
Outreach Worker you will provide temporary practical and emotional support to service
users within their home and make sure they understand the care and treatment choices
available to them. The service is time limited, usually up to 4 or 6 weeks, to adjust and
settle back in their homes.

Key Tasks

• Assess the needs of service users either in hospital before discharge, or in their homes,
and understand what barriers and enablers there might be to moving home.
• Aid the discharge process and improve patient experience through tasks such as:
• Key cutting
• Co-ordinating with colleagues for the setup of the home environment
• Checking heating, electric and gas is functional prior to discharge
• Allowing access to the property for cleaning/repairs
• Provide (and/or arrange) the practical and emotional support needed to enable
service users to move home and/or remain safely at home.
• Be vigilant and make referrals/signpost for additional needs that our wider team, or
partners can support. E.g. for benefit checks, energy advice, carers support,
befriending.
• Help maximise service user independence by adopting an enabling approach.
• Develop a listening and caring relationship with the service user and their
families/carers.
• Provide domestic practical support within the home such as:
• Light cleaning,
• Washing (e.g. bedding),
• Changing bed sheets etc.
• Prepare and service drinks and simple meals with or for the service user customer
ensuring nutritional needs are met.
• Accompany service user outside the home e.g. hospital/GP appointments etc.
• Support customers with other tasks e.g. making telephone calls on their behalf,
reading and responding to correspondence, completing simple forms (training will be
provided) as directed by the customer etc.
• Ensure services provided by AUKEL in the community are safe and person centred.
• Report safeguarding concerns, in accordance with AUKEL safeguarding procedures.
• Work within AUKEL’s expectations of professional boundaries and confidentiality.
• Provide cover in the case of sickness and annual leave periods of colleagues.
• Support service user reviews, ensuring all visiting information is uploaded on to
AUKEL’s organisational case management system.
• Meet with volunteers to provide support and guidance.

Administration

• Input all records, reviews, and visiting information to service user’s case notes in a
timely manner to maintain up-to-date and accurate records on AUKEL’s case
management system (currently Charity Log/Call-round App).
• Report all “no access” failed visits, or changes in service users’ condition or
circumstances in accordance with AUKEL’s policies and procedures.
• Ensure service user comment sheets are completed.
• Support the return of customer comment sheets and general feedback in respect of
your own service users.

Quality

• Ensure services provided in the community are delivered in line with the Care Quality
Commission (CQC) principles.
• Deliver all work in line with AUKEL quality mark standards.
• Commit to undertaking the Care Certificate, if not already held or in possession of NVQ
level 2.
• Complete essential training as required by AUKEL to provide safe services.

Liaison

• Work in collaboration with other agencies providing support within the customer's
home.
• Work under the direction of the service manager and project officer/s.
• Represent AUKEL and participate in appropriate external meetings and events to

The Role

As Youth Work Coordinator, you will take a lead role in the planning, coordination and delivery of youth clubs and activities. You will be responsible for setting up and developing new sessions, coordinating day-to-day delivery, supporting youth workers and volunteers, and ensuring high-quality youth work practice in line with the YMCA Youth Work Strategy and Principles.

The role includes working 2–3 days per week after school, with some evening work required. Additional daytime hours are allocated for planning, coordination, partnership working, and administration, bringing the total to 37.5 hours per week.

BACKGROUND

Church of England Birmingham (CofEB) is on a journey of growing churches at the heart of every community. Through this we are seeing growth in many exciting ways. Financially we are at a challenging point. We have had an operational deficit for a number of years and this cannot continue. The National Church has agreed to partner with us around this through their Diocesan Investment Programme enabling us to have the time, and resource, to tackle this head on. One key aspect of this is for us to increase our Common Fund returns (the giving from our parishes that supports the costs of our life together). We are taking a radical new approach to our finances which includes intentional reallocation of income to support the costs in our most economically deprived parishes and deeper relationships with each parish around finance through conversation rather than correspondence. Over the next five years we are hoping to be able to move to a financially sustainable platform which will enable us to better serve our communities in the future.

We have recently recruited a new Head of Generosity (who is also Bishops Advisor for Common Fund) to lead on this work and we are now looking to recruit the team to work with them. The focus of the team is to develop long term relationships with all of our parishes leading to:

• Greater support for our parishes.
• Increased generosity by, and within, our parishes.
• Increased Common Fund.

The current plan is to recruit two Giving & Generosity Advisor roles and one Giving & Generosity Support Officer role. Together this team will work with all of our parishes to roll out our financial approach, through in person meetings, and the support that is on offer as part of this. 

JOB DESCRIPTION

The primary focus of this role is to support the wider team in delivering this project. We have 146 parishes and the Head of Generosity will manage the team so that all parishes can be engaged as quickly as possible. It is the Head of Generosity and two Giving & Generosity Advisors who will be carrying out the in-person meetings with each parish. Those roles will work closely with a range of people, and teams, across Church of England Birmingham so that this work is integrated within our wider strategy and parishes are not confused by multiple disjointed initiatives. This will mean working with Archdeacons and Area Deans, Mission Support Team, Ministry Team, Property Team, Finance Team, Community Regeneration Team and Communications Team as well as others.

This role will manage the processes that enable the team to deliver and flourish.

Key responsibilities will include:

• Being the point of contact for parishes and other DBF teams as they will be the member of the team who are predominantly office based. 

• Develop strong relationships with parishes, and other key stakeholders, to enable the establishment of meetings, support around Generosity etc.
• Be the first point of contact for parishes when they have questions around Giving and Generosity.
• Manage activities and processes that enable the project to function e.g. a scheme to loan out contact giving devices.
• Support in the creation of resources and communication pieces.
• Keep the teams external, and internal, communication streams up to date (including website, and praying together). Ensuring that the team has a presence in all key CofEB channels.
• Encourage and assist parishes in the implementation of the Parish Giving Scheme, introduction of contactless giving mechanisms and related diocesan resources.
• Support the scheduling and support for training. This will be for clergy, treasurers and others.

• Manage the collation of data and information so progress of the team is clear and evidenced.
• Undertake resource projects.
• Support the Head of Generosity to respond to new and emerging needs and opportunities.
• Ensure a regular flow of team meetings and communication between the team.

• Coordinate team wellbeing activities in line with agreed pattern and linked to the wider CofEB rhythms.

• Some understanding of church governance structures and financial operations and requirements.

TERMS AND CONDITIONS

The post-holder will be employed by the Diocesan Board of Finance for a fixed term, ending on 2nd November 2030, subject to funding.

Salary and Pension: Salary of £28,000 plus membership of the Church Worker’s Pension Scheme with 12% employer contribution. We are open to discussion especially if you are ordained and are moving out of parish ministry. 

Hours: Full-time 35 hours (5 days) per week. Weekend and evening work may be required, for which the equivalent time may be taken back from standard working hours. We are happy to consider requests for flexible working and candidates seeking part-time hours.
