NHS England recently announced a new transformation fund to be allocated to pilot sites within 12 Sustainability and Transformation Partnerships/Integrated Care Systems across the country to test new and integrated models of primary and community mental health care. The role of Community Connector is fundamental to the developing of these innovative new teams and mental health services based within networks of GP Practices (Primary Care Networks). Indeed, whoever takes on this particular challenge will be instrumental in supporting the ongoing development and mobilisation of integrated primary care mental health services in the future. Along the way, you’ll work jointly with a Mental Health Practitioner to conduct assessments. We’ll also rely on you to help patients identify their socially determined needs and goals, provide self-management tools, and develop personal support plans. Delivering a range of motivational and structured psycho-social interventions, promoting independence through an enabling asset based approach and developing effective relationships with a range of agencies to facilitate a ‘joined up’ approach - these are just some aspects of this vital role.

As well as proven relevant experience, you’ll need an appropriate diploma or degree i.e., in counselling, psychology, social work, probation, mental health, Health and Social Care (level 3), NVQ’s level 3+ and/or community experience of drug, alcohol, mental health work. You’ll also need the ability to work co-operatively and flexible as part of a multidisciplinary team from a service hub in one of three locations and be willing to travel to and from a number of different locations on a daily basis. Comfortable working to the confidentiality, consent, information sharing and safeguarding policies of the integrated service, the ability to manage any challenging behaviour, anger and verbal aggression from patients is important too. And, it goes without saying that you have excellent communication skills and are proficient in the use of office IT.

Programme Manager – National Clinical Audit of Psychosis

Within the College Centre for Quality Improvement, we run an exciting programme of NHS-funded national clinical audits, including the National Clinical Audit of Psychosis, which provides important data that is driving forward improvements in care for people with first episode psychosis. We are seeking a dynamic and proactive person for the Programme Manager (maternity cover) post. 

The successful candidate will have experience in project management and quality improvement in healthcare and/or clinical audit and will have a keen interest in data analytics. They will show attention to detail and have excellent organisational, report writing and communication skills. Responsibilities will include managing a complex programme of work, liaising with funders and expert advisors, supervising the project team, developing and delivering of project plans, and managing all aspects of the project, e.g. developing data collection tools and collecting and analysing data.

Background:

First Step Trust (FST) is a charity that provides work experience, training and employment opportunities for people excluded from working life because of mental health problems or other disadvantages (workforce members).

Our approach is about giving workforce members the chance to overcome some of the difficulties facing them by getting them involved as colleagues. We provide ‘on the job’ training, advice and support to enable each person to improve their basic skills and knowledge of the various work undertaken. This will include supporting workforce members to gain recognised qualifications and access to work placements with our customers and other organisations.

We are in the process of developing a more formalised training and employment support programme (SMaRT Pathways) particularly, but not exclusively, in the area of the Automotive trade.  SMaRT Pathways will include the delivery of vocational training through the use of various technologies including video, Virtual Reality, Augmented Reality and other mediums making the learning process more accessible to those who struggle with traditional learning methods due to issues around literacy, numeracy and low levels of self-confidence/anxiety. 

Job Role:

The main objective of this post is to enable people with mental health difficulties and other disadvantages (workforce members) to engage with and access open employment.  The EDTW will achieve this by taking responsibility for proactively recruiting people to the project, overseeing their development whilst at the project, including delivery of training courses as and when required and supporting them to move on to employment or further training.

Main duties:

Networking with other services in the area to raise awareness about FST, promoting work placements and training courses as well as promoting the business services.

Recruitment and development of potential workforce members - being proactive in following up on enquires regarding work placements, interviewing potential workforce for work placements at the project.

Employment support – assist workforce looking to move on to paid employment by providing support in job search, CVs and interview preparation. Job Purpose

This role sits within our new mental health Prevention Hub - which is a drop-in service providing a safe and welcoming space for people who are feeling distressed, seeking support and advice for their mental health, there and then. This service is a 9 month pilot project and will initially operate 11am-7pm Monday-Friday, however there may be a need to cover weekends/late nights, depending on the need of our clients and service growth. 

Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone on a one-to-one or group basis to individuals experiencing mental health distress. This will consist of 1:1 support; de-escalating and providing interventions, signposting for specialised or longer-term support, and delivering collaborative self-management plans with clients to improve mental wellbeing. The aim of this service is to de-escalate, and provide holistic wellbeing plans. 

The objectives of the service include:

• To improve the mental wellbeing of people experiencing mental health distress, in Spelthorne
• To provide support to clients accessing the service- for instance: signposting, de-escalation, planned wellbeing activities etc. 
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To treat service users with respect, dignity and personalised support
• To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation
• To increase self-management skills of those accessing the service
• To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis.
• To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual.

The Role

The role of the Prevention Hub Coordinator will be to support the rest of the team in delivering interventions on a one-to-one and group basis. The role will also involve triaging and assessing all clients accessing the service, signposting and delivering collaborative self-management plans with clients to improve mental wellbeing. The role will involve supporting a team of volunteers in delivering interventions.

Coordinating the curation, planning, scheduling and delivery of community projects, activities and events at the Likewise Wellbeing Hub and Garden

Coordinating the Likewise Creative Therapies service referrals and delivery

Making good use of our community facilities, garden and kitchen to support wellbeing in the community. 

Communication, Information and Relationship

Working with relationship building, conflict, rupture, and repair with, and between, community members 

Being a point of contact with community members and activity hosts

Coordinating placement student team to manage communication channels for the community, including social media, digital content and website updates

Working in collaboration and partnership with our Programme Lead, Learning Programme Coordinator and Office/Hub Administrator to realise the full potential of our Community Programme 

Community Engagement

Increasing access and engagement with the Hub from a diverse community through partnerships, informal and formal referral networks

Working with the Programme Lead to build engagement with the community programme through programming, marketing and communications

Developing and building on the participation, voice, and sense of ownership of community members in the Community Hub 

Learning, Monitoring, Evaluation

Ensuring accurate and useful data collection for the Community Programme

Maintaining relationships with a wide range of stakeholders including partner charities, volunteers, Camden Council and community members.

We are looking for a full time Peer Engagement Practitioner.

This is an innovative new service, integrating the expertise, skills and knowledge of the network in Barnet with NHS Mental Health services. The purpose of the role is to provide peer led psycho-social support to clients, working within specialist Mental Health teams and the VCS delivery partners. This role will also serve to enhance the work of the MH teams, and other practitioners within the service, to ensure it remains clients focused, recovery informed, and embedded with experience led practice.

The role will carry an active caseload, working with clients to determine their recovery needs, identifying resources available within the community, providing expert advice and support, facilitating recovery focused peer support groups, and contributing to the development of new community resources.

We need someone like you to join the team at our Hammersmith & Fulham Employment Service. We support people living with mental health problems to develop a sense of independence, purpose and fulfilment; to develop their social networks and gain the skills they need to return to work or training where this is their goal. But we need your help. Your challenge? We help individuals with mental health problems to find paid employment, or unpaid voluntary work, plus we assist employers and employees to successfully resolve mental health problems in the workplace.

You will support the delivery of our Employment Service which will see you focus on motivating, supporting and providing person-centred advice and guidance to people receiving support to enable them to move into suitable and sustainable work.

As well as a good understanding of people with mental health problems, you have experience of networking/liaising with local employers and the initiative to develop and promote a service. Excellent motivational, presentation and listening skills are essential too, as is a willingness to travel in the local area and occasionally work outside of office hours. Customer focused, well organised and collaborative in approach, you are a great administrator and pride yourself on your word processing skills and ability to keep accurate computer records/use a database. A flexible approach to working to ensure effective service delivery is essential.

This is a full-time role requiring the post holder to work 37.5 hours per week.

From 1st June 2024 Richmond Fellowship will be merging with the mental health charity Humankind to form a single organisation that provides the joined-up mental health, housing and drug and alcohol support we’ve all known has been needed for decades. In October 2024, Humankind will then be renamed to reflect the new, bigger and better organisation.
