This exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.
Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.
The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be expected to work across all 3 services .
- 7 day rolling rota including early, mid and late shifts
- (K&C): medium – high support accommodation.
- (Lambeth): medium support accommodation.
- (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.
Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.
In these roles you will:

Work as part of a busy team environment working closely with those affected by rough sleeping.

Provide a safe and supportive environment at night to clients; and support a team of Assessment and Reconnection Workers, with key tasks; such as implementing reconnection or assessment plans and supporting clients into appropriate accommodation.

Windsor Road service offers 11 beds to people experiencing mental ill health. We are a CQC registered residential care home. We offer individual one to one interventions and sessions such as social anxiety, anxiety, finance and budgeting, positive self-image, building self-esteem, support with diet and nutrition, healthy eating, sleep hygiene, DBT skills, building resilience, life skills, solution-based therapy, stress management etc. We offer a range of activities in our service planner including a daily coffee morning, a ‘here and now’ group, community gardening, cooking, arts and crafts, social inclusion and family and friends’ days. We support our residents with daily living skills where this is needed, such as support with shopping and cleaning tasks, this could include prompting or supporting alongside them. We also work closely with other agencies and our residents wider care teams, so good communication skills are required. And of course, all of this is recorded on our information management systems, so good record keeping and documentation is also essential. Our service consists of 8 24/7 Recovery Workers, a Service Manager and Registered Manager.

Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.
The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.
Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.
This role consists of consists of day time working patterns including early shifts from 8am and late shifts from 2pm. Shifts are 8 hours long and 5 days a week.

“For My Future” is a bespoke 6-week course designed in 2019 specifically for people in the justice system. Focusing on both personal skills and practical ones (CVs, applications, interviews), the course is delivered either in person or online to small, single-sex cohorts over a 6-week period. To succeed in this role, you will have experience of supporting people in group scenarios and be confident presenting both in person and online. As a Support Workers, you will play a key role in supporting our residents to help them to enhance life skills and increase independence. A wide range of co-produced activities take place across the service, designed to empower and improve the daily living skills of customers.
Support Workers in the service work across all 4 projects in Hammersmith and Fulham; working with customers with varying levels of support needs. Many of the customers have complex needs including dual diagnosis and Support Workers are expected to liaise with other agencies within the borough in order to best support customers. Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions

About the Role:
Oaklands & St Kilda is a pair of 6-unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.
The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.
Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.
This is a Waking Night role working to support young people throughout the night at the service. This role completed 10 hour shifts 4 days a week across all days of the week.
